[1], which was a very interesting study. In their very well-designed and presented study, Gunes et al. [1] tried to assess the potential relationship between hematologic parameters and testicular torsion (TT). They showed significant differences in the neutrophil-to-lymphocyte ratio (NLR), platelet-to-lymphocyte ratio, and platelet count between TT patients and healthy controls. There was no predictive role of mean platelet volume (MPV) in the diagnosis of TT. They also reported significantly high sensitivity and specificity for the NLR in the prediction of TT diagnosis. In addition, the NLR was significantly correlated with the duration of symptoms in TT patients. \u0000 \u0000Routine peripheral blood counts may be useful in the diagnosis and prognosis of many disorders such as TT. White blood cell (WBC) count is one such beneficial inflammatory biomarker in clinical experience. Even if the WBC count is in the normal range, hematologic parameters, especially subtypes of WBCs like the NLR, have been shown to play a predictive role in the prognosis of acute and chronic inflammatory processes. Neutrophil and lymphocyte counts can be obtained with a basic hemogram test [2,3]. The NLR may be an indicator of systemic inflammation. The NLR is measured by dividing the neutrophil count by the lymphocyte count. In an acute setting, lymphopenia is a common finding during a stress response secondary to increased levels of corticosteroids. Also, lymphopenia is observed in inflammatory states due to increased lymphocyte apoptosis [2]. \u0000 \u0000Another simple blood count marker like the MPV value is also considered to be a useful indicator of thrombocyte activity. The level of platelets is enhanced in acute inflammation, leading to a decrease in the MPV level as result of the migration of a majority of the large reactive platelets to inflammatory sites, where they are consumed [4]. Cicek et al. [5] compared serum MPV levels in patients with TT and healthy control subjects. They concluded that the MPV may be useful as a test for the diagnosis of TT. The NLR and MPV are easily measured laboratory markers used to evaluate systemic inflammation, but these markers can be associated with many conditions. Because abnormal thyroid function, renal or hepatic dysfunction, hypertension, diabetes mellitus, metabolic syndrome, known malignancy, previous history of local or systemic infection, inflammatory diseases, and certain medications can change the inflammatory status of patients, so too is the measurement of the NLR and MPV affected in all of the above conditions [3]. In this context, it would be better if the authors had mentioned these factors. \u0000 \u0000In conclusion, the NLR and MPV may be influenced by many situations. To pursue this goal, the KJU focused on rapid critical reviews, fast publication, and easy­to­access online audiovisual contents. On April 11, a KJU workshop took place in Busan during the Korean Urological Congress & Expo. The workshop was held to educate the possible reviewers of the KJU. Professor Ock­Joo Kim from Seoul National University gave a lecture on medical ethics, and there were several lectures introducing the online submission system and homepage of the KJU. Furthermore, there was a suggestion to hold regular education programs for both editorial board members and reviewers to establish a rigorous reviewing system. The KJU international editorial board meeting was held on May 17 during the American Urological Association meeting at the Morial Convention Center in New Orleans, USA. The editorial team of the KJU reported the current status of the KJU publication to the editorial board members. There were many suggestions and discussions about how to introduce the KJU to potential authors worldwide. One of the important issues was the lack of acknowledgement of the KJU as an international journal. To overcome this, we should f ind and promote unique features which can specify the journal’s scope and distinguish the KJU from other journals in the field of urology. The KJU will continue to keep the open access policy. The open access policy leads to a high likelihood that the KJU will be accessed and allows for rapid dissemination of clinical and basic science information to physicians and researchers in the field of urology. {"title":"Postoperative ureteral obstruction after endoscopic treatment for vesicoureteral reflux.","authors":"Jae Min Chung,&nbsp;Chang Soo Park,&nbsp;Sang Don Lee","doi":"10.4111/kju.2015.56.7.533","DOIUrl":"https://doi.org/10.4111/kju.2015.56.7.533","url":null,"abstract":"<p><strong>Purpose: </strong>We undertook this study to evaluate the incidence, risk factors, management, and outcome of postoperative ureteral obstruction after endoscopic treatment for vesicoureteral reflux (VUR).</p><p><strong>Materials and methods: </strong>Ninety patients undergoing endoscopic treatment for VUR were retrospectively reviewed and classified into two groups according to ureteral obstruction: the nonobstruction group (83 cases, 122 ureters; mean age, 7.0 ± 2.8 years) and the obstruction group (7 cases, 10 ureters; mean age, 6.2 ± 8.1 years). We analyzed the following factors: age, sex, injection material, laterality, voiding dysfunction, constipation, renal scarring, preoperative and postoperative ultrasound findings, endoscopic findings, injection number, and injection volume. Additionally, we reviewed the clinical manifestations, natural course, management, and outcome of ureteral obstruction after endoscopic treatment.</p><p><strong>Results: </strong>The incidence of ureteral obstruction after endoscopic treatment was 7.6% (10/132 ureters). The type of bulking agent used and injection volume tended to be associated with ureteral obstruction. However, no significant risk factors for obstruction were identified between the two groups. Three patients showed no symptoms or signs after the onset of ureteral obstruction. Most of the patients with ureteral obstruction experienced spontaneous resolution within 1 month with conservative therapy. Two patients required temporary ureteral stents to release the ureteral obstruction.</p><p><strong>Conclusions: </strong>In our experience, the incidence of ureteral obstruction was slightly higher than in previous reports. Our study identified no predictive risk factors for developing ureteral obstruction after endoscopic treatment. In addition, intratubular germ cell neoplasia (ITGCN) is a precursor lesion to testicular germ cell tumor. Approximately 50% of patients with ITGCN will develop an invasive of testicular germ cell tumors within 5 years. Therefore, we evaluated that the incidence of ITGCN in postpubertal cryptorchidism.</p><p><strong>Materials and methods: </strong>Between January 2002 and August 2012, orchiectomy specimens from 31 postpubertal patients (aged 12 or over) with cryptorchid testis were reviewed. The specimens were evaluated for ITGCN using immunohistochemical stains of placental-like alkaline phosphatase and Oct 3/4 with routine hematoxylin-eosin stain. Additionally, the degree of spermatogenesis was assessed using the Johnsen score.</p><p><strong>Results: </strong>Mean age was 34 years (range, 17 to 74 years) at surgery. All patients were diagnosed as unilateral cryptorchidism. One patient (3.2%) of 20-year-old had ITGCN in surgical specimen with all positive markers. Histological assessment of spermatogenesis showed that mean Johnsen score was 3.42 (range, 1 to 9). In this study, we present a case of robot-assisted retroperitoneal lymph node dissection performed in an 18-year-old man who was diagnosed with a stage IIIb mixed germ cell tumor and who was initially treated with radical orchiectomy, followed by chemotherapy. Flaps and graft techniques are widely used in genital and urethral reconstruction. A graft is tissue that is moved from a donor site to a recipient site without its native blood supply. The main types of grafts used in urology are full thickness grafts, split thickness skin grafts and buccal mucosa grafts. Flaps are transferred from the donor site to the recipient site on a pedicle containing its native blood supply. Flaps can be classified based on blood supply, elevation methods or the method of transfer. The most used flaps in urology include penile, preputial, and scrotal skin. Each patient in the RIRS group was matched to one in the PNL group on the basis of calculated propensity scores by use of age, sex, body mass index, previous treatment history, stone site, maximum stone size, and stone volume. We compared perioperative outcomes between the unmatched and matched groups.</p><p><strong>Results: </strong>Patients underwent PNL (n=87, 66.4%) or RIRS (n=44, 33.6%). After matching, 44 patients in each group were included. Mean patient age was 54.4 ± 13.7 years. Perioperative hemoglobin drop was significantly higher and the hospital stay was longer in the PNL group than in the RIRS group. The operative time was significantly longer in the RIRS group than in the PNL group. Stone-free rates were higher and complications rates were lower in the RIRS group than in the PNL group without statistical significance. The presence of a stone located in the lower-anterior minor calyx was a predictor of stone-free status.</p><p><strong>Conclusions: </strong>RIRS and single-session PNL for patients with a main stone of 15 to 30 mm located in the lower-pole calyx showed comparable surgical results. However, RIRS can be performed more safely than PNL with less bleeding. The group had multiple renal calculi, which were bilateral in 3 cases and were located in a total of 23 sites. The calculi were located in 2 calyces in 10 cases, scattered in more than 2 calyces in 7 cases, and limited to 1 calyx in 3 cases. The average patient age was 37.35 months (range, 14-68 months). The average stone diameter was 2.0 cm (range, 1-3.0 cm). In all patients, an ultramini nephrostomy tract was established under ultrasound guidance (dilated to F10) with simultaneous sheath placement. The flexible URS was placed into the collecting system during holmium laser lithotripsy.</p><p><strong>Results: </strong>When ultramini PCNL was combined with flexible ureterorenoscopic holmium laser lithotripsy, the complete stone-free rate was 87% (20/23). The average level of hemoglobin decreased to 1.0 g/dL after the operation. No blood transfusions were needed. Levels of blood urea nitrogen, creatinine, and C-reactive protein were not significantly different before and after the operation. The average duration of hospitalization was approximately 4.85 days, and all cases were followed up for 6 to 12 months. Information on preoperative statin use was obtained from patient charts in an electronic database. We assessed the impact of statin use on recurrence-free survival (RFS), cancer-specific survival (CSS), and overall survival (OS).</p><p><strong>Results: </strong>Of these 277 patients, 62 (22.4%) were taking statin medications. Compared to the statin nonusers, the statin users were older, had a higher body mass index, and had higher rates of cardiovascular disease and diabetes. The 5-year RFS rates of statin users and nonusers were 78.5% and 72.5%, respectively (p=0.528); the 5-year CSS rates were 85.6% and 77.7%, respectively (p=0.516); and the 5-year OS rates were 74.5% and 71.4%, respectively (p=0.945). We investigated the relationship of HSP27 and c-FLIP expression to prostate-specific antigen, Gleason score sum (GSS), and pathologic stage.</p><p><strong>Materials and methods: </strong>Samples from 163 patients between May 2004 and April 2010 were analyzed: 83 from patients that had underwent a radical prostatectomy, and 80 from those that underwent transurethral resection of the prostate to alleviate urinary symptoms from benign prostate hyperplasia. c-FLIP and HSP27 expression were observed by immunohistochemistry staining. Samples with less than 5% expression-positive cells were scored as 1, with 5%-50% were scored as 2, and with more than 50% were scored as 3. Local reactions were identified as 0.5 and evaluated.</p><p><strong>Results: </strong>Both the presence of HSP27 within the tumor and the number of cancer cells positive for HSP27 were significantly correlated to GSS and pathologic stage (p<0.001, p=0.001, p<0.001, p<0.001). The same was true for c-FLIP expression (p<0.001). GSS was more highly correlated to HSP27 expression than to c-FLIP expression (r=0.814 for HSP27, r=0.776 for c-FLIP), as was pathologic stage (r=0.592 for HSP27, r=0.554 for c-FLIP).</p><p><strong>Conclusions: </strong>In prostate cancer, higher GSS and a more advanced pathologic stage were associated with a higher likelihood of having a HSP27-positive tumor and more HSP27-positive tumor cells. HSP27 expression was correlated with GSS and prostate cancer stage. A more advanced pathologic stage corresponded to a higher likelihood of having a c-FLIP-positive tumor and more c-FLIP-positive tumor cells.
