Azevedo-Filho, Fernando Henrique Morais de Souza","doi":"10.9734/jammr/2024/v36i55412","DOIUrl":"https://doi.org/10.9734/jammr/2024/v36i55412","url":null,"abstract":"Aims: To report the case of a pediatric patient with total left brachial plexopathy (TLBP), exhibiting partially restored motor function and impaired sensitivity, who was treated with the End-to-Side neurorrhaphy (ETSN) technique.\u0000Presentation of Case: A 13-year-old female was involved in a car accident. After clinical and imaging evaluation, the diagnostic hypothesis was TLBP with partially restored function and loss of protective sensitivity in the pincer region. A surgical intervention was recommended. Due to case characteristics, ETSN appeared to be the most appropriate technique.\u0000Discussion: ETSN is a technique utilized for repairing nerve injuries. It involves suturing the distal end of a transected nerve to the lateral side of a healthy donor nerve, aiming to restore neurons through peripheral plasticity. The regeneration mechanism promoted by this technique shares some similarities with that promoted by standard techniques, with important roles being assigned to Schwann cells (SC), for example.  However, this procedure has its own particularities, which still require further investigation. Generally, ETSN cannot replicate the efficacy of model techniques, but remains valid in select cases, such as instances involving nerve injuries exceeding 3-4 cm or in the absence of donor nerves. Therefore, ETSN stands as a choice and deserves consideration.\u0000Conclusion: The patient progressed with a good prognosis and recovered protective sensitivity in the pincer region. ETSN is efficient for peripheral nerve injuries with inaccessible proximal trunks or long nerve defects. It is a relatively newly recognized entity within the spectrum of ILDs. Diagnosing PPFE can be challenging due to its rarity and overlapping clinical features with other ILDs. We report the case of two patients, siblings, of first degree consanguineous parents, 37 and 32 years old, suffering from worsening dyspnea, dry cough and weight loss, the high resolution chest computer tomography objectified a platythorax, a bilateral pleural cap thickening, diffuse sub pleural reticulations with bilateral apical traction bronchiectasis, associated with septal and non-septal lines, the main scannographic lesions spared the lower lobes. After eliminating differential diagnoses, the diagnosis of PPFE in a familial form was made based on the radiological criteria proposed by Reddy and al and on according to the 2013 ATS/ERS consensus. Studies on the clinico-pathological consistency in the diagnosis of skin disorders from our part of the world are very rare. \u0000Objective: This study aimed at determining the consistency between the provisional clinical diagnosis and the histopathologiscal diagnosis of skin biopsy specimens. \u0000Materials and Methods: This study was carried out over a six-month period using specimens from the 58 patients who consented to skin biopsy during this time, and comparison was made between the provisional clinical diagnosis and the histopathological diagnosis. \u0000Results: Of the 58 subjects that participated in the study, 29 were males and 29 were females. Their ages ranged between 7 and 68 years, with a mean age of 37.3 years. \u0000The spectrum of histopathologically diagnosed disorders included skin tumours and tumour-like conditions (12 cases, 20.7%), psoriasiform disorders (10 cases, 17.2%), lichenoid disorders (9 cases, 15.5%), infectious disorders (8 cases, 13.8%), disorders of epidermal maturation and keratinization (7 cases, 12.1%), spongiotic disorders (7 cases, 12.1%), vesiculo-bullous disorders (3 cases, 5.2%), and others (3.4%). Overall, the histopathological diagnosis was consistent in 19 cases (32.8%), corroborative in 12 cases (20.7%), with clinico-pathological correlation, and inconsistent in 27 cases (46.6%), leading to the issuance of a completely new diagnosis. \u0000Conclusion: The observed degree of clinico-pathological inconsistency in the evaluation and diagnosis of skin disorders in our environment implies that skin biopsy for histopathological examination is highly valuable if patients are to be issued the appropriate diagnoses for the avoidance of inappropriate treatments. This study aimed to investigate the correlation between IL-6 and its IL- 6 gene (174G/C) single nucleotide polymorphism with the pathogenesis of COVID-19 severity in Egyptian patients. \u0000Materials & Methods: Egyptian patients proved to be COVID-19 positive were classified into three groups; Group 1 (mild cases), Group 2 (moderate cases) and Group 3 (severe and critical cases). Complete blood count, liver function tests, renal function tests, CRP, and coagulation profile, serum ferritin, D-dimer, procalcitonin, lactate dehydrogenase and detection of serum interleukin 6 levels, and IL-6 gene polymorphism were investigated for all enrolled 135 patients. \u0000Results: There was a significant decrease of platelets count of group II & III as compared to group I. There was a significant increase of total leucocytic count, CRP, ferritin, D – dimer, procalcitonin, blood urea and IL6 of group III as compared to group I & II. There was significant decrease of relative lymphocyte counts % of group III as compared to group I & II. There was significant increase of LDH of group II & III as compared to group I. There were no statistically significant differences were found as regards hemoglobin, SGPT, SGOT, serum creatinine and I.N.R & interleukin-6 174G/C distribution of the different studied groups. \u0000There were positive correlations between serum interleukin 6 levels and CORAD scores, ferritin count, LDH, and D- dimer, and blood urea, and only a negative correlation with relative lymphocyte count %. \u0000Conclusion: interleukin-6 plays an important role in the pathogenesis of COVID-19. Materials and Methods:  this literature review a search was made for scientific articles in virtual databases: PUBMED, ScIELO, Google Scholar, LILACS, BVS, ResearchGate. Articles that were not part of dentistry were excluded based on the theme of the Abstract. The 28 articles pertinent to dentistry and the theme of the abstract were included and fully analyzed. The following were aligned and distributed in the literature review: systematic reviews, literature reviews, clinical cases, laboratory studies, books, theses and dissertations. Orthognathic surgery is a type of surgery which involves correction of jaw and facial irregularities to avoid problems (speaking, chewing, breathing, and aesthetics). One of the most critical steps in this surgery is the maxillary mobilization, which involves moving the upper jaw into a new position to improve facial symmetry and function. Traditionally, surgeons have used Rowe forceps or Tessier mobilizer for maxillary mobilization. The purpose of paper is to describe maxillary mobilization using BCPE instrument in orthognathic surgery in detail and demonstrate the use of BCPE in maxillary mobilization as a new and innovative technique in orthognathic surgery. It transcends all strata of the society including socio-cultural groups, families and the community as a whole. Amongst people diagnosed with the Human Immuno-deficiency Virus (HIV), IPV could have adverse health consequences and this could have serious implications for disease progression, transmission and control. This study was therefore aimed at assessing the knowledge, prevalence and forms of IPV against people with HIV/AIDS in Sokoto metropolis, Nigeria.\u0000Methods: A cross-sectional descriptive study design was used to recruit a total of 330 respondents which comprised adult male and female HIV/AIDS patients receiving comprehensive health care services in selected health facilities in Sokoto metropolis. Data on knowledge, prevalence and forms of IPV was elicited using a set of semi-structured interviewer- administered questionnaire downloaded into Open Data Kit (ODK).  Data collected was entered into and analyzed using SPSS statistical software version 25 with level of statistical significance set at p<0.05.\u0000Results: More than two-thirds, 259 (78.5%) of the respondents were diagnosed with HIV within the last one to ten years with almost all, 323(97.9%) having good knowledge of IPV. Only 28 (8.5%) of the respondents experienced at least one form of IPV with more females (9.5%) compared to 4.5% of males experiencing IPV. The various forms of IPV experienced by the respondents included sexual (4.2%), physical (6.7%) and psychological (8.5%).    \u0000Conclusion: Although almost all the respondents were knowledgeable about IPV, only a few of them experienced one form of IPV or the other. These materials do not require the prior application of the adhesive system and, at the same time, can promote a durable seal at the tooth-restoration interface. However, there is little clinical information about these materials. Therefore, this review aimed to gather information on the clinical behavior of these self-adhesive materials by conducting a bibliographic search from 2010 to February 2024 in the PubMed, Embase, Web of Science, Scopus, and Cochrane Central Register of Controlled Trials databases. Clinical follow-up studies of restorations performed with these materials were eligible for selection. The search terms used were “self-adhesive composite” or “self-adhesive flowable composite” where 14 complete articles were evaluated and 12 were selected. Only clinical follow-up studies of enamel and/or dentin restorations performed with self-adhesive resins were eligible for selection. This systematic review concluded that the self-adhesive flowable resin showed results comparable to those of the conventional flowable resin. {"title":"Demographic Profile and Clinical Outcomes of Patients Presented after Unsupervised Medical Termination of Pregnancy in a Tertiary Care Hospital in North India- A Cross-Sectional Study","authors":"Sheelson Sharma, Jagriti Bhatia, Saru Arora","doi":"10.9734/jammr/2024/v36i45404","DOIUrl":"https://doi.org/10.9734/jammr/2024/v36i45404","url":null,"abstract":"Aims: To study the demography, clinical Profile and complications of unsupervised abortion among women in a tertiary care hospital in Bathinda. \u0000Study Design:  Cross-sectional analytical Study \u0000Place and Duration of Study: Department of Obstetrics and Gynecology, Adesh Institute of Medical Sciences and Research, Bathinda, Punjab, between April 2023 to September 2023 \u0000Methodology: We included 100 patients of reproductive age (15-49 years) presented in Department of Obstetrics and Gynecology after unsupervised Medical Termination of Pregnancy. After the necessary intervention, the patients were interviewed using a predesigned proforma. Data were collected after obtaining written informed consent and study was approved by Institutional ethics committee. Data were analyzed by using SPSS 28. \u0000Results: A total of 100 patients were included. The mean age of patients was 27.7 years, with most falling within the 20-30 age range. A significant portion had two or more children (52%), and the majority were housewives (61%) from rural areas (62%). Educational levels varied, with 32% completing 12th grade and 6% having a graduate degree or higher. 89% reported within three days. Vaginal bleeding was universal (100%), while pain abdomen was reported in 53% of cases. Reason of MTP included accidental pregnancy (59%) and spacing between children (38%). Complications included Retained products of conception (RPOC) in 37% followed by unaffected pregnancy in 10% of patients. \u0000Conclusion: This study highlights the significant burden of unsupervised Medical Termination of Pregnancy (MTP) and its associated complications among women in Bathinda, Punjab. Pacífico, Ismael Felipe Gonçalves Galvão, Marcio Handerson Benevides de Freitas, Thiago José Monteiro Borges da Silva Valente, Kaio Felipe da Silva Vicente, João Marcos da Silva Dantas, Luciana Maria Queiroz de Oliveira Borges, Pompéia Freire da Silva","doi":"10.9734/jammr/2024/v36i45403","DOIUrl":"https://doi.org/10.9734/jammr/2024/v36i45403","url":null,"abstract":"Aims: This study aims to report a case of small cell neuroendocrine carcinoma with a primary site in the tonsil in a 76-year-old male patient and review the main aspects.\u0000Presentation of Case: In this study, the patient had a palpable cervical mass on the right (predominantly affecting level V) and a large primary tumor in the tonsillar site on the right, making it necessary to perform a biopsy for histopathological and immunohistochemical studies.\u0000Discussion: Primary small cell neuroendocrine carcinomas (SNEC) are poorly differentiated neoplasms with poor prognosis and low incidence in the head and neck regions. SNEC originating in the head and neck, which have the tonsils as their primary sites, are even rarer, with only 14 cases being reported in the English literature over the last 40 years.\u0000Conclusion: Due to the scarcity of data in the literature on this pathology due to its rarity, therapeutic strategies have not yet been formulated. The clinical and imaging data in this study were essential for staging and primary site definition.
