{"title":"Particle size distribution for jet nebulizers commonly employed in the pediatric clinical setting","authors":"J. Mallol","doi":"10.1089/JAM.1993.6.213","DOIUrl":"https://doi.org/10.1089/JAM.1993.6.213","url":null,"abstract":"ABSTRACT Two jet nebulizers employed in the pediatric practice were evaluated under usually prescribed operating conditions to determine the particle size distribution of their aerosols. The nebulizers, Hudson Up Draft II (UPD) and Bennett Twin (BT), were operated with different flow rates and volumes of fill, using normal saline (NS) and gentamicin (G) diluted in normal saline. Thus, six nebulization conditions were built. Particle size was measured by laser diffraction. There were wide differences in mass median diameter (MMD), geometric standard deviation (GSD), and percentage of aerosol mass contained in particles less than 5.3 microns (μ), 4 μ, 3 μ and 1.9 μ between the systems. The MMD for UPD nebulizer driven by a flow rate of 5.5L/min and loaded with NS (system A) or G (system E) were not different, 4.4 μ and 4.4 μ, respectively. Six rhesus monkeys (5-7.5 kg) were studied while anesthetized with 1 ml of ketamine (100 mg/ml) only or with ketamine pretreatment followed by sodium pentobarbital (30 mg/kg). Technetium-99m labeled sulfur colloid solution was deposited in the nose and clearance from the anterior nasal region and turbinate region was measured for 30 minutes using gamma camera scintigraphy. Clearance in monkeys anesthetized with ketamine only was approximately 2-fold faster than in monkeys anesthetized with ketamine plus sodium pentobarbital. These and other studies indicate ketamine anesthesia in monkeys is superior to other anesthetics because clearance is similar to that in conscious man. b) Deposition site effects. Two different techniques were compared in anesthetized mechanically ventilated dogs (mean body wt.± SD, 13.6 ± 0.9 kg). Monodisperse di-(2-ethyl-hexyl) sebacate particles (mean aerodynamic diameter±SD, 0.89±0.097 μm) were inhaled as bolus (half-width 8 ml) at 10-85% of the normalized (50% functional residual capacity, FRC) inspired volume (Bolus Technique). Alternatively, the total inspired volume contained the test aerosol, formally representing inspiration of a train of aerosol boluses of infinitesimal small volume (Single-Breath Technique). The particle concentration was continuously recorded at the airway opening by a miniaturized in-line aerosol photometer incorporating a 2 mW laser diode (λ = 820 nm) and a photodiode.
