Ishizaki, Masaru Tanaka","doi":"10.3812/jocd.38.618","DOIUrl":"https://doi.org/10.3812/jocd.38.618","url":null,"abstract":"Eruptive syringoma is a rare type of syringoma, a benign tumor of the eccrine ducts, and is characterized by the presence of widespread hyperpigmented papules on the trunk with onset around puberty. Here, we report two cases of eruptive syringoma presenting with characteristic dermoscopic findings. Case 1 involved a 21-year-old man who had developed a widespread skin rash on his trunk around the age of 19 years. Clinical examination showed small brown papules spreading on the trunk and both axillae. The differential diagnoses included verruca plana and Darier's disease. Case 2 involved a 67-year-old man who had been aware of itchy brown papules on the trunk since he was in his twenties. Dermoscopy in cases 1 and 2 showed homogeneous light brown pigmentation with multifocal whitish areas and a delicate pigment network at the periphery, which are typical characteristics of eruptive syringoma. Histopathological examination showed numerous small ducts and islands of epithelium embedded in a fibrous stroma in the upper dermis. These findings were compatible with the diagnosis of eruptive syringoma in cases 1 and 2. On comparing the dermoscopic and histopathological findings of syringoma, the whitish area with unclear borders corresponded to the fibrous stroma in the upper dermis, and the unclear borders corresponded to the dermal lesion. Watanabe, Aya Osuga, Hiroaki Fukuda, Yoshinobu Kikukawa","doi":"10.3812/jocd.38.445","DOIUrl":"https://doi.org/10.3812/jocd.38.445","url":null,"abstract":"再発性性器ヘルペス(recurrent genital herpes:RGH)患者232名と,皮膚科,婦人科・産婦人科(以下,婦人科),泌尿器科の医師231名を対象に,RGHの患者ニーズおよび医師の診療実態に関するアンケート調査を行った. 回答を得た患者の男女比は6:4で,再発頻度が年1~2回の患者が8割を占めた.患者全体の6割が内服抗ウイルス薬(以下,内服薬)を単独または併用で使用していたのに対し,3割は外用抗ウイルス薬(以下,外用薬)のみで治療していると回答した.現在の治療を「満足」とした患者は,Patient Initiated Therapy(以下,PIT)で44.6%,5日間投与で31.1%,再発抑制療法で32.5%,外用薬のみで17.6%であった. 診断にウイルス抗体検査を用いると回答した医師のうち,受診時の皮疹におけるウイルスの有無を判断する目的で抗体検査を実施すると回答した医師は,婦人科および泌尿器科で半数を超えた. Ohyama","doi":"10.3812/jocd.38.611","DOIUrl":"https://doi.org/10.3812/jocd.38.611","url":null,"abstract":"Alopecia areata (AA) is a commonly encountered autoimmune-mediated hair loss disease clinically characterized by well-demarcated hair loss patches and histologically by dense peribulbar lymphocytic cell infiltration leading to the destruction of hair follicles. Because of its distinctive clinical presentation, other hair loss diseases presenting clear hair loss patches can be misdiagnosed as AA. Thus, the confirmation of AA diagnosis by the combination of diagnostic approaches, including physical examination, hair pull test, trichoscopy, and skin biopsy is important. Trichotillomania (TT), scarring alopecia (represented by lichen planopilaris and chronic lupus erythematosus), tinea capitis, and triangular alopecia can manifest hair loss patches or areas hardly distinguishable from those in AA. Such findings as the absence of hair shaft pluckability or atrophic changes in the hair bulb, microscopic detection of fungi within isolated plucked hair shafts should be helpful to distinguish the aforementioned non-AA hair loss diseases from AA. Trichoscopic detection of follicular microhemorrhage, V-sign, and flame hairs are useful for the diagnosis of TT, while loss of the hair ostia is typical for scarring alopecia. Triangular alopecia is less frequently encountered and is distinguished by the presence of vellus-like hairs with in clinically hair loss patches as detected by trichoscopy. AA may present hair loss pattern resembling those of androgenetic alopecia (AGA) and female patten hair loss (FPHL). Naka","doi":"10.3812/jocd.38.62","DOIUrl":"https://doi.org/10.3812/jocd.38.62","url":null,"abstract":"目的と方法:実臨床における爪白癬患者に対するホスラブコナゾールL-リシンエタノール付加物(ネイリンⓇカプセル,以下ホスラブコナゾール)の有効性および安全性を検討する目的で,2018年8月~2020年2月に当院を受診し,爪白癬の診断の下にホスラブコナゾールを処方された患者を対象としてカルテ情報を収集した. 結果:対象は爪白癬患者360例(足爪白癬340例,手爪白癬28例,そのうち両者の重複例8例)で,男女とも70歳台が最も多かった.ホスラブコナゾール12週投与を終了し投与開始から1年以上経過した足爪白癬84例における治療成績は完全治癒59例(70.2%),転帰不明22例(26.2%),無効3例(3.6%)であった.完全治癒は治療開始後3カ月と6~12カ月に確認できた例が多く,転帰不明例の多くはドロップアウト(最終来院)時に改善傾向を示していた.360例中ホスラブコナゾール12週の投与を完結できた症例は249例(69.2%)で,治験で除外されていた高齢者や若年者,手の爪白癬または難治な爪白癬に対しても本薬の高い有効性が認められた.投薬中の臨床検査にて260例中ALT/ASTは18例(6.9%)に,γ-GTPは61例(23.5%)に異常値を認めたが,皮膚症状など他の副作用を含め重篤な例はなかった. 結論:ホスラブコナゾールは実臨床においても完全治癒率と投与継続率が高く,肝機能検査など適切な対処により重篤な副作用を回避できることから,合併症などで内服が困難な例や外用薬で早期完治が期待できる例を除き,すべての爪白癬症例において早く確実に治すための第一選択薬として用いられるべき薬剤と思われる.","PeriodicalId":17355,"journal":{"name":"Journal of The Japan Organization of Clinical Dermatologists","volume":null,"pages":null},"PeriodicalIF":0.0,"publicationDate":"2021-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"73725257","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
