Methods & methods: A retrospective analysis of patients with available anti-TNF levels attending an IBD clinic at a tertiary centre from 1st January 2014 – 31st May 2020 was completed. Demographics, disease activity scores, endoscopic findings and biomarker data were collected. The area-under-curve (AUC) on the receiver operating characteristic curve was plotted to measure and compare the performance of variables in predicting endoscopic remission. This was supplemented by linear discriminant analysis. Results: One hundred and eighty patients were included. Faecal calprotectin was better at distinguishing between quiescent and active endoscopic disease than anti-TNF level .AUC 0.78 (95% CI 0.68 – 0.89) versus 0.62 (95% CI 0.48 – 0.75). Faecal calprotectin had a higher sensitivity (77% vs 50%) and specificity (71% vs 64%) compared to anti TNF levels in predicting endoscopic activity. We found that a faecal calprotectin threshold of 200ug/g and similarly an anti TNF level of 6.21ug/ml optimally predicts endoscopic disease. Using linear discriminant analysis, faecal calprotectin was weighted against C-reactive protein (CRP), albumin, platelet count, anti-TNF, and has shown to be better at predicting mild to moderate disease activity (Log FC=0.74). Conclusion: Faecal calprotectin as opposed to anti-TNF level is more likely to predict endoscopic disease activity. We plan to do a prospective study to confirm these findings and to develop a set of clinical cut-offs to improve disease management.","PeriodicalId":163845,"journal":{"name":"Gastroenterology & Hepatology: Open Access","volume":"15 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2022-03-25","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"114348920","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Methods: TLD (Thermoluminescence Dosimetry) chips were used in this project. Typically, each patient under an irradiation of 200 cGy during 25 sessions received a total of 5000 cGy. Radiation therapies were performed in such a way that the patient was subjected to 10 MV photovoltaic radiations in a 10×10 Cm2 area in the mastectomy region. Meanwhile, according to the depth of the tumor inside the chest, the radiation of gamma rays of cobalt 60 was used in surrounding regions. The absorption dose coefficients were the same in the TLD obtained from the photovoltaic radiation 10MV and the gamma beam of cobalt 60. Results: Then the absorbed dose as calculated by hospital dosimeters standard was compared with the absorbed dose measured by TLD. The areas determined as zero absorption doses by Hospital dosimeters standard were calculated as non-zero absorbed dose by TLD. There were also areas where the doses calculated by TLD were different from the hospital dosimeters standard. Conclusion: To reduce the possibility of side effects of additional radiation, it is suggested that in the final absorption, which is determined for the patient during radiotherapy, doses be calculated for the area outside the treated area, too.","PeriodicalId":163845,"journal":{"name":"Gastroenterology & Hepatology: Open Access","volume":"83 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"1900-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"117061847","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Bibliographic review: Celiac disease (CD) is an autoimmune disease due to intolerance to gluten, which makes it difficult for the body to absorb nutrients and water. CD has a prevalence of up to 1% of the world's population, of which approximately 300,000 Brazilians are carriers. The pathophysiology of CD occurs by an inflammatory activation that occurs mainly by immunological processes mediated by T cells, damaging the microvilli and leaving the intestine with a smooth appearance. Due to the inflammatory process, the classic picture of malabsorption generates symptoms that include diarrhea, steatorrhea and lack of appetite, as well as extra intestinal manifestations that include dermatitis, malnutrition, pubertal delay and others. Currently, it is understood that there are not many treatment options for CD, since the gluten-free diet is the only one that is effective for these patients. Therefore, adherence to such therapy is not so simple, since there are a number of social and economic difficulties that hinder this process. Final considerations: Although CD has a simple therapeutic option, it involves several intercurrences which impair adherence to treatment and can cause consequences for celiac patients. That said, promoting access to information about CD is important not only for celiac patients, but also for the general population in order to facilitate better and simplified access to gluten-free products.","PeriodicalId":163845,"journal":{"name":"Gastroenterology & Hepatology: Open Access","volume":"37 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"1900-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"121431656","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Its cause is multifactorial, its diagnosis greatly underestimated, and its prevalence increases with age. Fecal incontinence is defined as any involuntary loss of feces – liquid or solid – or gas in patients older than 4years of age. Fecal incontinence can be present in up to 15% of patients older than 70years. Only 10 to 30% of symptomatic patients seek medical attention. Up to 40% of symptomatic women have major fecal incontinence with even greater impact on quality of life. Even if missed, it remains so small a number and for fear of negative publicity, it goes unpublished. It is felt that, missing an abdominal injury must be having an altered final outcome on the patients. With this question in mind, the old data, kept prospectively was analysed, with an aim to find out the prevalence of missed abdominal injury, why were the injures were missed and how to deal with them early and effectively so as to have a possible positive change in the final outcome. Materials and methods: It was a retrospective interventional study from Jan 2002 to May 2017, where cases of blunt abdominal injuries were included and war/ insurgency related gunshot, penetrating and bowel injuries were excluded in two large military hospital in peace locations. Only cases with blunt injuries including the transferred patients from the other hospital were included in the study, where the injuries were missed at three level, namely clinically unsuspected, at CT scan and at laparotomy. Result: There were 72 patients with blunt abdominal injuries. There were 67 males and five females with age range from 8- 72 years (mean 30.70 SD13.759).Forty-three (43) patients were managed by observation and 18 patients were operated. Twelve (12) injuries were missed mostly due to, not suspecting an injury, un-conscious patient with severe head injury and misinterpretation of CT images. Six patients underwent laparoscopy. Only one patient, a laparotomy was avoided. There were three mortalities from the missed organ injury group. Conclusion: Missing an injury is presumed to be relatively uncommon. Mesenteric and isolated pancreatic injury are most often missed. Diagnostic laparoscopy confirms mesenteric injury and saponification of fat suggest pancreatic injury and duodenal injury even missed at laparotomy, when done very early.","PeriodicalId":163845,"journal":{"name":"Gastroenterology & Hepatology: Open Access","volume":"446 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"1900-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"127216814","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

In contrast to standard proton pump inhibitors, vonoprazan does not require acid activation or any time constraints for its administration. As a result of its rapid onset and prolonged action, vonoprazan is a suitable option for PPI. In this study, we compare the efficacy of vonoprazan and PPIs in treating Gastroesophageal Reflux Disease (GERD), H. pylori, and post-endoscopic submucosal dissection (ESD) bleeding and ulcers. A PRISMA-guided systematic review was conducted on Pubmed. It included English or Portuguese articles published in the last five years describing adult patients treated with vonoprazan and/or PPI for GERD, H. pylori, or post-endoscopic dissection of the gastroesophageal submucosa bleeding and ulcers. There were six articles on GERD. Half suggested vonoprazan was superior to PPIs, while the other half considered it non-inferior or of comparable efficacy. There were 19 articles on H. pylori eradication, and vonoprazan outperformed PPI in 68,42% (13/19) of them. Vonoprazan was more effective than PPIs in 8 of the 13 studies chosen for post-ESD and ulcer prevention. As a result, in most studies, vonoprazan is either superior or not inferior to PPIs. However, the racial bias of the results is a significant limitation, as many of these studies were conducted in the Japanese population. More research on greater racial diversity is required. Nonetheless, in the current scenario, vonoprazan is a potential alternative pharmacological treatment for post-ESD bleeding and ulcers, GERD, and H. pylori.","PeriodicalId":163845,"journal":{"name":"Gastroenterology & Hepatology: Open Access","volume":"485 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"1900-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"116029343","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
