Chavan, Oso Bolanle Idowu","doi":"10.18535/cmhrj.v3i3.170","DOIUrl":"https://doi.org/10.18535/cmhrj.v3i3.170","url":null,"abstract":"The onset and progression of diabetic nephropathy represent a significant issue in diabetic management as it is the primary microvascular consequence of both type 1 and type 2 diabetes mellitus and a significant contributor to end stage renal disease-related mortality. Major risk factors for nephropathy in children and adolescents include hyperglycemia, HbA1c, diabetes duration, gender, blood pressure, dyslipidemia, uric acid, family history and genetic factors, smoking, puberty, and obesity. Metabolism and hemodynamic variables interact on a complex basis during disease development. A series of intricate molecular events take place in response to hyperglycemia, which disturbs the body's metabolic environment and causes glomerular enlargement, tubular inflammation, mesangial expansion, oxidative stress, and renal fibrosis. Fortunately, the main approaches for preventing the beginning and reducing the progression of diabetic kidney damage are rigorous glucose control and antihypertensive medications. As children and adolescents with T1D are at risk for developing early diabetic nephropathy, patients with T1D must have the best metabolic control, an early diagnosis, and timely treatment of dyslipidemia and hypertension. Prospects for better diabetic nephropathy outcomes are improving as novel approaches are developed. Novel biomarkers are increasingly proven to be more reliable instruments than the traditional microalbuminuria, which can forecast the development of disease. In addition to glomerular and tubular indicators, inflammation and oxidative stress markers have proven to be reliable diagnostic aids. {"title":"Ventricular Fibrillation in Acute Coronary Syndrome with Elevated ST Segment: Incidence and Predictive Factors.","authors":"Hanane Zouzou","doi":"10.18535/cmhrj.v3i3.168","DOIUrl":"https://doi.org/10.18535/cmhrj.v3i3.168","url":null,"abstract":"Background: Ventricular fibrillation is the principal cause of sudden cardiac death; in acute coronary syndrome (ACS) with elevated ST segment, its incidence and predictive factors, have been the subject of several international studies; however, its epidemiological data is lacking in Algeria.\u0000Aims: The main objective of our study is the determination of the frequency of ventricular fibrillation in acute coronary syndrome with elevated ST segment, the secondary objective was the analysis of predictive factors of this arrhythmia, and related mortality.\u0000Methods and materials:  In this prospective study, conducted in the cardiology department of Hussein Dey hospital (Algiers-Algeria), 467 patients with acute coronary syndrome with elevated ST segment (87 women and 380 men) were enrolled between 28 February 2014 and 16 July 2015. The average age is 60 ± 13 years; at admission, a Holter recorder was attached for continuous ECG monitoring during 48 hours\u0000Kruskal’s ANNOVA or H tests were used for comparison of quantitative variables, χ2 test or Fisher’s exact test, were used for qualitative variables, all tests were performed with 1st species risk of 5%.\u0000Results:  The frequency of ventricular fibrillation is 5.8% (27 patients), CI 95%: [3.7%-7.9%], multivariate analysis identified the following independent predictors: low diastolic blood pressure: high ST segment elevation, large diastolic left and right ventricular diameters.\u0000The risk of mortality expressed by Hazard Ration (HR) is 12.9 (CI 95%: [4.9-34], p < 0.001), the predictors of mortality are right ventricular failure and failure of thrombolysis.\u0000Conclusion: Ventricular fibrillation is the principal cause of sudden cardiac death, during acute coronary syndrome with elevated ST segment, its predictive factors according to our study are: low diastolic blood pressure: high ST segment elevation, large diastolic left and right ventricular diameters, its occurrence increases the risk of hospital mortality, this high risk is partly explained by the therapeutic strategy.","PeriodicalId":155428,"journal":{"name":"Clinical Medicine And Health Research Journal","volume":"61 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-05-03","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"132195429","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

It probably invades the biological width, during rubber dam isolation, impression taking and adhesive procedures. This article proposes a literature review of a new minimally invasive technique known as cervical margin relocation which allows to relocate subgingival margin to supragingival level using an appropriate material. A literature search was conducted in the Medline/PubMed database following the main key words: “cervical margin relocation», «deep margin elevation”, “proximal box elevation”, “indirect restorations”, “adhesion”, “marginal adaptation”, “subgingival margin”. A total number of 44 articles were included.\u0000Based on the reviewed literature, it can be concluded that cervical margin relocation technique emerged as an interesting alternative to surgical crown lengthening. The Mini-Nutritional Assessment tool was used to investigate the nutritional status and dietary practices. It was identified by using weekly food consumption and 24 hours dietary recall questionnaires. Data were entered in Epi-Data (version 3.1) and analyzed by SPSS (version 20).\u0000Result: Among 202 respondents (101 each from community dwellings and old age home), 44.5% were at risk of malnutrition, 5.9% were malnourished and 49.5% had normal nutrition status in community dwelling. Similarly, 55.4% were at risk of malnutrition, 17.8% were malnourished and 26.7% had normal nutrition status in old age home. In both settings, consumption of protein is low and consumption of vegetables is high. There was a significant association between socio-demographic variables such as living settings, age and education with nutritional status of elderly population. Age (AOR =0.0.186, p= 0.01) and education (AOR = 5.419, p= 0.006) shows significant association with nutritional status.\u0000Conclusion: Overall half of them were at risk of malnutrition in both settings due to poor dietary practices, loss of appetite, weight loss, morbidity, neuropsychological problems among senior people. Cardinal focus should be given to improve the consumption of protein in dietary practices. With In-vitro diagnostics, in order to evaluate the early existence of a disease, classify and quantify toxic molecules, tumeor cells or infectious diseases, nanotechnologies may improve the efficiency and sensitivity of diagnostic approaches by means of fluids such as saliva or tissue samples in which selective nanoscales perform multiple analyses on the subcellular scale. In the dental interest, the high mortality rate of oral squamous cell carcinoma is often attributed to difficulties in detecting the disease at an early treatable stage. In recent years, the ability of gold nanoparticles subjected to surface plasmon resonance capable of causing an optical contrast to discriminate between cancer and normal cells, and their conjugation with antibodies, has been explored, allowing the expression of relevant biomarkers to be traced for molecular imaging in the reflectance confocal microscope, demonstrating its potential for cancer detection in reflection based imaging systems on the basis of biomolecular changes.\u0000Local anaesthesia: Local anaesthesia is one of the most common dental procedures. However, despite being frequently present in ALs, the impact of survivin transcription in this type of hematologic malignancy remains undefined. Some studies report that survivin predicts an unfavorable clinical outcome in ALs, while others do not. In the present study, despite the small number of patients evaluated, the transcription of survivin at diagnosis did not seem to be related to a worse response to treatment or a worse prognosis. This is because, of the six patients who expressed survivin, three are in complete remission and, of the three who died, two were already considered to have an unfavorable prognosis regardless of survivin transcription, one due to advanced age and the presence of leukocytosis in the liver. Leukometry at diagnosis was another prognostic factor evaluated in this study. Radomir","doi":"10.18535/cmhrj.v3i2.135","DOIUrl":"https://doi.org/10.18535/cmhrj.v3i2.135","url":null,"abstract":"Background: The negative impact of medication overuse headache (MOH) on the quality of life (QoL) of the patient is undoubted.\u0000Objective: The aim of this study was to identify clinical and headache related parameters that directly affect HRQoL of MOH patients.\u0000Patients and Methods: 183 patients (111 men and 72 women) firstly diagnosed as having MOH and 81 healthy subjects (22 men and 59 women) in control group (CG) were enrloed in this study. The age of the study subjects range from 18 to 71 years. HRQoL was assessed using a Short Form - 36 questionnaire (SF-36), measuring its Physical Composite Score (PCS), Mental Composite Score (MCS) and Total score (TS).\u0000Results: All HRQoL domeins (PCS, MCS, TS) were lower in MOH compared to the CG (p <0.001). In MOH, the depression itself is a risk factor for all aspects of HRQoL, for PCS (B = -0.70, 95% CI -1.32 - - 0.08, p = 0.027); for MCS (B = -0.71, 95% CI -1.14 - - 0.29, p = 0.001); for TS (B = -0.69, 95% CI -1.16 - - 0.22, p = 0.005)), with female gender being an associated risk factor only for PCS (B = -15.47, 95% CI -26.79 - - 4.14, p = 0.008). The most active ingredient, caffeine, could be responsible for the physiological or clinical effects of Cola nitida in humans. Such a practice could culminate in the abuse of the said substance. Caffeine might alter hormonal profiles and thereby affect menstrual function, which might have a direct bearing on fertility. Amongst other factors, testosterone and progesterone are important to the chances of fertility in the humans. Besides, it has been well documented that the probability of pregnancy is reduced by 5% per unit of body mass index exceeding 29 kg/m2 . From some published evidences of the high prevalence of infertility nowadays, the need for this study was thereby necessitated. Here we report the influence of Cola nitida on testosterone and progesterone concentrations in the normal weight humans under resting condition in Ambrose Alli University. Twenty (20) normal weight volunteers (10 males and 10 females) and non-habitual Cola nitida chewers, aged 18-28 years were used for the study. 0.5g/kg body weight of Cola nitida was administered to each subject to be chewed as a bolus. After ingestion, 50ml of water was given to each volunteer to flush the masticated Cola nitida down the gut. The subject was allowed to rest for 90 minutes. Blood sample was collected from the medial cubital vein using vacutainer syringe. The radio-immunoassay principle was used for the estimation of testosterone and progesterone levels. The results showed that Cola nitida consumption by the normal weight male and female subjects significantly (P<0.05) reduced serum level of progesterone in the females (from 16.47±039 ng/ml to 4.690±0.592 ng/ml) and serum testosterone level in the males (from 7.392±0.479 to 4.727±0.466 ng/ml). First 3-month  follow-up: no infection, no dislocation, range of motion: 0/0/900, walk no pain, knee score: 90/100. 8-year follow-up : walk, slight run no pain; range of motion: 0/0/1000; knee score  evaluated  excellent: 95/100. The ADI-R and ADOS can be used to confirm ASD. These tools have not been tested in a non-sighted population. \u0000Through our clinical case, we highlight the clinical and therapeutic complexity of visually impaired children with ASD. \u0000Case report: A., 4 years and 7 months old, was admitted to the department for a diagnostic evaluation of a communication and language disorder concomitant with congenital blindness. At birth, bilateral anophthalmia was diagnosed. The complete pediatric evaluation was normal. \u0000After profound clinical observation, the diagnosis of ASD in comorbidity with congenital blindness was retained. The management was multidisciplinary. \u0000Discussion: ASD is the most clinically noted and scientifically studied disorder in visually impaired children. Specialists find difficulties in differentiating between the original developmental symptoms of visually impaired children and the symptoms relevant to ASD. \u0000Diagnostic investigation has been based on clinical observation. Several authors have demonstrated that careful observation can differentiate social interaction disorders caused by blindness from those of ASD. \u0000The treatment of children with ASD is essentially based on visual access to information, not adapted to ASD children with blindness. \u0000Conclusion: Blindness associated with ASD is a dual handicap.
