Mahsood","doi":"10.36351/pjo.v40i1.1636","DOIUrl":"https://doi.org/10.36351/pjo.v40i1.1636","url":null,"abstract":"Purpose: To determine the early and late complications of Ahmed Glaucoma Valve (AGV) implant surgery.\u0000Study Design: Interventional case series.\u0000Place and Duration of Study: Ophthalmology department of MTI-Hayatabad Medical Complex Peshawar, from May 2018 to February 2023.\u0000Methods: All the patients between May 2018 and February 2023 who underwent AGV implant were included in the study. Data included patient's age, gender, indications for surgery, type of AGV implant used, length of follow-up period, intraocular pressure (IOP), number of topical glaucoma medications used, early and late complications, and the final outcome. Continuous variables (duration of follow-up, best corrected visual acuity, IOP, and number of topical anti-glaucoma medications) were represented in means and standard deviations. Paired t-test was applied for comparison of means between baseline and last follow-up data. A p-value of < 0.05 was considered statistically significant.\u0000Results: A total of 28 eyes were included. Mean age of the patients were 28.75 ± 20.5 years. Duration of follow-up was 213.96 ± 169.33 days. There were 53.85% females. Commonest indication was congenital glaucoma 7 (25%). Majority, (n = 18, 64.3%) of the eyes had previous cataract surgeries followed by glaucoma, 13 (46.4%). The commonest early post-operative complication was hyphema (25%) and commonest late complication was hypertensive episode 32.1%. Majority of the eyes 23 (82.1%) did not require any further intervention for these complications.\u0000Conclusion: Majority of AGV complications are self-limiting, necessitating just conservative therapy. {"title":"Lucidis 124m Edof Iol; Experience from Two Tertiary Care Centers and Quality of Life Study","authors":"Zia ul Mazhry, Faiza Hassan, Muhammad Abdullah, Laiba Asif","doi":"10.36351/pjo.v40i1.1719","DOIUrl":"https://doi.org/10.36351/pjo.v40i1.1719","url":null,"abstract":"Purpose: To determine visual outcomes and post-operative complications after Lucidis 124M EDOF IOL implantation after phacoemulsification and to see the effect of these lenses on Quality of life of the patients.\u0000Study Design: Mixed study design (Quasi Experimentaland qualitative analysis).\u0000Place and Duration of Study: WAPDA Teaching Hospital and Acuity Eye Centre, Lahore, from November 2017 to October 2022.\u0000Methods: Hundred eyes of seventy five patients underwent Lucidis EDOF IOL implantation after phacoemulsification using 2.75 mm clear posterior corneal incisions. Criteria used to document visual outcomes were uncorrected and corrected distance and near vision measurements. Quality of life (QoL) was assessed by interviewing the patients at least 6 months postoperatively using google forms.\u0000Results: Hundred eyes of 75 patients were enrolled. There were 39 (52%) males and 36 (48%) females. Mean age was 1.48 ± 0.50 years. Meanbaseline un-corrected visual acuity (UCVA) was 2.83 ± 1.48. Eighty two percent eyes had corrected vision of 6/6 and 75 eyes achieved N.6 for near. Fifty patients out of 75 responded to the questionnaire regarding QoL. Sixteen percent patients had glare, 85% reported no difficulties during night driving and 68% had no problem in watching TV without glasses. Sixty eight percent did not experience any itching, watering, or discomfort and 90% responded that their QoL had improved. None of the patients had lens deposits, discoloration or decentration. {"title":"Latent Convergent Strabismus Clinical Features and Management in Sudanese Patients","authors":"Aisha Hassan Osman, Saif Hassan Al-Rasheed","doi":"10.36351/pjo.v40i1.1735","DOIUrl":"https://doi.org/10.36351/pjo.v40i1.1735","url":null,"abstract":"Purpose: To describe the clinical features and management of latent convergent strabismus among Sudanese patients.\u0000Study Design: Descriptive observational study.\u0000Place and Duration of Study: Makkah Eye Complex, Khartoum, Sudan, from December 2018 to August 2020.\u0000Methods: The study included 137 patients, diagnosed with latent convergent strabismus (esophoria). Demographic data and ocular examination findings including visual acuity, refractive error, and degree of esophoria was collected. Esophoria was assessed by prism cover test (PCT) at near and distance.\u0000Results: Esophoria was more common among females (n = 107, 78.1%) and in age group 15 to 25 years. Headache was the most common complaint (n = 64, 46.7%). Most of the patients (n = 114, 83.2%) had 1 to 5 ∆ of esophoria with a mean of 4.17 ± 2.99∆. Majority of the patients (n = 132, 96.3%) had normal near point of convergence (NPC) 6.98 ± 3.10 cm. Positive fusional vergence (PFV) was high at 24.06 ± 9.01∆, whereas negative fusional vergence (NFV) was low at 5.0 ± 0.21∆D, P = 0.001. Majority of the patients (76.6%) responded well to the refractive correction, 11.7% patients to vision therapy and remaining 11.7% to both refractive correction and vision therapy. Correlation coefficients revealed that there was a positive correlation between, refraction, NPC, and PFV (P ˃ 0.05).\u0000Conclusion: Sophoria was more common among females and young adults. Most of the esophoric patients had uncorrected hyperopia, good PFV and relatively poor NFV amplitude. All participants underwent detailed ocular examinations. A unilateral cover test was performed with the prism bar to achieve binocular alignment. Deviation magnitude was determined by considering the best refractive correction. Fixation targets were chosen as single letters one line above the best-corrected visual acuity on the distance and near chart.\u0000Results: This study included 5054 participants with a mean age of 35.35 ± 19.4 years. Strabismus was detected in 110 participants (2.17%) including 26 (0.51%) with esodeviation, 74 (1.46%) with exodeviation, and 10 (0.19%) with vertical deviation. The prevalence rates for strabismus, exotropia and esotropia were 2.38%, 1.64%, and 0.56% in females and 2.00%, 1.31%, and 0.51% in males respectively. The results did not indicate a significant gender difference in terms of strabismus prevalence (p>0.05). Among the deviated eyes, there were 25.5% with hyperopia, 39.1% with myopia, and 35.5% with emmetropia. Anisometropia and amblyopia rates were 2.79% and 2.55%, respectively. The results did not reveal a significant correlation between strabismus prevalence, exotropia, and esotropia and age (p=0.56). The familial strabismus prevalence was 5.45%.\u0000Conclusion: Exotropia was more prevalent form of strabismus. Myopia was the dominant refractive error associated with deviated eyes. We identified various methods of Visometry, published articles and patents for the inventions based on the assessment of visual acuity in deaf-mute children in the Russian Federation and parts of countries formerly part of the Soviet Union, in order to identify shortcomings and search for solutions. According to the total number of 126 cited scientific papers, the most common methods for assessing visual acuity in deaf–mute patients included Golovin-Sivtsev table (89.1%) and new JEI/JEI technique (8.9%). However, 2% of the doctors used tables of \"illiterate\" E, Teller Acuity cards with hand gestures, combining Teller Acuity cards with hand gestures to trigger reactions, Lea symbols with visual cues enabling visual cues or pointing to stimulate recognition and Graphic tables with sign language. Patients who underwent elective phacoemulsification operation under sedation and were randomly divided into the Propofol + Sodium thiopental (Thiofol) and Propofol + Etomidate (Etofol) groups. There were 32 patients in each group. Demographic information, sedation level, medical condition, anesthetic complications, hemodynamic parameters prior to, during, and following the operation, and the visual analog scale (VAS) were measured by an anesthesiologist and compared.\u0000Results: The groups showed significant differences in the levels of Ramsay scores (P<0.001). The frequency of Ramsay scores 2 and 3 was 43.75%, 56.25%, and 0%, 100% in Etofol, and Thiofol groups, respectively. After informed consent, patients were divided into two groups: Group S (n = 20) was given 1% prednisone eye drops for 14 days while group N (n = 20) was given 0.1% nepafenac acid eye drops for 14 days. 30 days after surgery, central macular thickness (CMT), using OCT, was assessed and documented in both groups. Data was analyzed by SPSS 21.\u0000Results: Mean age of patients was 44.02 ± 6.08 years. There were 60% (24) males and 40% females. Mean pre-operative CMT difference between two groups was statistically insignificant. Post-operative CMT was significantly lower in group N as compared to group S [243.00 ± 21.71 µm vs 223.90 ± 17.52 µm; p = 0.004]. There were two groups and each group had fifty-one patients. All patients underwent pars plana vitrectomy and silicon oil tamponade. Group 1 was control group. Group 2 had intravitreal injections of methotrexate 500mg at the end of surgery. We repeated the injections at 1st, 2nd, and 4th week postoperatively.\u0000Results: There were 56 (54.9%) male and 46 (45.1%) female patients. Postoperatively visual acuity improved in thirty cases (58.82%) in group 1 and in forty-two cases (82.35%) in group 2. Postoperatively PVR developed in 11 (21.57%) patients in group 1 while in group 2 no patient had PVR. Recurrent Retinal detachment occurred in 9 (19.61%) patients in group 1 and 3 (5.88%) patients in group 2. Chi square test demonstrated statistically significant difference in the frequency of development of PVR and re-detachment of retina between two groups, p<0.05.\u0000Conclusion: Methotrexate is an effective adjuvant used in Pars plana Vitrectomy for rhegmatogenous retinal detachment repair. Bananzada, Ahmed Abbas Kadhim, Khitam Fahkir AlHasseny","doi":"10.36351/pjo.v40i1.1757","DOIUrl":"https://doi.org/10.36351/pjo.v40i1.1757","url":null,"abstract":"Purpose: To determine the etiologies and demographic distribution of uveitis in a referral center in Baghdad, Iraq, and to compare the results with a similar study done in the same center in 2014.\u0000Study Design: Descriptive observational study.\u0000Place and Duration of Study: Ibn Al-Haitham Teaching Eye Hospital, Iraq, from January 2019 to June 2022.\u0000Patients and Methods: Two hundred and fourteen patients with uveitis were included. They underwent complete ocular and relevant systemic examination. Data was collected and analyzed using descriptive statistics. Comparison was done with the data retrieved from 2014.\u0000Results: Out of 264 patients, only 214 (81%) completed follow up. Others did not show up again due to Corona virus Pandemic. There were 122 (57%) females, 95 (44.3%) were unilateral, 62 (28.9%) were granulomatous, 62 (28.9%) were anterior, 42 (19.6%) were intermediate, 4 (1.8%) were posterior and 106 (49.7%) were panuveitis cases. Cause was identified in 161 (75.3%) patients while 53 (24.7%) were considered idiopathic. Out of 214 patients, 84 (39.2%) were due to infectious causes like tuberculosis, herpes (herpes zoster or simplex viruses), and toxoplasmosis. On the other hand, 130 (60.7%) cases were non-infectious for example, Vogt-Koyanagi-Harada disease, Behcet disease, connective tissue diseases such as Ankylosing Spondylitis, or idiopathic.\u0000Conclusion: A minor change was found from the 2014 study to 2023. Tuberculosis is now the most common cause of infectious uveitis instead of Toxoplasmosis. Agni, Indra Tri Mahayana","doi":"10.36351/pjo.v40i1.1690","DOIUrl":"https://doi.org/10.36351/pjo.v40i1.1690","url":null,"abstract":"Purpose: To determine the role of intra-silicon oil methotrexate injection to prevent Proliferative Vitreoretinopathy.\u0000Study Design: Quasi experimental Study.\u0000Place and Duration: Ophthalmology department of Sahiwal Teaching Hospital, Sahiwal from February 2022 to August 2022.\u0000Methods: Study included patients of either gender, older than 19 years with rhegmatogenous retinal detachment with Proliferative vitreoretinopathy (PVR) grade B or less. There were two groups and each group had fifty-one patients. All patients underwent pars plana vitrectomy and silicon oil tamponade. Group 1 was control group. Group 2 had intravitreal injections of methotrexate 500mg at the end of surgery. We repeated the injections at 1st, 2nd, and 4th week postoperatively.\u0000Results: There were 56 (54.9%) male and 46 (45.1%) female patients. Postoperatively visual acuity improved in thirty cases (58.82%) in group 1 and in forty-two cases (82.35%) in group 2. Postoperatively PVR developed in 11 (21.57%) patients in group 1 while in group 2 no patient had PVR. Recurrent Retinal detachment occurred in 9 (19.61%) patients in group 1 and 3 (5.88%) patients in group 2. Chi square test demonstrated statistically significant difference in the frequency of development of PVR and re-detachment of retina between two groups, p<0.05.\u0000Conclusion: Methotrexate is an effective adjuvant used in Pars plana Vitrectomy for rhegmatogenous retinal detachment repair.
