Inaya, Mohammed Abdul-Ghani Al-Itabi","doi":"10.32792/utq/utjmed/16/2/8","DOIUrl":"https://doi.org/10.32792/utq/utjmed/16/2/8","url":null,"abstract":"Background: Sepsis during neonatal life is one of main leading attributer of the neonatal mortality, it consider as a major morbidity constituents, especially in the 3rd word and developing countries. \u0000Identification, diagnosis and treatment delays of this main problem consider as the main cause of mortality for this group of population. \u0000Aims of the study: to determine the main risk factors for neonatal sepsis in Al-Nasiriya public hospitals at 2017 \u0000Methods:A case control - hospital based study was done in Al-Nasiriya public hospitals –Southern of Iraq, extended over a period of 2017, neonates who had sepsis were the Cases, while with their index of pediatrician diagnosis while neonates who hadn’t sepsis consider as controls. clinical and proper laboratory investigations was done for to prove the inclusion and diagnostic criteria for both Cases and controls. Control and cases had been selected by using systematic random sampling. Spss version 23 had been used for statistical analysis . graph Bad had been used for epidemiological analysis. P value < 0.05 consider as significant. dependent and independent association had been tested by using binary logistic regression model, while identification of the associated risk factors to neonatal sepsis was tested by multivariable logistic regression. \u0000Results: A total of 150 neonate had been studied allover 2017, sixty cases and ninety control, with mean gestational age (37.5467± 2.36753), . near three quarters(72%) of NS were with EOS(<7 days), while the remainder were with LOS. The only Maternal age (21-35), CS as a mode of deliver, presence of( bleeding, PROM, maternal fever), and low birth weight of the of the neonate were strong risk factors for neonatal sepsis , APGAR <7, MSAFA<7 and Not crying immediately might be consider as neonatal sepsis predictors that might give hint for early initiation proper antibiotic \u0000Conclusion: Both neonatal and maternal factors had contributed to the risk of NS. Objectives: To measure selected health indicators of the catchment population of two primary health care centres and to support data base for the purpose of better care provision and to compare pattern between urban and rural communities. Methods: A cross-sectional survey involving 557 families (3785 individuals) from two primary health care centres catchment population, namely: Al-Zuwain Village in northern part of Basrah governorate and Hay Al-Muhandisin in Basrah city centre. Results: The main findings were the similarity of age pattern in both populations and the very sharp decline in population share as age advanced. The percent of married persons was high in Al-Zuwain compared to Al-Muhandisin population. Acute illness episodes were equally reported in both population with some variation in some individual disease entities. The use of preventive care was very acceptable and comparable in both populations. The local primary health centre was the main source of curative care in Al-Zuwain but not in Al-Muhandisin population. Conclusions: Household surveys represent a good source of additional information on various aspect of population, they are feasible and of multipurpose approaches to data acquisition. In this study, The overall pattern of services utilization is similar in rural and urban population with few exceptions. Methods: 200 patients with unexplained infertility randomly divided into two groups the first group received( 5mg letrezole from the day 3-7 of menstrual cycle), the second group received (100 mg clomiphene citrate from the day 2-5 of menstrual cycle), follicular development followed by serial U/S, when one or more follicles reach > 18mm in diameter ovulation trigger by hCG and timed intercourse was advise later on . Pregnancy test was performed 5 days after the miss period to confirm the pregnancy, the main outcome was the pregnancy rate and the secondary outcome was follicle development and endometrial thickness. {"title":"Patterns of Documenting Patients' Medical Records and Antibiotic Use for Cases with Acute Respiratory Infection in Basrah hospitals","authors":"A. Hussein","doi":"10.32792/utq/utjmed//15/1/1","DOIUrl":"https://doi.org/10.32792/utq/utjmed//15/1/1","url":null,"abstract":"The present study was carried out to evaluate the process of documenting patients' medical records in addition to the pattern of antibiotic use in Basrah general hospitals. A retrospective type of study was conducted for antibiotic prescription in 562 inpatients' medical records of those who were admitted to paediatric wards in five hospitals in Basrah governorate because of acute respiratory infections. The study shows that 86% of the patients stayed for less than 5 days at the hospital with 13% of them stayed between 5-10 days. All of the 562 patients' records were reviewed for documentation of the investigations taken during hospitalization, use of antibiotics and follow up while in hospital. Around two thirds (65.5%) of the records were found to have no documentation for any investigation during hospitalization. In addition, antibiotic therapy was found to be used in 83.3% of the cases. By comparing the WHO indications for using antibiotic therapy in acute respiratory infections cases, it was found that antibiotics were prescribed without justifiable indications in 44.7% of patients. The study recommended re-enforcing the adoption of updated drugs use protocols and guidelines especially on the use of antibiotics in hospitals and specifically in inpatient pediatric age groups. Recently, the obesity prevalence rate reached an alarming level in both developed and developing countries. Objectives: This study aims to determine the extent of overweight and obesity among infants and children aged 6-60 months in the center of AL-Nasiriya governorate and their relationship with different socio-demographic and lifestyle factors. Methods: A cross-sectional study was carried out on 225 infants and children aged 6-60 months selected from children who have consulting primary health centers that randomly selected in AL-Nasiriya city during the period from October 2015 till the end of February 2016. BMI and Z-score was measured and compared to standard tables. Data on socio-demographic factors and dietary pattern was obtained by questionnaire. Results: The proportion of overweight was 11.1% and for obesity 7.6% according to weight for height Z-score for all children. For children >24months, we found that 10.8% were overweight and 8.8% were obese by using BMI. In this study, there is a significant positive association between father's education and dairy products intake with overweight and obesity. Conclusion: The prevalence of overweight and obesity among infants and children aged 6-60 months in Al- Nasiriya was high. The treatment of choice for most parotid tumors is surgical excision. One of the main aims of superficial parotidectomy is to minimize injury to the facial nerve and maximize the rate at which it recovers. Two techniques are used for dissection of the facial nerve in parotid surgery: antegrade technique and retrograde technique . Objectives: The aim of this prospective study is to compare between the two techniques to know the best method of dissection to preserve the facial nerve integrity. Patients and Methods: This study presents the surgical excision of benign parotid tumors in 60 patients who complained from a mass in the parotid region. These cases were collected from maxillofacial surgery unit in AL-Shaheed Gazi AL-Hariri Hospital / Medical city/Baghdad from May 2009 to July 2011 and in AL-Hussain hospital in Thi-Qar from 2013-2016 .The age of those patients ranged from (19 – 60) years with a mean of 40.9 years. The patients divided into two groups, the 1 st group (30 patients) underwent antegrade superficial parotidectomy and the 2 nd group (30 patients) were underwent retrograde superficial parotidectomy. The assessment of facial nerve function is categorized into 6 grades according to House- Brackmann grading system. This assessment was started in 1 week, 1 month, 3 months and 6 months postoperatively. Results: The rate of nerve injury in antegrade technique was 30% at the end of week 1 and 20% at the end of month 1 while for retrograde technique was 40% at the end of week 1 and 10% at the end of month 1, It shows that the nerve injury in week 1 is more in retrograde than antegrade technique .The nerve recovery in antegrade technique was 40% at week 1 and 55% at the end of month 1 while for retrograde technique 70% recovered in the first week and 30% at end of month 1 . It shows that the recovery from injury is faster in retrograde than ante grade technique. Conclusions: There were more initial serious nerve injuries in the retrograde groups than in the antegrade groups but recovery was faster in the retrograde group. Twenty albino mice were taken (25-30) gm and divided in to two groups, group I (10 mice) which received normal saline intramuscularly and group II (10 mice) which received ) 25 mg /kg body weight ( daily of tramadol intramuscularly for 40 days, the biochemical assay showed significant increase (p value < 0.05) in the Aspartate aminotransferase (AST), alanine aminotransferase (ALT), Urea and Creatinine in experimental group compared to the control group. The histological examination regarded for liver tissues degenerative changes in the hepatocytes, dilated central vein, focal necrotic area, congested vessel, congested hepatic vein and cytolysis while kidney tissues show widening bowman capsule, interstitial nephritis, necrosis, degenerative change, lymphocyte infiltration in renal interstitial nephritis and degenerative changes in renal proximal tubules. Table (1) show effect of tramadol on Serum Alanine Aminotransferase (ALT) and Serum Aspartate Aminotransferase (AST) * refer to significant differences between compared mean, ** refer to highly significant increase between compared mean. There are weakness in all immunization coverage in most centers but Vitamin A , storage of vaccine , training of immunization team , presence of records , infection control equipments had good score in immunization units which had 81.1%. Maternal units had lowest evaluating percentage was 74.6 % because there are weak indicators in female medical staff , units according to standard , home visit , presence of sonar ,1st & 4th maternal visit coverage .Health promotion unit had 91.6% ,there are weak score for community participation , halls. While IMNCH units had 88.4% average percentage and most weak indicators were for number of medical staff , adequate place for ARI equipments program ,Thermometer. The continuous training & psychological health services had 82.6% , 95.8% respectively .Laboratory units evaluating percentage was 76.6% & most weak indicators were in structure staff , shaker , balance , spirit lamp , dispenser , calibration, KLB , G , AFB & water bath .In this study the main weak indicators regarding health visitor program in internal net work , transport vehicle , health visitor's maps & bags ,updating numbering of houses therefore this program had 84.4%. Mahdi, Mohamed Naji Kassim","doi":"10.32792/utq/utjmed/17/1/9","DOIUrl":"https://doi.org/10.32792/utq/utjmed/17/1/9","url":null,"abstract":"Background: \u0000Rheumatoid arthritis (RA) is the most common form of inflammatory arthritis, which is an autoimmune disease characterized by chronic inflammation of synovial joints, ultimately leading to joint destructions and permanent disability. \u0000In RA oxidative stress are impaired (which caused by free radicals) might have an essential role in the etiology of RA. \u0000Objectives: \u0000The objective of this study was to determine oxidative stress by measuring malondialdehyde and enzymatic status by estimating superoxide dismutase, catalase and glutathione peroxidase in patients of RA and then comparing with healthy individuals. \u0000Setting and design: \u0000A total 42 patients with Rheumatoid Arthritis (20 females, 22 males) in the age of 30-50 years were included, as a control, 50 matched healthy volunteers (20 females and 30 males) were involve, non of the subjects smokes nor receiving any form of drugs. {"title":"Incidence and Management Modalities of Splenic Injury in Blunt Abdominal Trauma in Al-Hussein Teaching Hospital at 2017..","authors":"","doi":"10.32792/utq/utjmed/16/2/6","DOIUrl":"https://doi.org/10.32792/utq/utjmed/16/2/6","url":null,"abstract":": Study was done Over the past one year, starting from January 2016 to the end of October 2017, an observational, analytical –cross-sectional study had been conducted in Al-Hussein Teaching Hospital in Al-Nasiriya city to study the estimate the incidence, severity, and mode of splenic injuries in blunt abdominal trauma, other objectives were to detect sensitivity investigations, and modalities of management in this injuries. A representative sample of 67 individual aged (6-77) years through multistage sampling had been included in the study . The diagnosis of the cases was clearly depending on the history, physical examination and investigations (ultra-sonography and CT scanning) The overall 67 patients with blunt splenic injury were identified (47 males, 20 females). The mean age of patients with splenic injury was 24.7 years .The most common cause of injury was RTA (88.2%). The most common grades of splenic injury were grade I, II. Majority of patients arrived to emergency unit after 2 hours .The most patients with RTA were managed as splenectomy (60.0%). The abdominal pain and abdominal tenderness were the most common clinical finding in splenic injury patients arrived to emergency unit. More than half of patients were stable at time of arrival to emergency unit (62.6% s ) .Splenectomy was the most common type of management (50.7%), while the conservative management was 19 most of age under 20 The study will help in establishing the priorities to act on according to the problems and their impact.
