As this portion of the population grows, chronic diseases also increase, with a resulting increase in demand for health services, which, in turn, can generate scarcity and/or constraints on resources. The diseases that affect older adults are mostly chronic and multiple. Hospital admissions become more frequent and bed occupancy time is longer than in other age groups. It comprises a large data set with over 17,000 patients over a 4 year period from February 2016 to February 2020. The intermediate care team is able to support the older person at home in clinical crises or in need of social support thus avoiding hospital admissions; this is done through daily visits and geriatricians review via daily virtual ward rounds. Hospital admission avoidance (HAA) and re-admission rate at 30 days (HRA30) has been in excess of 80% and less than 10% respectively. Clinical governance was shared between geriatricians and patients ‘General Practitioner. The service is popular with General Practitioners and with patients. {"title":"Using the metabolism model of GH-Method: math-physical medicine to study geriatric concerns","authors":"Gerald C. The author has been living with severe type 2 diabetes for 25 years and has suffered five cardiac episodes, renal complications, retinal problems, bladder infections, foot ulcer, and thyroid issues. Therefore, he started to self-study diabetes since 2010. In 2014, he utilized his learned medical knowledge of metabolic disorders with finite-element modeling technique of structural and mechanical engineering and advanced mathematics to develop a mathematical metabolism model. This model contains 10 categories (Figure 2: body output: weight, glucose, blood pressure, lipid; body input: food, exercise, sleep, stress, water drinking, routine life patterns) and ~500 detailed elements to calculate two dynamic variables, metabolism index (MI) and general health status unit (GHSU). Based on continuous data input, an APP on the smartphone can show both MI and GHSU continuously and dynamically on the screen to inform patients about their general health status, especially endocrine-related chronic disease conditions through body outputs (Figure 3). This is a cross-sectional study with a descriptive-exploratory quantitative approach, with simple random sampling, carried out in Tomar do Geru/SE/Brazil with 298 elderly people. Data collection was performed using the Vulnerability to Abuse Screening Scale (VASS), the items that make up the instrument seek to identify the phenomenon from four domains. Responded as an affirmative about the vulnerability domain, 86.69%; dependency, 28.19%; discouragement 79.31%; coercion, 65.66%. Introduction: The use of interventional medical technology in cardiac and terminally ill elderly patients needs to be associated with palliative care as clinical support measures in intensive care. This resource assists in decisions at the end of the patient's life and directs the attention of the health team to ensure patient comfort and family satisfaction. In this regard, information and communication technology assist the entire care process. Method: It is a descriptive and exploratory study of quantitative and statistical character. Based on the innovative methodology of the current times, using uniform and ethical parameters for the elaboration of assistance based on auxiliary technology and the knowledge of medical and nursing professionals. Results and Discussion: 99 articles were analyzed, discussed and quantified, showing experiences of palliative care in intensive care units using the variables of technology and cardiologic instruments and the participation of medicine and nursing. Conclusion: The theme “palliative care” should be further investigated in order to improve the relationship between patients, their families and the health team.
