We describe the anaesthetic management of a patient presenting with a compromised airway due to severe soft tissue swelling associated with Lemierre’s syndrome. In this case, preoperative computed tomography and nasal endoscopy delineated the anatomy. The otolaryngology team were prepared for an airway emergency. High flow humidified nasal oxygen maximised oxygenation during awake fibreoptic intubation and the time available for rescue tracheostomy if required.","PeriodicalId":14916,"journal":{"name":"Journal of Anesthesia & Critical Care: Open Access","volume":"62 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2017-12-11","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"83656115","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Among non-immune factors associated with the progression of GBV, an important role is played by hyperuricemia [1,4,5]. As early as 1948, E.M. Tareyev wrote in his monograph “Hypertonic Disease” about the frequent combination of arterial hypertension (AH) and metabolic disorders, including not only lipid and carbohydrate disturbances, but also purine metabolism [6]. It is well known that a decrease in the glomerular filtration rate (GFR) is accompanied by a slowing of the uricosauric function of the kidneys. Uric acid (MC) has the ability to initiate and maintain endothelial dysfunction, as well as stimulate the proliferation of smooth muscle cells, both in the systemic vascular bed and in the renal vessels [4,7]. Along with this, the widespread use of loop, thiazide diuretics and cyclosporine [1,8,9] also has a significant effect on the degree of excretion of uric acid in chronic kidney disease (CKD). In addition, at the stage of pronounced decrease in the nitrogen excretory function of the kidneys, AH occurs in almost every second patient [1,2]. Cardiovascular risk in GBV is increased several hundred fold, and finding out the pathogenetic mechanisms of heart damage is of great practical importance in the development of new therapeutic strategies for preventing and/r reducing adverse cardiac complications in the stage of dialysis therapy [1,2,10,11]. In this connection, the purpose of the present work was to study the role of hyperuricemia in the progression of cardiovascular complications in the course of the pre-dialysis stage of the disease. The average rate of CLABSI among critically ill patients in the United States is 0.9-3.4 infections per 1000 catheter days [1,2]. With 250,000 cases of CLABSI occurring annually and costing $6000 to $90,000 per event, there is significant interest in reducing this occurrence [2,3]. CLABSI originates from both intraluminal and extraluminal sources. Extraluminal colonization is thought to be due to organisms present at the catheter insertion site and likely reflects the quality of aseptic technique utilized upon catheter insertion in addition to insertion site care [4]. Daily maintenance of the sterility of the catheter insertion site is an intervention that may be underappreciated. Currently recommended catheter insertion site maintenance techniques for the prevention of CLABSI involve using either sterile gauze or sterile, transparent, semipermeable dressings to cover the site [5].","PeriodicalId":14916,"journal":{"name":"Journal of Anesthesia & Critical Care: Open Access","volume":"279 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2017-11-24","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"77582580","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The pain predominantly occurs unilaterally and involves the mandibular and /or maxillary branch or, rarely, the ophtalmic branch [1]. Most cases are classic or primary TN, which lacks objective evidence of motor or sensory deficit and is not attributed to another disorder, while a minority of cases are symptomatic or secondary TN, which is related to underlying causes such as multiple sclerosis, vascular compression and tumors [2,3]. However most cases are not associated with abnormalities on conventional MRI taken with inappropriate techniques. It has been mentioned in the literature that TN is usually caused by vascular compression of the trigeminal nerve root at the brainstem by a tortuous vessel. This compression results in demyelination of the nerve root entry zone, leading to ectopic impulse generation, ephaptic transmission of impulses, and the symptoms of TN [2].","PeriodicalId":14916,"journal":{"name":"Journal of Anesthesia & Critical Care: Open Access","volume":"14 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2017-11-14","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"83471852","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
