The chemical industry has a great potential for catastrophic accidents due to the critical process parameters and the significant volume of hydrocarbon materials. This study aimed to investigate the relationship between the components of occupational safety climate and the occurrence of unsafe behaviors in the chemical industry. Methods: This cross-sectional study was conducted in 2019 in the petrochemical industry in the south of Iran. In this study, 269 operational personnel were surveyed. To analyze the safety climate, the standard occupational safety climate questionnaire was used. To study the occurrence of unsafe behaviors, a checklist of unsafe behaviors was used according to the causes of accidents that have occurred in the studied industry during the last 15 years. Data analysis was performed using one-way analysis of variance, Pearson correlation coefficient, and multiple linear regression in SPSS software version 25. Results: Significant inverse relationship was observed between the workplace safety dimension and unsafe behavior. There was also a significant relationship between the dimension of management commitment and the experience of accident and near-miss and unsafe behavior (P-Value <0.05). The regression model results showed that the most critical factors affecting the occurrence of unsafe behaviors included the components of management commitment, workplace safety, probability of ignoring the error, education level, and work experience (R = 0.559, P-Value <0.05). Conclusion: The results of the present study showed that the dimension of management commitment, level of workplace safety, level of education, and work experience in chemical industries are among important and influential factors in the occurrence of unsafe behaviors. In this regard, prioritizing strategies is important due to financial and time constraints. Therefore, the present study aimed to Identify and prioritizes the strategies of a trauma hospital by Analytical Hierarchy Process. Methods: This descriptive study was carried out at trauma hospital in Yazd in 2018 with the aim of identifying the internal (weakness and strength) and external (opportunities and threats) factors and prioritizing each factor by AHP. The research population consisted of all senior and middle managers of the hospital who were selected purposefully. Information gathering was done by interviews and focused group discussion as well as the researcher-made questionnaire. The data were analyzed by AHP method and Expert Choice 11 software. Results: In the phase of identifying, 101 weaknesses, 23 strengths, 35 opportunities and 14 threats were identified. The highest priority in each area was related to the experienced and committed staff (0.289), weak monitoring system to monitor physicians (0.235), hospital geographical location factors in terms of centrality in the city (0.283) and negative attitudes of people regarding high death rates (0.310), respectively. Conclusion: According to the results, it is recommended to use the capacities of the University of Medical Sciences to monitor medicines and to set up smart systems in order to increase the supervision of medical staff. Meanwhile, military hospitals need more serious attention because of their critical position. The purpose of this study is to identify and prioritize the factors affecting the management of medical equipment supply chain in a military hospital in Tehran and finally evaluating the performance of the hospital in this regard. Methods: At first, by examining previous researches and interviewing with some of staff and hospital managers, the influential factors in the management of the medical equipment supply chain were identified. in this regard, The MAXQDA 2020 software was used for this purpose. Then, effective methods were weighed using Fuzzy Analytic Hierarchy Process (AHP). Finally, the current performance of the hospital was assessed using a questionnaire. Results: Based on the findings, the factors affecting the supply chain of medical equipment supply were divided into 5 general categories. These 5 categories which found their weights obtained from fuzzy AHP, respectively: Resources and stakeholders, internal operations management, sustainability, integrity and information management. Also, the results of the performance evaluation showed that the current condition of the hospital is 43%, which is actually less than the acceptable level. Conclusion: The importance of medical equipment supply chain management is not only due to its important role in the quality of treatment but also because of its major contribution to the hospital's operating budget. It should be noted that the factors affecting this issue are directly and indirectly related to each other.
