CS often presents with mirror-image dextrocardia and other cardiac abnormalities, making surgical management of CS challenging. The use of VA-ECMO and an intra-aortic balloon pump (IABP) to address CS combined with dextrocardia has not been reported previously. In this case report, we summarize our experience treating perioperative CS using VA-ECMO and IABP in a male patient with dextrocardia combined with coronary artery disease (CAD). During off-pump coronary artery bypass (OPCAB), the patient developed CS and underwent emergency transfer to on-pump coronary artery bypass (ONCAB). The patient then received ECMO and an IABP, which successfully restored hemodynamic function. {"title":"Primary Extranodal Non-Hodgkin Lymphoma of Cheek","authors":"","doi":"10.33140/ijcmer.02.06.04","DOIUrl":"https://doi.org/10.33140/ijcmer.02.06.04","url":null,"abstract":"Non-Hodgkin’s lymphomas (NHL) comprise a group of extremely diverse lymphoproliferative neoplamas that have a predisposition for extranodal sites. Amongst the extranodal locations rare involvement of oral cavity has been reported and occurrence of these neoplasms on the cheek are thin on the ground. The most commonly encountered is the Diffuse large B-cell lymphoma. The B-cell neoplasms are clonal tumors of mature and immature B cells and the relatedness of B-cell lymphomas to normal B-cell counterparts forms a key aspect in their nomenclature of classification. Diffuse large B-cell lymphoma, not otherwise specified (DLBCL, NOS), consists of medium-sized to large B cells with a diffuse growth pattern. This lesion is a morphologically and molecularly heterogeneous entity which does not meet the diagnostic criteria of any specific large B-cell lymphoma neoplasms. The mechanism of fibrosis is based on the activation of collagen, nuclear factor kappa (NF-kβ) activation, and inhibition of ne-crosis, apoptosis, caspase-3 and proliferation. The HepG2 cell line is an evolved human liver cancer cell that exhibits a human hormone response. Hepatocyte stem cells have antifibrotic, anti-inflammatory, and im-munomodulatory properties, can suppress apoptosis and necrosis, and promote proliferation. Methods: This is an experimental in vitro study using pre-test and post-test. Hepatocyte stem cells were obtained from the neonatal umbilical cord. The cell lines were divided into four groups. Analysis was performed on below variable including caspase-3, NF-kβ, necrosis, apoptosis, proliferation and cytopathological capacity. Results: Cytopathological studies of the hepG2 cell line showed fibroblast growth. Mesenchymal stem cells reduced NF-kβ activity, but not caspase 3 activity. Mesenchymal stem cells also significantly reduced apoptosis and necrotic activity, but silymarin was not significantly affected. Mesenchymal stem cells also significantly increased proliferation (p <0.001), but silymarin was not significantly affected. {"title":"Gender Influence on Changes of Coagulation Factors in Fresh Frozen Plasma at Kisii Teaching and Referral Hospital","authors":"C. O. Increased risk of venous thrombosis has been described to be associated with high plasma levels of several coagulation factors. Objective: The objective was to evaluate gender influence on changes of coagulation factors in fresh frozen plasma at Kisii Teaching and Referral Hospital. Methodology: This study was a longitudinal study involving time series analysis of fresh frozen plasma stored at -18°C for five weeks. A sample of 180 ml plasma was obtained from the blood centrifuged at 4000rpm which was aliquoted into three parts each containing 60ml. The first aliquot was used to assess the changes in coagulation factors in FFP at baseline during the first week of sample collection, the second aliquot was used to assess the changes in coagulation factors in FFP storage at -18°C temp after three weeks of storage, the third aliquot was used to assess the changes in coagulation factors in FFP storage at -18°C temp after five weeks of storage. Coagulation factor analysis was performed using Erba Mannheim ECL 105 coagulation analyzer, India factor results recorded. Thawing for subsequent coagulation factor analysis and serial testing of stored cryoprecipitate and fresh frozen plasma was done using Stericox Plasma Thawing Bath at 37°C, for 45 mins before before analyzing the samples. Standard storage conditions for the aliquots were monitored and maintained to ensure homogeneity. Results: The findings showed no significant association between gender and FFP with p-values for three sampled weeks of the study with [0.333, 0.345 and 0.940] respectively greater than 0.05 standard alpha values. This means that, the coagulation factors in FFP were not affected by the gender of the blood donor. To determine the impact on the intervention, data was acquired through a baseline and end-of-project review. A quasi-experimental design was used in this investigation, which included a baseline and end line survey with the same variables. The research was carried out in two geo demographic groups of districts including plain land districts with easy access to services, such as Tangail and Khulna, and hard to reach locations, such as Netrakona, Cox's Bazar, and Bandarban. Program impact on uptake of maternal and child health services, as defined by the health services/behaviors for the index pregnancy, was analyzed using difference-in-difference analysis. All programs that provided information and counseling to women and their families, often in conjunction with other program measures, increased the number of women who received ANC from a medically certified person during their pregnancy significantly. The community-based programs also resulted in a significant increase in the use of neonatal and child health services. It is due to severe neuroinflammation with reestablishment of immunocompetence. Approximately 25% of patients with HIV and PML develop IRIS. Clinical trials on the treatment of PML-IRIS is lacking with only limited evidence supporting the use of investigative therapies such as corticosteroids and Maraviroc. We report a case of a 36-year old Filipino male, known hypertensive and diagnosed with HIV, who presented with memory problems. He was eventually treated as a case of PML-IRIS on the 3rd week after initiation of ART. He was treated with a course of intravenous Dexamethasone, oral Mefloquine and intravenous immunoglobulin, and ART was continued. Research results for patient 1 compliance need oxygenation in bronchopneumonia patients is fulfilled whereas in patient 2 with fulfillment need oxygenation in bronchopneumonia patients is not fulfilled. One important aspect of QS is the production and detection of diffusible signal factors (DSFs), which are small molecules that can diffuse across bacterial membranes and trigger various responses. DSFs have been shown to play a crucial role in regulating a wide range of biological processes, including biofilm formation, virulence, and antibiotic resistance. In recent years, researchers have started to explore the potential of DSFs in biocontrol applications, where they can be used to interfere with the communication and coordination of pathogenic bacteria. This review discusses the role of DSFs in QS and how they can be utilized to develop improved biocontrol techniques. We also discuss the various strategies that have been developed to interfere with DSF-mediated QS, such as using DSF analogs or enzymes that degrade DSFs. We highlight some of the promising applications of DSFs in biocontrol, including their use as biocontrol agents as part of plant microbiomes. Methodology: A documentary, retrospective and meta-analytic study was carried out with a non-probabilistic selection of indexed sources published from 2020 to 2023 and search with the key words. Results: Prevalence of data that sustain the non-exposure to risk events derived from mobility policies in public transport safety and the health of users. Discussion. Political comparisons reveal the prevention of the effects of risk events on the health of users.
