White DDS, MS","doi":"10.1016/j.adaj.2025.07.005","DOIUrl":"10.1016/j.adaj.2025.07.005","url":null,"abstract":"<div><h3>Background</h3><div>Despite lowering financial barriers to oral health care through the Patient Protection and Affordable Care Act, income-related access-to-care disparities remain, especially in Oregon. The authors examined the role of a novel dental care organization, the Willamette Dental Group (WDG), in addressing the nonfinancial barriers to oral health care that keep access-to-care disparities in place.</div></div><div><h3>Methods</h3><div>This longitudinal observational study used dental claims and enrollment data from the Oregon Health Plan from 2014 through 2021. The primary outcome measures were any dental visit and an examination visit. Enrollees in the Oregon Health Plan aged 0 through 18 years who were members of WDG’s Medicaid population were compared with those in the non-WDG Medicaid population. A propensity score subclassification matching method using child-, family-, and community-level variables was used to approximate a randomized study and reduce bias.</div></div><div><h3>Results</h3><div>The percentage of children who had any dental visit or an examination visit was significantly (<em>P</em> < .001) higher among the WDG members than non-WDG members. Compared with non-WDG programs, a child in the WDG program had a 40% higher chance of having any visit and a 48% higher chance of having an examination visit.</div></div><div><h3>Conclusions</h3><div>Children enrolled with WDG under the Oregon Health Plan experienced substantially higher oral health care use than children in non-WDG programs. Future studies should focus on the correlation of improved use with oral health outcomes at WDG.</div></div><div><h3>Practical Implications</h3><div>The innovative practice policies of WDG may lead the way for other large-scale dental delivery programs hoping to improve use for children enrolled in Medicaid. The definitions based on histology, morphology, and other characteristics have become obsolete, and the accepted terminology is gingival enlargement. The authors aim was to explore the clinical importance of GEs.</div></div><div><h3>Types of Studies Reviewed</h3><div>The authors searched electronic databases (ie, PubMed, Ovid, Embase, Web of Science, and Google Scholar) for studies that were focused on GEs and their previous synonyms and published from January 1, 1950, through January 31, 2025.</div></div><div><h3>Results</h3><div>The authors found articles in which researchers elucidated the etiologic, pathophysiologic, and histologic aspects of GEs. They also discovered some consensus on the management of these conditions.</div></div><div><h3>Conclusions</h3><div>The term gingival enlargement is the most accepted terminology and appears to be the most correct scientifically. Identifying the etiologic factor for GEs is crucial. Management of GEs varies from rigorous plaque control to surgical modalities.</div></div><div><h3>Practical Implications</h3><div>Knowledge about the various contributing factors, clinical manifestations, histologic appearances, and differences in management strategies is important. {"title":"Patient trust in artificial intelligence for orthodontic advice: A systematic review.","authors":"Sanjana Santhosh Kumar, Dimitrios Michelogiannakis, Xiuhui Xu, Rachel Chacko, Junad Khan","doi":"10.1016/j.adaj.2025.08.003","DOIUrl":"https://doi.org/10.1016/j.adaj.2025.08.003","url":null,"abstract":"<p><strong>Background: </strong>Artificial intelligence (AI) large language models are being used increasingly for health care-related queries. This systematic review assessed the reliability of AI chatbot responses to patient questions related to orthodontics.</p><p><strong>Types of studies reviewed: </strong>PubMed, Web of Science, Scopus, Ovid MEDLINE, and Embase were systematically searched for studies published from January 1, 2022, through February 28, 2025. Only cross-sectional studies were included. Data were screened and extracted, and the risk of bias was assessed.</p><p><strong>Results: </strong>Of 105 identified studies, 13 met the inclusion criteria. ChatGPT (OpenAI) showed 68.7% and 92.6% accuracy rates, scoring well on Likert scales and DISCERN evaluations. However, some studies reported lower scores (2.2/4.0, 3.8/10.0) and mixed results for Bard (Google) and Gemini (Google), highlighting inconsistencies in chatbot performance.</p><p><strong>Practical implications: </strong>Whereas large language models, particularly ChatGPT, cannot replace professional consultations, they offer generally reliable orthodontic information and can be valuable tools for addressing routine patient questions. Based on evolving commentary, AI is likely to serve as a frontline support system in the future, helping manage common inquiries, reduce clinician workload, and improve cost-effectiveness while ensuring that final care decisions remain with qualified professionals.
