There is a correlation between anaemia worsening and kidney function decline, showing an increased risk of progression to end-stage kidney disease (ESKD) in anaemic patients. The aim of this study is to evaluate the effect of Ferric Sodium EDTA, in combination with vitamin C, folic acid, copper gluconate, zinc gluconate and selenomethionine (Ferachel forte®) treatment in nondialysis-dependent chronic kidney disease (NDD-CKD) elderly patients with secondary anaemia not responders to ferrous sulphate therapy, in comparison with an oral liposomal iron formulation. Methods: We enrolled thirty patients in our Geriatric Clinic of the S. Giovanni di Dio Hospital, Fondi (Latina), Italy. All patients have been treated for 6 months with ferrous sulphate prolonged-release tablets 1 tab/day, containing 105 mg of ferrous ion (T0). After evaluation of blood parameters at T1 (after 6 months) patients were randomized to treatment with Ferric Sodium EDTA in combination with vitamin C, folic acid, copper gluconate, zinc gluconate and selenomethionine (Ferachel Forte®) 1 tab/day, containing 30 mg of ferric ion (Group 1; N=15), or with Ferric liposomal formulation, containing 30 mg of ferric ion (Group 2; N=15), for 6 months (T2). Iron blood parameters of haemoglobin (Hb), sideremia, ferritin, and total transferrin saturation (Tsat) have been recorded at T0, T1 and T2. Results: At T1 time the mean results of blood parameters evaluated in patients treated with ferrous sulphate did not show a marked improvement of anaemic disease. After shifting to different therapies (Group 1 and Group 2) for 6 months both groups of treatment reported statistically significant improvements of iron blood parameters of Hb, sideremia, ferritin and Tsat. The results obtained allow us to confirm the real superiority of the oral administration of Ferric Sodium EDTA, in combination with vitamin C, folic acid, copper gluconate, zinc gluconate and selenomethionine (Ferachel forte®) in improving blood parameters of Hb, sideremia, ferritin and Tsat, in comparison to the other formulations used in this preliminary study. Conclusions: This study confirmed the efficacy and the safety of Ferric Sodium EDTA, in combination with vitamin C, folic acid, copper gluconate, zinc gluconate and selenomethionine (Ferachel forte®) treatment in NDD-CKD elderly patients with secondary anaemia not responders to ferrous sulphate therapy.","PeriodicalId":160607,"journal":{"name":"Nephrology and Renal Diseases","volume":"1 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"1900-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"125360876","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Deposition of amyloid fibrils derived from circulating acute-phase reactant serum amyloid A protein causes systemic amyloidosis, a serious inflammatory disorder. Study design and methods: We document a male patient who developed reactive amyloidosis (AA type), most likely secondary to his longstanding periodontitis. A careful dental and periodontal examination was performed. Acute-phase reactants were measured at baseline and at 4 to 6 weeks after completion of periodontal therapy. Intra-oral and renal biopsies were performed. Results: The mean probing depth was 8.20 ± 0.74 mm. The serum inflammatory markers improved more dramatically at the second week of periodontal therapy than any other time intervals. Conclusion: Periodontitis may be an additive factor to traditional etiological factors for systemic AA amyloidosis as well as being a primary etiological factor. There, treating or preventing periodontal diseases may alleviate disease burden or may even prevent the development of systemic amyloidosis.","PeriodicalId":160607,"journal":{"name":"Nephrology and Renal Diseases","volume":"42 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"1900-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"133929249","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Methods: Two groups of patients, one on HHD and one on ICHD were compared. Both groups consisted of 9 patients, age between 50 and 60 years, similar dialysis age and basic nephropathy. All patients were given two questionnaires: RAPA test (Rapid Assessment of Physical Activity) for assessing levels of physical activity and the World Health Organization Quality of Life bref test. Results: The Rapa test showed a sedentary lifestyle in the majority of patients in the two groups: only 1 pt on ICHD reached 5 points and 2 pts on HHD 7 points. 3 pts on ICHD did not practice any minimum activity and only 1 pt on HHD group (p<0.05).The quality of life test showed that 2 pts on ICHD did not reach the minimum value of 60 points, while only one pt on HHD. The mean score was higher in the HHD group (80 ± 12) vs ICHD (73 ± 13). In addition, the ICHD pts had, during the 12 months of observation, 3 hospitalizations for various clinical reasons, while the HHD pts had only one hospitalization (p<0.05). Conclusion: Our study confirms the need to set up an adapted physical activity programme for all dialysis patients. Programme that contributes to improve the quality of life of patients on ICHD and patients on HHD, even if the latter seems to have a better score deriving from a minimal physical activity and a management of the disease.","PeriodicalId":160607,"journal":{"name":"Nephrology and Renal Diseases","volume":"63 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"1900-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"132188657","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Methods: Management of the research data for this study involved collection, entry, processing, storage, retrieval, archival, distribution and documentation of information collected according to a written protocol. Efficacy and safety were measured using a Fisher’s exact test with a two-tailed p-value. A p-value of less than 0.05 was considered significant. Study population : An EPIC report was obtained for hemodialysis patients that received heparin 5000 units Sub-q every 8 hours or every 12 hours from March 13, 2014 to April 28, 2020. 4303 patients were included, and 273 patients were assessed for eligibility. 214 patients were analyzed. Results: The primary endpoint–vascular event, ischemic event, death related to coagulopathy, anaphylaxis, hyperkalemia, hypertriglyceridemia, heparin induced thrombocytopenia (HIT), or sepsis in patients receiving heparin who were on HD occurred in 12 of 107 patients (11.2%) in the heparin 5000 units Sub-q every 8 hours group and 10 of 107 patients (9.3%) in the heparin 5000 units Sub-q every 12 hours group (p=0.676). We have shown in proteinuric diseases that miRNA133a downregulates MRP2 by binding to its 3`UTR region, inducible by endothelin-1 (ET-1) via the ETB-receptor. The mechanism of nephrotoxicity in CyA treatment is still elusive. In particular, co-morbidities/clinical parameters, the morphological extent of MRP2 damage and the involvement of miRNA133a as a potential mechanism leading to nephrotoxicity have not been investigated. Aims: To analyze co-morbidities, and the role of ET-1 and miRNA133a in CyA renal toxicity. Methods: The effect of ET-1 alone or in combination with CyA on MRP2 and miRNA133a levels was studied in human proximal tubular cell culture. Comorbidities/clinical factors in 230 biopsies from 124 patients were analyzed. Immune histology and morphometry of MRP2 expression was performed in 36 CyA biopsies (tubular damage, arteriolar damage, no damage) and in controls (5 null biopsies, 5 probes of normal renal tissue from benign tumor resection). On those specimens, a qRT-PCR for the detection of mrp2 and miRNA133a levels was also investigated. Results: No single clinical parameter was statistically found responsible for CyA nephrotoxicity. Morphometric analysis revealed an increasing degree of MRP2 loss, i.e., no CyA toxicity to arteriolar / combined arteriolar, and tubular toxicity which showed the maximum degree of damage. In human proximal tubular cell culture, the combination of ET-1 and CyA had a higher effect of MRP2 loss/miRNA133a upregulation than ET-1 alone. By qRT-PCR, CyA induced marked tubular damage by downregulating the mrp2 gene via the upregulation of miRNA133a. However, transcriptional levels for mrp2 in arteriolar damage were high. Conclusion: Downregulation of MRP2 by miRNA133a is an important mechanism of CyA tubular nephrotoxicity. Urinary analysis as detection assay and treatment options by an ETB-receptor antagonist are discussed.","PeriodicalId":160607,"journal":{"name":"Nephrology and Renal Diseases","volume":"46 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"1900-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"115755582","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The pathophysiological mechanisms of the genesis of cardiac involvement, both acute and chronic, are still not completelly understood. However, in experimental models, a relationship was observed between the levels of parasitemia and the high intensity of acute myocarditis with the severity of cardiac symptoms during the chronic phase. Patients with congestive heart failure may present with renal insufficiency and anemia. This triad is denominated the cardio-anemic-renal syndrome. Our results demonstrate that in the early stages of infection there is a transient but marked renal disfunction in mice infected with T. cruzi. In addition, interference in renal/cardiovascular axis, such as the renin angiotensin-aldosterone system block, decreases the mortality of the animals around 30%. Kidney damage during the infection with T. cruzi is underestimated. Experimental results showed the pathophysiological interconnections with the evolution of acute heart failure. We hope from the results compilation to suggest new diagnostic and therapeutic protocols that minimize cardiac symptomatology during the acute phase and the onset of dilated cardiomyopathy during the chronic phase. *Correspondence to: Gabriel Melo de Oliveira, Lab.
