Selvi","doi":"10.4274/tjod.37084","DOIUrl":"https://doi.org/10.4274/tjod.37084","url":null,"abstract":"SUMMARY Spontaneous acute pneumothorax may occur as a result of spontanous rupture of subpleural blebs or bullae and it is extremely rare during pregnancy. Spontaneous acute pneumothorax and pneumomediastinum were diagnosed in a young pregnant woman admitted with the complaints of swelling and pain on her neck and upper thorax following 2 days' dyspnea. Nasal oxygen, analgesics and antiemetics were used as needed during ten days' hospitalisation. Physical findings, chest X-ray and oxygen saturation improved at the end of this period. Since supportive treatment was sufficient, invasive treatments such as surgery or thorax tube were not required. Although rare, pneumothoax should be remembered in any pregnant woman with dyspnea and chest-pain and must be confirmed radiographically to distinguish it from other diseases and conditions. Previous studies report a cure rate of 80-92% with TOT. In this study, we aimed to evaluate treatment success in patients to whom TOT was applied in our clinics. Materials and methods: Patients to whom TOT was applied in our clinics between January 2009 and April 2013 were retrospectively evaluated. Follow up records were investigated to find out our TOT success rate and other urogynecologic problems that developed during follow up. Presence of ongoing urinary incontinence was accepted as TOT failure. Results: 107 cases to whom TOT was applied and with accessible data were included. Median age of the cases was 59 (26-84). 72% (n=77) of these were in menopause, 91.6% (n=98) were multiparous and 59.8% (n=64) had simultaneous gynecologic surgeries. Most frequent additional operation was vaginal hysterectomy (n=42, 39.3%). Median postoperative follow up duration was 2.4 years (0.5-4years). 13.1% (n=14) of the cases had ongoing urinary incontinence during follow up. Our TOT success rate was 87.9%. This rate was 87.5% in cases with and 88.4% in cases without a simultaneous surgery (p>0.05); 85.7% in postmenopausal and 93.3% in premenopausal cases (p>0.05). During follow up, cystocele developed in 8.4% and cuff prolapsus developed in 11.2% of the cases. Conclusions: Cure rate of TOT cases in our clinics is in accordance with the numbers in the literature. The underlying main pathology is an end organ resistance to androgen stimulation, and as a result, this impairs the differentiation of the body structures and systems which this hormone affects. Inheritance of this disorder is by means of an X-linked recessive gene that is responsible for the intracellular androgen receptors. CAIS may be observed in several members of a family. For this reason, the family members of the affected individual should be screened. Hastalarin insan koryonik gonadotropin (hCG) uygulanan gunde olculen endometrium kalinligi, toplam kullanilan gonadotropin dozu, stimulasyon sureleri, 3. gun bazal follikul stimule edici hormon (FSH) ile bazal estradiol (E2) duzeyleri, toplanan oosit ile fertilize oosit sayilari ve transfer edilen embriyo sayilari kaydedildi. Bu parametrelerin gebelik basarisi uzerine etkileri degerlendirildi. Hastalar 39 yas (n=47), 40-41 yas (n=37) ve 42 ve uzeri yas (n=23) olmak uzere uc yas grubu karsilastirildi. Bulgular: Canli dogum orani 39 yas grubunda %23,2 ve 40-41 yas grubunda %14,7 olarak gozlendi. Kirk iki ve uzeri yas grubunda ise canli dogum gorulmedi. Bu calismanin sonuclari yasin, toplanan oosit ve fertilize oosit sayisinin gebelik gelisimi uzerine etkisinin oldugunu gosterdi. Sonuc: Gebelik orani 39 yasindan sonra hizli bir sekilde dusmektedir; intrasitoplazmik sperm enjeksiyonu sonrasi, hastalarda 39 yasindan itibaren gebelik basarisi 3 yil sonra 9 kat azalmistir. J Turk Soc Obstet Gynecol 2014;2:78-83 Anahtar Kelimeler: Kadin, yas, gebelik orani Objective: The objective of this study is to investigate the effect of age on the prognostic factors and fertilization potential of infertile women who are 39 years or older. Material and Methods: 107 patients with advanced maternal age (≥39 years) were included in this retrospective study. The endometrial thickness on the day of human chorionic gonadotropin (hCG), total dose of gonadotropin used, duration of stimulation, levels of day-3 basal follicule stimulating hormone (FSH) and basal estradiol (E2), number of retrieved oocytes and fertilized oocytes, and number of embryos transferred were recorded. These parameters were used to evaluate their effects on the success of pregnancy. Patients were categorized into three age groups: 39 years old (n=47), 40-41 years old (n=37), and ≥ 42 years old (n=23) and age groups were compared. Results: The rate of live birth was %23.2 in 39 years and %14.7 in 40-41 years. No live birth in 42 years or older patients was recorded. The results revealed that that the age, number of retrieved oocytes and fertilized oocytes predict the success rate of pregnancy. Conclusion: Pregnancy rate dramatically decreased after the age of 39; the success of pregnancy after the intracytoplasmic sperm injection decreases 9 times, three years later starting age of 39. The negative effect of advanced maternal age on pregnancy and live birth rates was observed in our clinical study, as well. It can affect all age groups but is seen mostly in young women. The diagnosis of CVST is not easy because of variability of clinical symptoms and signs. The patients can present with headache, papilledema, focal deficit, seizures and coma. The most frequent symptom is severe headache. The most common causes of CVST are pregnancy, puerperium, systemic inflammatory diseases, inherited and acquired coagulation disorders, trauma, lumbar puncture, drugs and infection. The use of oral contraceptive (OC) increases the risk of cerebral sinus thrombosis. If there is an additional hereditary prothrombotic factor, the risk of CVST is more. CVST affects superior sagittal sinus and the lateral sinuses more frequently. More than one sinus is affected in one-third of cases. Computed tomografi helps to exclude other intracranial condition but diagnostic value of CT in CVST is limited. Combination of magnetic resonance imaging (MRI) and magnetic resonance venography (MRV) have replaced cerebral angiography for diagnosis of CVST. The treatment of CVST contain supportive (hydration, antibiotheraphy, anticonvulsant) and anticoagulan treatment. Heparin is effective and safety for the treatment of CVST because of this, it is the first-line treatment. The safety of heparin treatment also has been shown in those patients who had hemorrhagic infarcts. Outcomes of CVST is variable from death to complete improvement, but in general it is favorable. Recurrence of CVT is not common. In this study, we aimed to investigate the relationship between mineral density of femur neck-lumbar vertebral bones and endogen sex steroid hormone levels in postmenopausal healthy women. Material and methods: Estradiol[E2], total testosterone[TTE], free testosterone[FTE], dehydroepiandrostenedion sulfate[DHEAS], androstenedione and sex hormone-binding globulin[SHBG] measurements were performed, lumbar(L1-L4) and femur neck T scores were calculated with dual energy x-ray absorbsiometry method in 415 postmenopausal healthy women who referred to our clinic between 2006 June and 2007 May. Intermeasurement relationships and correlations were also investigated. Results: The mean age was 55.08±6.74(35-82) years, mean lumbar vertebral T scores was -1.23±1.36(-1,85-3,00), mean femur neck T scores was ‐0.56±3.24(-4,40-2,99), mean body mass index(BMI) was 26.99±4.23(18,31-40,00) and mean years since menopause was 8.90±7.20(1,00-50,00) years. In Pearson correlation analysis, lumbar vertebral T score correlated with age(p<0.001;r=-0.203), years since menopause(p<0.001;r=-0.190), BMI(p=0.004;r=0.141) and DHEAS(p=0.024;r=0.112). Femur neck T score correlated with BMI(p=0.028;r=-0.108) and DHEAS(p=0.022;r=0.114). In multiregression analysis, when lumbar vertebral T score was an dependent variable, it showed a significant association with body mass index(p=0.028; s=0.131) and years since menopause (p=0.048; s=-0.149) independently. When femur neck T score was dependent variable, it showed a significant association with TTE(p=0.001; s=0.267), SHBG(p=0.001; s=-0.432) and free androgen index (p=0.005; s=0.407) independently. Conclusion: In postmenopausal women, level of gonadal hormones is one of factors that determine osteoporosis. According to this study, androgenic parameters such as TTE, free androgen index and SHBG are associated with the bone mineral density at femur neck in postmenopausal healthy women.
