The lectin pathway stands out in this scenario, since understanding the activation of this pathway during COVID-19 infection can provide valuable information about correlated immunological mechanisms, especially in the uncontrolled inflammatory response of severe cases. Aim: Thus, the objective was to synthesize the available information on the role of the lectin pathway in the activation of the complement system and its relevance in the immune response and pathogenesis of COVID-19 infection. Methods: This is a systematic review carried out according to the PRISMA guidelines using the PICOS strategy. The quality of the studies and risk of bias were evaluated using the Checklist Hawker. After the analysis, 11 pertinent studies with the objectives of this study were included. Results: Patients with COVID-19 had high levels of mannose-binding lectin recognition protein (MBL), especially those with thromboembolic complications. MBL reduction due to certain genotypes (BB or AB) was associated with a more severe form of the disease. The interaction between VL recognition proteins and SARS-CoV-2 virus proteins suggests the activation of the complement system through this pathway. The interaction of SARS-CoV-2 protein N with the VL activator potentiates complement activation. There is a correlation between the genetic polymorphisms in the MBL2 gene and the expression of MBL in the tissues during infection. Conclusion: Although some studies have not found consistent correlations between complement markers and disease severity, it is consensus that complement system activation is present in patients with COVID-19, and high levels of activation products are associated with more severe forms of the disease, suggesting that inadequate regulation of the complement system may contribute to the uncontrolled inflammatory response and serious complications of COVID-19. Methods: This prospective work was performed on 60 pediatric patients presenting with failed penile hypospadias. Each participant was exposed to general assessment, local examination (Examination of the penis and examination of the testis and scrotum), oral examination, conventional lab tests [full blood picture, liver enzymes, kidney functions, and prothrombin time and activity], and ultrasonography. Results: Age, number of previous operations, type of previous urethroplasty, associated chordee, the length of uretheral defect were no statistically significant variation among the study groups. The aim of this study is to evaluate the myocardial functions and serum cardiac troponin (cTn-I) in children with convulsive status epilepticus without evidence of previous cerebrovascular diseases. Methods: This study was a case control study which was carried out in the duration of 1 year from December 2021 to December 2022 in Tanta University Hospital, Pediatric Department, Intensive care unit (ICU) and the emergency room. The diagnosis of status epilepticus had been made in accordance with the criteria of the International League against Epilepsy clinically as a generalized seizure (epileptic seizure with duration more than 5 minutes or recurrent again without regaining consciousness in between seizures). Another thirty (30) healthy children (16 males, 14 female) with age range from (3-15) years, with the mean age was (6.367±3.449) years, with no history of epilepsy or family history of neurological diseases, were enrolled in this study and were served as control group. Inclusion criteria were no sex predilection, Patients with age of (3-15) years, and children with convulsive status epilepticus. Convulsion type with duration more than 5 minutes or recurrent again without regaining consciousness in between seizures. Results: There was significant increase in serum level of cardiac troponin cTn-I among children with idiopathic convulsive status epilepticus than healthy Control children, (P <0.001*). All patients with elevated serum levels cTn-I had documented ECG changes. The test result had a high positive predictive value which indicates high specificity of the of mean serum level of cardiac troponin test and that, Children have high probability of having myocardial ischemia and injury following attacks of idiopathic convulsive status epilepticus. Conclusions: Convulsive status epilepticus triggers severe physical activities, while skeletal muscle contraction increases the cardiac afterload in the tonic stage, leading to a transient imbalance in the cardiac tissue demand, which is associated with myocardial cell damage. Despite guidelines, VTE prophylaxis rates remain low. In surgical settings, VTE prophylaxis is effective but often administered inappropriately. The National Institute of Health and Care Excellence (NICE) guidelines provide a comprehensive approach to VTE risk assessment and prophylaxis, emphasizing patient safety. Aims: This audit at Lister Hospital- East and North Herts NHS Trust in the United Kingdom aims to assess compliance with NICE guidelines for VTE prophylaxis in surgical patients, assess the impact of the newly introduced Electronic Prescribing and Medicines Administration (ePMA) system, to identify areas for improvement in patient care and policy development. Methods: This retrospective observational study analysed Lister Hospital's medical records to assess compliance with NICE guidelines for VTE prophylaxis in 88 surgical ward patients between Dec 2021 and Mar 2022. Data was collected from medical charts using convenience sampling, allowing for nonrandomized selection. Researchers assessed protocol adherence using six steps: 1st VTE assessment checklist completion, 1st Dalteparin prescription, 1st Dalteparin administration, 2nd VTE assessment, TEDS prescription, and TEDS administration. Results: This audit revealed variability in protocol adherence among patients, with concerns such as incomplete checklists n=49 (55.7%), low stocking prescription adherence n=36 (40.9%), and rare second VTE assessments n=7 (8%). Younger patients showed better protocol completion, and gender differences were noted, with females being more compliant with TEDS usage. Post-ePMA implementation, there were significant decreases in adherence for 1st VTE Checklist (p = 0.00001), second VTE assessments (p = 0.05), and TEDS prescription (p = 0.0001) compared to the pre-ePMA group. Conclusion: Adherence to NICE guidelines is currently suboptimal and interventions are required to optimise the adherence to guidelines and improve the effectiveness of ePMA. Improving understanding of the importance of adherence to the NICE guidelines on VTE prophylaxis, addressing protocol completion gaps, and enhancing education of ePMA system usage are key strategies for optimizing the ePMA model's effectiveness in the future. Medical practitioners in the surgical sections should undertake educational sessions via posters or presentations outlining the NICE guidelines. Appropriate and comprehensive training sessions tailored towards evidenced shortcomings in completing the protocol and ePMA usage is recommended, with further bi-annual mandatory training sessions to ensure and reinforce staff competencies. Transitioning through teenage years as an uncircumcised male in these communities can be challenging. This report highlights the psychosocial pressures of uncircumcised teenagers and the need for early formal circumcision to prevent self-injurious behaviour. Case Presentation: We present a 13-year-old male, who performed a self-circumcision using a kitchen knife after enduring repeated teasing episodes at annual sports camps. He was resuscitated and later had penile skin reconstruction done. He did not have an underlying psychological disorder. Discussion: Self-circumcision is rare and mostly reported amongst patients with underlying psychological disorders. Cases amongst teenagers with no underlying pathologies are ever rarer. Negative peer pressure in the form of teasing and push such patients to self-injurious behaviour. Early appropriate surgical intervention following self-circumcision results in good clinical outcomes. Conclusion: Uncircumcised teenagers in traditionally circumcising communities may suffer adverse psychological trauma from peers leading to self-harm. This research has as general objective to understand the impact of different training programs, with short intervention time (12 to 20 weeks) in the Metabolic Syndrome indicators of adults and elderly individuals. Through this integrative literature review with studies from the years 2010 to 2020, we sought to perform analyzes on the effect of resistance, aerobic and combined training protocols on the metabolic status of people with Metabolic Syndrome. The following metabolic, physiological and anthropometric variables were analyzed: Systolic Blood Pressure (SBP), Diastolic Blood Pressure (DBP), HDL-c, TG, Glucose and Waist Circumference (CC). The results showed that the Resistance Training in conjunction with caloric restriction showed greater effectiveness in reducing DBP and SBP. The increase in HDL-c was found in two of the four Resistance Training interventions. No aerobic training protocol showed significant changes in glucose levels. It was also shown that the Concurrent Training had a greater impact on the reductions in SBP, Glucose and CC. National Medical Center “La Raza”, Mexican Institute of Social Security, Mexico City, in the year 2022. \u0000Methodology: Case-control study carried out in 73 pregnant patients with SP. Case group: 34 patients with pregestational Body Mass Index (BMI) with obesity (BMI >30) who remained obese during pregnancy. Control group: 39 women matched by gestational age with normal pregestational BMI (BMI <25) who remained with BMI normal during pregnancy. Blood concentration of total LDH enzyme and arterial blood gas parameters upon admission to the Intensive Care Unit (ICU) were compared. The LDH correlations vs arterial blood gas parameters were calculated. Statistical analysis: descriptive statistics, paired Student's t test and Pearson correlation coefficient (r) were used with the SPSS™ version 24 program. The P value <0.05 was significant. \u0000Results: No differences were found in the blood concentration of total LDH enzyme (case group 669.54±71.79 vs control group 474.75±30.33 U/L, P= .21). The arterial blood gas parameters showed data of compensated metabolic acidosis in both groups, but without significant differences (pH P= .42, lactate P= .060, bicarbonate P= .89, std bicarbonate P= .74, ecf base deficit P= .93, base deficit P= .73). The correlations between blood concentration of total LDH enzyme and arterial blood gas parameters were not significant. \u0000Conclusion: Blood concentration of total LDH enzyme was similar. Both groups showed compensated metabolic acidosis without differences of gasometric parameters. The objective was to study absences for medical reasons in the workplace, particularly in a company that carries out the activities of conventional handling by the continuous loading and unloading of any type of goods, the activities of berthing of containers, storage. \u0000Material and Methods: This was a 2-year retrospective study of the files of workers absent for medical reasons. We had carried out a descriptive analysis of absences and an evaluation of the financial costs and the number of working days lost by the company. \u0000Results: Fifty-five percent (55%) of the workers had had at least one absence with an average absenteeism rate of 1.58%/year. The average number of days of absence per worker was 12.85 days and the average number of stoppages was 2.25. The main medical reasons for stoppages were ordinary pathologies 66.27% of stoppages and surgical pathologies i.e. 22.34%. Multiple stops were statistically significant with gender (p=0.01), age (p=0.03) and seniority (p<0.001). \u0000Conclusion: ordinary infectious or non-infectious diseases and surgical pathologies were the main medical reasons for absenteeism in this company. Case reports of Rothia spp. endocarditis in the scientific literature are scarce, with limited knowledge of relevant data on Rothia endocarditis and clinical aspects of its treatment. The objective of this literature review is to compile clinical aspects of endocarditis caused by Rothia species, analyzing results and clinical practices to elucidate the most important risk factors, comorbidities, prognostic factors, and appropriate antibiotic treatment options. \u0000Methods: Employing the PRISMA model, a systematic review was conducted utilizing PubMed, SciELO, and Google Scholar databases, encompassing articles published from 1978 to November 2023. Pertinent aspects were systematically recorded and summarized for subsequent analysis. \u0000Results: Rothia dentocariosa (48.69%), Rothia mucilaginosa (22.37%), Rothia aeria (14.47%), and Rothia kristinae (14.47%) were recognized as the agents of endocarditis cases. Patients exhibited an average age of 48.5 years, with a notable male preponderance (71.6%). Clinical manifestations of Rothia spp. endocarditis presented similar features compared to other Gram-positive bacterial endocarditis cases. The mortality rate was notably lower than observed in other infectious endocarditis instances (11.84%). Predominant risk factors included preexisting cardiovascular diseases (50%), followed by odontological procedures, caries, and precarious oral hygiene (17.1%), immunocompromised status (14.47%), injectable illicit drug use (11.84%), and diabetes (9.21%). Embolic events were documented in 35.53% of patients, predominantly in Central Nervous System (28.95%). Mycotic aneurysms were identified in 6.58% of cases. Resistance to antibiotics was identified in only 13.16% of strains causing endocarditis, although certain strains displayed characteristics indicative of multidrug resistance. \u0000Conclusion: Despite its rarity, Rothia spp. endocarditis exhibits clinical parallels with endocarditis caused by other Gram-positive bacteria, but with a comparatively lower mortality rate.
