Given the known prevalence changes in other disorders which involve hypersensitivity, KD trends are of interest.Design and setting: Analysis of an administrative hospitalization database between 1990-2009 in New York was performed to determine changes in Kawasaki disease hospitalization rates and assess therapeutic interventions and clinical manifestations. For trend comparisons, hospitalization rates were also examined in 2 other disorders with hypersensitivity components: asthma and anaphylaxis.Patients: De-identified inpatient recordsMain outcome measures: Hospitalization rate changes and associated clinical features over timeResults: There was a significant increase in KD hospitalizations over the study period (negative binomial regression, p<0.0001). In patients under the age of 5 (78% of all patients), the hospitalization rate was 14.6 per 100,000 in 1990. By 2002, the hospitalization rate reached an apex of 22.5 per 100,000. This trend differed from asthma and anaphylaxis hospitalizations, which showed an overall decline, and an overall continued increase, respectively. In patients who were Asian, Pacific-Islanders, Native Hawaiian or Native Americans, there was a greater increase in KD hospitalization rates than in other patients (negative binomial regression, p=0.003). Cardiac abnormalities were coded in 6.1% of hospitalizations. Four deaths were reported, only one of which was a likely cardiac death.Conclusion: In New York State, there have been unique dynamic changes in KD hospitalization rates, which are more accentuated in the Asian population. KD continues to be an uncommon but significant disorder which results in hospitalization for young children. BACKGROUND & INTRODUCTION Kawasaki disease (KD) is a self-limited vasculitis and the leading cause of acquired heart disease in the United States among children less than 5 years of age (1). It has a particular predilection for Asian populations, which is evidenced by higher hospitalization rates for this disorder in Japan, Taiwan and Hawaii (2-4). Although the etiology of the disorder is unknown, it is felt to be most likely secondary to infection and/or an altered immunologic response. The former is suggested by the fact that KD occurs in epidemics, is self-limited, and has a seasonal presentation. The later is evidenced by the response to immunomodulatory treatment with intravenous immunoglobulin (5). As there have been dynamic changes in the incidence other diseases that relate to hypersensitivity, such as asthma and anaphylaxis (6-7), it is of interest to examine whether the KD incidence has also changed. Moreover, in this study we examined a period of time and geographic area where there have been shifts in the racial proportions in a populous American state. Wheat","doi":"10.5580/2baa","DOIUrl":"https://doi.org/10.5580/2baa","url":null,"abstract":"The efficacy and safety of an oral standardised lipid extract of New Zealand green lipped mussel (Perna canaliculus) marketed as Lyprinol ® was assessed as maintenance therapy for children with moderate asthma. A total of 71 children aged 6 to 13 years were enrolled in a 16-week, single centre, double-masked, placebo-controlled, parallel-group trial and randomly assigned to receive either Lyprinol or placebo (2 capsules twice daily). Patients were maintained on as-needed beta-agonist therapy and inhaled corticosteroid (ICS) throughout the study. Results. Lyprinol improved the percentage of children reporting little or no trouble with their asthma at three months of treatment (97% vs. 76% p=0.057). Both groups were able to tolerate a dose reduction of ICS. There were fewer mild and moderate asthma exacerbations overall in the Lyprinol group. Lyprinol was well tolerated. This immediately triggers increased patient surveillance due to the possibility that the lactate may signify inadequate delivery of oxygen to the tissues that results in a lactic acidosis type A. Type A lactic acidosis may be related to septicemia or heart failure as very common factors and intensive care unit management would be a consideration. Alternatively, it may simply be due to the more benign type B2 lactic acidosis that is now commonly seen in asthma (1,2,3,4). Rajanna","doi":"10.5580/2aca","DOIUrl":"https://doi.org/10.5580/2aca","url":null,"abstract":"This study was done to assess the safety and efficacy of rush immunotherapy (RIT), and was conducted in the Allergy Centre of Kempegowda Institute of Medical Sciences Hospital & Research Centre, Bangalore, India. Twenty patients in the age group of 18-50 years were included, of which thirteen patients had allergic rhinitis, and seven patients had allergic rhinitis with asthma. All of these patients were positive for skin prick test for house dust, dust mites, tree pollens, grass pollens, and weed pollens, either singly or in combination. They received RIT as per the protocol. Among these patients, the safety of RIT was assessed based on reactions during RIT and efficacy was assessed by estimating total serum IgE and IgG at baseline, after two weeks, and after six weeks. Local reactions were observed in five patients, which subsided without any medication. Systemic reaction was observed only in one patient following a 9 injection on Day 2. This patient responded satisfactorily to Inj. Chlorpheniramine maleate, Inj. Hydrocortisone, and Oxygen inhalation and completed RIT. Late systemic reactions were not noted in any of the patients. Estimation of total serum IgE showed a significant reduction and total serum IgG showed a significant increase after six weeks. It was concluded that rush immunotherapy was tolerated by most patients with a systemic reaction rate comparable to conventional immunotherapy. All patients were able to reach maintenance dose months sooner than weekly schedules. Most of the time, clinical judgment is adequate for diagnosis. But in some, especially early and borderline cases of leprosy histopathology, the study of epidemiological and demographic factors can be important.Method:The aim of the present study is to study epidemiological and demographic factors among patients of leprosy, to describe the clinical pattern of the disease, and to correlate clinical diagnosis with histopathological findings with AFB (acid-fast bacilliM. leprae) status. A total of 100 cases were studied. A detailed history was taken, and a complete examination of the patient was carried out, including general, local and systemic examinations. The biopsies were taken and stained using the routine haematoxylin-eosin method and modified Fite-Faraco stain for detection of acid-fast bacilli. The histopathological diagnosis was based on the scheme put forth by Ridley and Jopling.Results:The study shows that 34 patients (34%) belonged to a sexually active age group i.e., 21-30 years (Table-1). The highest number of cases (29%) were clinically diagnosed as borderline tuberculoid (BT), followed by tuberculoid (TT) (24%), borderline leprosy (BL) (21%), lepromatous leprosy (LL) (20%) and the fewest cases were mid-borderline leprosy (BB) (6%). 26% of patients were histologically diagnosed as TT and BT, followed by BL (25%), LL (20%) and BB (03%). Modified Fite-Faraco stain for AFB in skin biopsy were negative in TT (26%), BT (26%) and BB (03%) cases. 21 out of 25 cases of BL and 19 out of 20 cases of LL were positive for AFB.Discussion:This age difference may be due to differences in exposure, opportunities for infection and immunological differences in children and adults. Many medications, diseases and infections have been implicated in the development of leukocytoclastic vasculitis. A review of available literature reveals case reports of leukocytoclastic vasculitis induced by medications such as infliximab, rituximab, etanercept, cocaine and numerous others and infections such as HIV and Streptococcal pneumonia as well as malignancies and chronic inflammatory conditions such as ulcerative colitis and collagen vascular diseases. Leukocytoclastic vasculitis induced by food has also been reported. Here, we report an interesting case of cutaneous leukocytoclastic vasculitis that developed in a patient after months of consuming a fast food diet for breakfast, lunch and dinner, which would improve only after abstaining from such a diet. Currently the combination of peg interferon and ribavarin is considered the standard treatment of HCV infection 2 . Although combination of these two drugs is most effective but it is also associated with increased risk of side effects due to immunomodulatory effect of both the drugs 3 . Dermatological side effects in the form of skin rashes, dryness, pruritus, pyoderma gangrenosum, alopecia, toxic epidermal necrolysis, new or exacerbation of psoriasis, induration and ulceration at the injection site can occur 4 . On the other hand various skin disorders such as lichen planus, necrotising vasculitis or porphyria cutanea tarda are more commonly associated with hepatitis C infection 5 . We as dermatologist are referred cases with cutaneous manifestations from various specialties and should be aware whether these changes are due to disease itself or its treatment. We observed 30 HCV positive and HIV negative cases with dermatological manifestations (Table-1) on combination treatment with Ribavarin and peginterferon who came to our OPD over a period of two years for skin opinion. Out of those 30 cases, 28 patients were males and only 2 were females. They were in the age range 35 to 55 years. In this article we decided to search new entities reported electronically in Pubmed and EBSCO indexes in the last decade after year 2002. Twenty-six new entities were found out after our investigation. In this review, the reported entities have been classified according to the sections of dermatologic disorders.
