Methods Nine hundred and thirty-two medical staff in a county hospital were surveyed with use of the Hospital Survey on Patient Safety Culture. Information was analyzed by one-way ANOVA and multiple linear regression analysis. Results Nine hundred and thirty-two questionnaires were distributed, of which 661 of those returned were valid. The subscale-level results showed that the positive response rate for “teamwork across units” was higher than 75.0%, indicating it was an area of strength. Five areas – “nonpunitive response to error,” “staffing,” “communication openness,” “overall perceptions of patient safety,” and “frequency of event reporting” – had potential for improvement, with a positive responsive rate lower than 50%. Twenty-nine percent of respondents gave their work area a patient safety grade of “excellent” or “very good.” Further, 60.1% of respondents had reported no event in the previous 12 months. Multiple linear regression analysis indicated that position and number of years working in this hospital were the factors influencing patient safety culture. Conclusion Patient safety culture in the county hospital has potential for improvement, especially in the areas of “nonpunitive response to error,” “staffing,” “overall perceptions of patient safety,” “communication openness,” and “frequency of event reporting.” Statement of Significance It has been recognized for almost 20 years that safety culture is important in ensuring high-quality and safe care. This article describes the results of a patient safety culture survey undertaken in one Chinese county hospital, which distributed the Hospital Survey on Patient Safety Culture to 932 health care staff. It reaffirms that there is still a long way to go until hospitals have successfully established positive safety cultures. In terms of relevance, we believe the findings will be most useful to the hospital where the study was undertaken. Many of the recommendations in the discussion should be useful for the hospital.","PeriodicalId":152216,"journal":{"name":"Family Medicine and Community Health Journal","volume":"8 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2017-12-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"115241165","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

We aimed to assess the prevalence of and risk factors for convulsions (not occurring from fever) during pregnancy in Indian women. Convulsion is a key symptom suggestive of eclampsia. Methods Cross-sectional data from India’s third National Family Health Survey, conducted during 2005–2006 were used. Self-reported information on convulsions during pregnancy was obtained from 39,657 women aged 15–49 years who had a live birth in the 5 years preceding the survey. Multiple logistic regression analysis was used to explore risk factors for convulsion in pregnancy. Results Overall, 1 in 10 women reported having convulsions in their most recent pregnancy. The prevalence was significantly higher in women living in rural areas compared with those living in urban areas (11.3% vs. 7.4%; P<0.0001), with marked state and geographic variation. The odds of convulsions were significantly higher in women with a twin pregnancy [odds ratio (OR) 2.12; 95% confidence interval (CI) 1.45–3.11], a previously terminated pregnancy (OR 1.32; 95% CI 1.20–1.45), diabetes (OR 1.37; 95% CI 0.99–1.89), or asthma (OR 1.85; 95% CI 1.35–2.54), in women who were alerted to pregnancy complications (OR 2.78; 95% CI 2.50–3.08), in Sikh women (OR 1.73; 95% CI 1.28–2.33), in women in a low social group (OR 1.40; 95% CI 1.25–1.58), and in women residing in central India (OR 1.51; 95% CI 1.31–1.74) or eastern India (OR 1.33; 95% CI 1.14–1.54) with reference to their counterparts. Conclusion Our findings from a large population-based nationally representative sample of Indian women indicate a high prevalence of convulsions, a symptom suggestive of eclampsia, and its association with several maternal, lifestyle risk factors and sociodemographic characteristics.","PeriodicalId":152216,"journal":{"name":"Family Medicine and Community Health Journal","volume":"81 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2017-12-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"132452805","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Methods A randomized, double-blind, controlled intervention trial evaluated the effect of nutritional supplementation on the neurocognitive outcomes of 49 HIV-affected school-age children in western Kenya. The intervention consisted in providing the mother, target child, and siblings with one of three isocaloric biscuit-type supplements – soy, wheat, or beef – on 5 days per week for 18 months. Neurocognitive outcomes of the target children were assessed by a battery of eight measures and followed up longitudinally for up to 24 months. Results Mixed effects modeling demonstrated significant differences in the rates of increase over time among all three groups (F test degrees of freedom of 2, P<0.05) for Raven’s progressive matrices performance, but not for verbal meaning, arithmetic, digit span backward, forward, and total, embedded figure test, and Beery visual-motor integration scores. Conclusion HIV-affected school-age children provided with soy protein supplementation showed greater improvement in nonverbal cognitive (fluid intelligence) performance compared with peers who received isocaloric beef or wheat biscuits. Soy nutrients may have an enhancing effect on neurocognitive skills in HIV-affected school-age children.","PeriodicalId":152216,"journal":{"name":"Family Medicine and Community Health Journal","volume":"41 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2017-12-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"115159909","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

{"title":"Family medicine and community health have borderless health topics","authors":"Wei Wang","doi":"10.15212/FMCH.2017.0143","DOIUrl":"https://doi.org/10.15212/FMCH.2017.0143","url":null,"abstract":"Received 25 October 2017; Accepted 9 November 2017 2016 and 2017 have been a time of overturned expectations, full of uncertainty, as evidenced by the United Kingdom’s vote to leave the European Union, the victory of Donald Trump in the US presidential election and the humanitarian catastrophe in Syria. From the perspective of achieving the goal of universal health coverage, border control in a health care setting is a new challenge to the principle of “access to health care is a human right”. In this issue, FMCH addresses health issues from “prevalence of and risk factors for eclampsia in pregnant women in India” [1] to the “effects of biscuit-type feeding supplementation on the neurocognitive outcomes of HIVaffected school-age children: a randomized, double-blind, controlled intervention trial in Kenya” [2] and “assessment of family physicians’ knowledge of childhood autism” [3] to the topic “counseling strategies for nutritional anemia by family physicians in Saudi Arabia, 2016: Implication for training” [4]. In addition, Koshy et al. [5] present their study on “Seguin Form Board as an intelligence tool for young children in an Indian urban slum”. In terms of translational and applied sciences in community setting, Liu et al. [6] review the current progress of “additive manufacturing techniques and their biomedical applications”. Additive manufacturing (AM), also known as three-dimensional (3D) printing, is gaining increasing attention in medical fields, especially in dental health and implant medicine, a novel topic for family medicine doctors and community health workers. As FMCH is a China focused health journal, two articles in this issue present Chinese insight into the “survey and analysis of patient safety culture in a county hospital” [7] and “an automated management system for the community health service in China” [8] aiming to provide evidence for strategy plan to improve patient safety culture, and to explore a concrete way for health administrative departments to manage the community health service (CHS) with a China focus. Lastly, Hussein [9] presents a case of probable sporadic Creutzfeldt-Jakob disease to a primary care clinic whereby neuropsychiatric symptoms were the patient’s initial presentation which later manifested with declining cognitive impairment, myoclonus and extrapyramidal symptoms. By covering these topics, this issue of FMCH invites your participation in a debate of the relevant questions in the routine clinical practices of Family Medicine and Community Health: FMCH addresses borderless topics from health, suboptimal health and illness, and from community health management to hospital treatment in terms of preventive, predictive and personalized medicine [10].","PeriodicalId":152216,"journal":{"name":"Family Medicine and Community Health Journal","volume":"27 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2017-12-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"128620143","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Because AM technologies have many advantages in comparison with traditional technologies, such as the ability to manufacture patient-specific complex components, high material utilization, support of tissue growth, and a unique customized service for individual patients, AM is considered to have a large potential market in medical fields. This brief review presents the recent progress of 3D-printed biomedical materials for bone applications, mainly for metallic materials, including multifunctional alloys with high strength and low Young’s modulus, shape memory alloys, and their 3D fabrication by AM technologies.
