Utell","doi":"10.1089/JAM.1997.10.1","DOIUrl":"https://doi.org/10.1089/JAM.1997.10.1","url":null,"abstract":"ABSTRACT A flow cytometric assay was developed that specifically measures Fc receptor-mediated phagocytosis independent of other phagocytosis-related receptors. Suspensions of rat alveolar macrophages were incubated with fluorescein isothiocyanate (FITC)-labeled polystyrene microspheres and subsequently were analyzed by flow cytometry. Coating of microspheres with bovine serum albumin was used to generate \"neutral\" particles demonstrating low phagocytosis by alveolar macrophages (AMs). AM attachment and uptake of these particles was increased tenfold with IgG opsonization. This increase was abolished completely by blocking of AM Fc receptors with IgG. The quenching effect of ethidium bromide (EB) on FITC fluorescence was used for differentiating attached from internalized particles. A single group of individuals inhaled randomly from two nebulization systems in a cross-over fashion. For System A, gentamicin 80 mg labeled with 4 mCi Tc 99m DTPA, diluted to 2 ml were placed in a jet nebulizer (JN) driven by an airflow of 5.5 1/min. For System B, the volume fill was 4 ml and the flow 10 1/min. The mass median diameter (MMD) of aerosol in System A and B was 4.4 m and 3.1 m, respectively. The nebulizer in System B released twice the volume (3.47 ± 0.04 ml) of gentamicin solution compared to System A (1.71 ± 0.03 ml) during similar nebulization time. Intrathoracic deposition was 4.0 ± 2.7% for System A and 7.1 ± 2.8% for System B. Lung deposition in patients inhaling from System A and B was 3.2 ± 1.8% and 6.2 ± 2.1%, respectively (p < 0.05). Tissue attenuation correction factors (ACFs) were determined in a group of 10 healthy volunteers, using four different techniques: measurement of body thickness, transmission scans, a sealed radioactive source, and a perfusion scan. Different combinations of these ACFs were then used to calculate deposition values from data obtained in the same volunteers in a previous study in which the deposition pattern of a drug inhaled from two different asthma inhalers was investigated. In addition, deposition patterns were determined from data that had not been corrected for tissue attenuation. Lung and stomach ACFs were relatively independent of the correction method (means, 1.84–2.16 for the lungs and 3.42–3.81 for the stomach), although ACFs for the oropharynx were more variable (means, 1.41–2.29). We have developed propellant-driven aerosols to deliver metered doses of DNA to the respiratory tract. Plasmid DNA was combined with surfactants to suspend it in a propellant. When aerosolized, respirable-sized DNA particles were produced. Aerosols of a β-galactosidase–containing plasmid induced transfection and gene expression in the respiratory epithelial cells of aerosolexposed mice. Part 1, published earlier,(1) focused upon aerosol bolus dispersion, which measures convective gas mixing. Here, Part 2 focuses upon aerosol-derived airway morphometry (ADAM), which assesses the diameters of airways and acini. This method is discussed in terms of its validity, ability to detect known interventions and diseases, and reproducibility (administering and performing this procedure were already discussed(1). With ADAM, airway and acinar diameters are expressed in terms of the effective airway diameter (EAD), which can be measured as a function of volumetric depth in the lungs. Evidence that ADAM is valid is provided by studies showing EAD agrees well with morphometric indices of airway and acinar size, especially once the distribution of aerosol is accounted for, and changes appropriately in response to known interventions and several diseases. AmBisome (4.6 mg of ampB/ml) was generated with an AeroTech II (AT) nebulizer, producing an aerosol during 20 of nebulization of 111.3 μg of ampB/L and with a mass median aerodynamic diameter of 1.8 μm. Antifungal activity of AmBisome was not affected by aerosolization. When mice were treated with aerosolized AmBisome(4.5 mg of ampB/ml) every other day for a total of 3 treatments (0.6 mg/kg/20 min), lungs rapidly accumulated ampB. Some hours after aerosolization, there was a gradual loss of drug from the lungs. Multiple treatments led to an accumulation of ampB with a maximum concentration after the third treatment of 43 mg of ampB/gm of lung tissue. AmpB was detected in lung tissue 14 days after treatment (24 mg/g). AmpB was not detected in the blood.
