Yokoyama","doi":"10.3812/jocd.39.394","DOIUrl":"https://doi.org/10.3812/jocd.39.394","url":null,"abstract":"Clinicopathologic Myopericytoma is a benign subcutaneous tumor that occurs in the limbs of adults; a rare variant of myopericytoma occurs intravascularly. A 43-year-old man presented with a 3-year history of a 15-mm mass in the subcutis of his right inner thigh. The patient had no history of trauma, although due to his work he had to stand for 8 hours per day. Several tumor lesions were scattered in the subcutaneous tissue. Oval and spindle cells with no atypia were collected around the endothelial cells in a concentric arrangement. By immunostaining, tumor cells were positive for smooth muscle actin and negative for desmin, CD31, CD34, and factor VIII, which suggests the tumor was present in a vessel. Based on these findings, the lesion was diagnosed as intravascular myopericytoma (IVMP). To clarify the etiology of IVMP, we compared our case with 15 previously diagnosed cases of intravascular myopericytoma by age, sex, duration, site, and morphology. The patients had 14 dermatophytoma lesions. Surgical avulsion was performed at the first visit. Ten of the 14 nails were evaluated. By approximately 1 year, seven nails were completely cured with no clinical involvement of the target toenail as shown by the negative potassium hydroxide test. ; tTreatment failed for three nails (<30% improvement or worsening). The second efficacy evaluation was performed at the last visit or when changing therapeutic drugs were changed. This e second efficacy evaluation revealedshowed complete cure in eight nails, marked improvement (>60% improvement in nail involvement ratio) in one nail, and improvement( (30% or more to less than 60%) ) in one nail. All 10 evaluable dermatophytoma lesions disappeared in 16 months. Matsui","doi":"10.3812/jocd.38.434","DOIUrl":"https://doi.org/10.3812/jocd.38.434","url":null,"abstract":"過酸化ベンゾイルゲル（ベピオ®ゲル 2.5 %，以下，本剤）は，2014年12月に尋常性痤瘡を効能として承認された，抗菌作用および角層剥離作用を有する薬剤である．2015年7月より本剤の日常診療下における尋常性痤瘡に対する特定使用成績調査を実施し，観察期間12ヵ月間での安全性および有効性について検討した． 安全性解析対象症例の15.2%（169/1109例）に副作用が認められた．重篤な副作用として適用部位紅斑が1例認められたが，それ以外は非重篤であった．副作用発現症例169例すべてに，医薬品リスク管理計画で「重要な特定されたリスク」とされている皮膚刺激症状がみられており，そのうち119例は本剤使用1ヵ月以内に発現していた．女性や乾燥肌，敏感肌の症例等で皮膚刺激症状の発現頻度が高い傾向がみられたが，特に本剤の使用を回避すべき患者層はなかった． 全顔の皮疹数の減少率（中央値）は，12ヵ月後までの最終評価時において，炎症性皮疹が80.0%，非炎症性皮疹が66.7%，総皮疹が73.9%であった．また，最終評価時の全般改善度「著明改善」または「改善」と判定された症例は，顔面で71.4%（788/1103例），顔面以外で64.1%（59/92例）であった．Skindex-16日本語版を用いたquality of life評価において，症状，感情，機能および総合スコアの全てが本剤使用開始時と比べて3ヵ月後に有意に減少しており，本剤使用12ヵ月後においても減少状態が維持されていた． 以上より，本剤は実臨床において，急性炎症期だけでなく長期使用した場合にも有用な薬剤であることが示された。","PeriodicalId":17355,"journal":{"name":"Journal of The Japan Organization of Clinical Dermatologists","volume":null,"pages":null},"PeriodicalIF":0.0,"publicationDate":"2021-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"76008549","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
