To determine the factors that generate hypernatremia in relation to blood pressure measurement (MDPA) in high Andean settlers. The applied method was Barochiner’s based on home blood pressure measurement (MDPA), it also offers advantages over other ambulatory blood pressure (BP) measurement methods, such as greater acceptance by patients, lower cost and Treatment was monitored in some rural communities such as Huascahura, Rancha, Socos, Vinchos, Quinua, Acosvinchos, Acocro and Mollepata. Among the results obtained we have the following: the pathophysiological effect of hypernatremia in relation to the measurement of blood pressure (MDPA) in high Andean settlers of 36% The prevalence of 80%, in relation to the measurement of blood pressure (MDPA) in settlers high Andes. The factors that generate hypernatremia in relation to blood pressure measurement (MDPA) in high Andean settlers such as low consumption of beverages, dehydration, poor diet, pathologies and self-medication. The great impact it had on hypertensive patients has generated a change in the medical care paradigm, giving impetus to a virtual care modality, telemedicine. The objective of this work is to evaluate the technique of blood pressure measurement (equipment and procedure), its knowledge in this regard and, based on this, educate the hypertensive patient. Material and methods: descriptive and observational study of 426 patients, over 18 years of age, who requested a consultation by telemedicine in a community hospital, during July and November 2020. The patient was questioned about the equipment used, the measurement technique and on the knowledge of the blood pressure figures that defined him as hypertensive. Results: Of 426 patients, 49% were 62-year-old women. 92.95% had a blood pressure monitor at home, of which 73% were electronic arm, 16.16% were electronic wrist and 10.84% manual. Only 53% had a validated blood pressure monitor. 35% of the total made at least 2 records and 39.2% knew their blood pressure values considered normal. Methods: Aortic fragments of rabbits submitted to the following protocols were analyzed: all animals received a diet plus 0.5% cholesterol for 3 months and/or followed by: group A (n=8) - sacrificed at 3 months; group B (n=10) - followed by 3 months of standard diet; group C (n=20) followed by 3 months of 0.1% cholesterol diet; group D (n=12) - followed by 9 months of 0.1% cholesterol diet. Histological sections of aortic arch fragment were analyzed in hematoxylin-eosin and picro-sirius red staining. The atherosclerotic plaques were analyzed, classifying them according to the predominance of cellularity (type 2) or presence of extracellular matrix and fibromuscular cap (type 3) and associated with a large amount of extracellular cholesterol crystals (type 4). Results: We did not observe a statistically significant difference in the intima/media relationship, although there was a tendency to larger plaques in groups with lipid diet for prolonged time (p<0.06). We found after 3 months of hypercholesterolemic diet (group A), high serum cholesterol levels (1972±127 mg/dL) and plaques with higher cellularity (type 2), destructuring of the media layer, especially along the internal elastic lamina. The modification of the diet for 3 months (groups B and C) reduced serum cholesterol levels, modifying the characteristics of the plaque, verifying more developed plaques (types 3 and 4) in animals with lipid diet. After 12 months (group D) plaques with matrix predominance in relation to cellularity were observed, with a large amount of crystal of cholesterol and collagen fibers. {"title":"Comparison of heart rate and symptoms in two different methods of digoxin prescription","authors":"M. Mousavi, M. Methods: The current clinical trial was conducted on patients receiving a digoxin regimen of 0.125mg/day or 0.25 mg/day except for Mondays and Fridays (0.25mg/day-5/7) for atrial fibrillation (AF) or heart failure (HF). After measuring digoxin level and heart rate and scoring the clinical symptoms, the enrolled patients were switched to another method of digoxin prescription. The symptoms were reevaluated after four weeks of changing the regimen of digoxin. The study was registered in the Iranian Registry of Clinical Trials (IRCT) (No. 20110512006463N2). Results: A total of 77 patients completed the study. The mean age of the enrolled group was 65.16±11.64 years, and 44 patients (55.7%) were female. The mean digoxin level was 0.76±0.35 in the 0.125mg/day regimen, which changed to 0.94±0.56 in the 0.25mg/day- 5/7 regimen (P=0.034). Heart rate (HR) and daily activity scores, pain, spiritual feelings, energy, dyspnea, palpitation, nausea, blurred vision, and quality of life did not show any significant changes in the two regimens. The results of subgroup analysis in patients with AF were similar in terms of HR and symptoms, while in the subjects without AF, the scores of daily activity, dyspnea, palpitation, and quality of life significantly improved with the 0.25mg/day-5/7 regimen. Conclusions: Results of the current study showed that the HR and symptoms were not considerably different in the two regimens, whereas the digoxin level was significantly higher in the 0.25mg/day-5/7 regimen. Trilogy of Fallot is a rare congenital heart disease comprising of pulmonary valve stenosis, atrial septal defect, and right ventricular hypertrophy. Case: We describe a patient who has incidental findings of pansystolic murmur during physical examination and was ultimately diagnosed with severe pulmonary stenosis with large atrial septal defect on transthoracic echocardiography. Possible management plan was discussed with patient including combined percutaneous atrial septal defect occlusion and pulmonary balloon valvuloplasty in treating Trilogy of Fallot.
