Particularly in unstable pertrochanteric hip fractures, Proximal Femoral Nail (PFN) is often preferred for internal rigid fixation of the fracture and early mobilization of the patient. Implant failure findings such as relapse or fracture of the proximal sliding screw after PFN, fracture of the distal screw, and fracture of the PFN from the proximal or distal area are rare. We present the case of an advanced-age active female patient with an unstable pertrochanteric hip fracture who developed two proximal PFN fractures after PFN, and we document the two revisions with PFN+ graft we introduced for her. This may severely affect social and psychological status. It is known that early repair of frontal nerve is more effective than delayed repair. However, it is not well defined how the frontal nerve repairment responds to emergency coaptation. A case report of a 27 years old male whom had a right frontotemporal laceration after a trauma has been presented. The patient was operated immediately and the frontal nerve was explored under microscope. Either ends of the frontal nerve were found within the injury zone and repaired. Frontal muscle reanimation started after one week and recorded to be nearly fully gained in post-operative one month. House-Brackmann Grade VI injuryimprovedto a Grade II nervefuncitonwithin 1 monthafteroperation. {"title":"Higher Survival Advantage With Hyper-CVAD Chemotherapy Regimen Before Allogeneic Stem Cell Transplantation In Patients With High Risk Adult Acute Lymphoblastic Leukemia: Two-Center Experience","authors":"Emrah Kılıçaslan, S. Hyper-CVAD has become a widely used treatment for approximately 2 decades in the treatment of adult ALL. We conducted a retrospective analysis of 30 patients treated with Hyper-CVAD at two centers between 2014 and 2020. In all (n:30) patients treated with Hyper-CVAD, CR rate was 86.7%, induction mortality was 10%, refractory disease was 3.3%, the median OS was 38 months (95% CI 7.78–68.2 months), the median DFS was 29 months (95% CI 9–49 months), the 2-year OS rate was 56.5%, and the 2-year DFS was rate 56.7%. For standard risk (n: 12) ALL patients, the median OS was 20 months (95% CI 0–43 months), and median DFS was 7 months (95% CI 0–25 months). For high risk (n: 18) ALL patients, the median OS was 38 months (95% CI 0–76 months), and median DFS was not reached. Emotional traumas occur in children due to sequelae caused by injuries and the rest of their lives are negatively affected. The aim of this study is to investigate the etiology, epidemiology, and injury type of pediatric maxillofacial traumas, and to make recommendations to prevent injury to children. \u0000Materials and Methods: This descriptive retrospective study was conducted in a regional tertiary hospital. The medical records of all maxillofacial trauma-related admissions to the emergency department between January 2015 and February 2020 were retrospectively reviewed. Etiology of trauma, demographic characteristics, type of trauma, accident type, injury site, administered treatments, and mortality were recorded. \u0000Results: 1029 patients aged 4months-18years (mean 77.67±59.21 months) including 288 females (28%) and 741males (72%) were included in the study. Most of the patients (n = 390, 37.9%) were in the 13-18years of age group. The vast majority of the study population lived in the urban area (753-73.2%). Most injuries were seen in the summer (438-42.6%). After head+cervical injuries, the most common upper extremity injuries (171-16.6%) were seen, the least thoracic injuries (69-6.7%). 57 (5.5%) patients were required hospitalization and the mortality rate was 1.2% (n = 12). \u0000Conclusions: Pediatric maxillofacial traumas occur mostly in the male gender, in summer, in urban areas, and due to motorcycle accidents. Gereç ve Yöntemler: Trakya Üniversitesi Tıp Fakültesi, Çocuk ve Ergen Ruh Sağlığı ve Hastalıkları Anabilim Dalı tarafından “Dikkat Eksikliği ve Hiperaktivite Bozukluğu” tanısı ile takip ve tedavi edilen; 30 erkek vaka, olgu grubu olarak çalışmaya dahil edildi ve kontrol grubu ile karşılaştırıldı. Gruplar; yaş (yıl), boy uzunluğu (cm), vücut ağırlığı (kg) ve vücut kitle indeksi (kg/m2), 25-hidroksikolekalsiferol (Vitamin D), serum kalsiyum değerleri (mg/dl) ve DEXA (Dual enerjili x-ray absorpsiyometrisi) sonuçları ile karşılaştırılarak değerlendirildi. Sonuçlar: Çalımamızda büyüme parametreleri [boy(cm), vücut ağırlığı (kg), vücut kitle indeksi (kg/m2)], Vitamin D düzeyleri, lomber ve kalça kemik mineral yoğunluğu açısından gruplar arasında istatiksel olarak anlamlı fark saptanmadı. Serum kalsiyum düzeyleri, proksimal femur ve lomber vertebra Z skorlarının olgu grubunda istatistiksel olarak anlamlı düzeyde düşük olduğu tespit edildi (p=0,001; p=0,013; p=0,015). Tartışma: Metilfenidat tedavisi, DEHB olgularında altın standart farmakolojik tedavidir. Literatürde hiçbir çalışmada DEHB olgularında metilfenidat kullanımına bağlı olarak kontrol grubuna kıyasla daha iyi büyüme parametreleri ve kemik yoğunluğu saptanmamıştır. Çalışmamızda büyüme parametreleri açısından gruplar arası fark saptanmadı. Kemik parametrelerinde ise, kalça ve lomber vertebra Z skorları ve serum kalsiyum düzeylerinin olgu grubunda istatistiksel olarak anlamlı düzeyde düşük olduğu tespit edildi. Vitamin D düzeyleri açısından gruplar arası istatiksel olarak anlamlı fark tespit etmemiş olmamıza rağmen ortalama değerler açısından olgu grubumuzun Vitamin D eksikliği, kontrol grubumuzun ise Vitamin D yetersizliği tanımına uymasının önemli olduğunu düşünüyoruz. INVESTIGATION OF THE EFFECTS OF LONG-TERM METHYLPHENIDATE USE ON GROWTH AND BONE DENSITY IN CHILDREN AND ADOLES WITH DIAGNOSIS OF ATTENTION DEFICIENCY AND HYPERACTIVITY DISORDER Abstract: Aims: Our aim in this study is to investigate the effects of methylphenidate, which is the most commonly used pharmacological treatment agent, on growth and bone structure in patients with ADHD. Material and Methods: Patients with the diagnosis of \"Attention Deficit and Hyperactivity Disorder\" was followed and treated; 30 male cases were included in the study as a case group and compared with the control group. Groups; age (years), height (cm), body weight (kg) and body mass index (kg / m2), 25-hydroxycholecalciferol (Vitamin D), serum calcium values (mg / dl) and DEXA (Dual energy x-ray absorptiometry) compared with the results. Çalışma, 01 Ocak– 31 Aralık 2021 tarihleri arasında XXX Eğitim ve Araştırma Hastanesi’ne ayaktan başvuran 15-99 yaş arası hasta grubunda gerçekleştirildi. Tüm hastaların Hb, HCT, MCV, MCH, serum demir (Fe), serum ferritin düzeyleri çalışma kapsamında değerlendirildi. Anemi prevalansı, DEA prevalansı, anemi varlığının biyokimyasal ve demografik veriler ile ilişkisi istatistiksel olarak karşılaştırıldı. Çalışma kapsamında 82116 hastanın laboratuvar sonuçları değerlendirildi. Çalışmaya dahil edilen hastaların 19326 (%23.5)’inde anemi, anemi saptanan hastaların ise 2007 (%11.5)’sinde DEA saptandı. DEA tanısı alan hastaların 1734 (% 86.3)’ü kadın, 273 (%13.7)’si erkektir. DEA tanılı hastalar yaş gruplarına göre değerlendirildiğinde hastaların %44’ünün 25-44 yaş aralığında olduğu görüldü. Cilt tulumları arasında en sık eritematoz döküntüler olsa da eritema nodozum hastalık seyri sırasında görülebilir. Eritema nodozumun enfeksiyoz nedenleri arasında çok farklı etkenler bildirilmiştir. Ancak brusellozun etken olması sonucu gelişen olgular nadirdir.
