There are at least 27 causativeagents against which vaccines are available and manymore agents are targeted for development of vaccines.2The number of antigens used in the immunisationprogrammes varies from country to country. However, there are a few selected antigens which are given againstdiphtheria, pertussis, tetanus, poliomyelitis, measles andhepatitis B which are part of immunisation programmesin most of the countries in the world. The first vaccine, against small pox, was discovered in 1798, and fromthe early 18th century, the British health administrationintroduced vaccination against small pox in the Indiansub-continent.3 The first doses of smallpox vaccinelymph in India arrived in May 1802.4 The most strikingsuccess of these efforts has been the eradication ofsmallpox disease from the world.5 EPI is implemented in192 countries of the world. But data shows that despitebeing a proven cost-effective preventive public healthintervention, the benefits of immunisation is not reachingmany children who are at risk of the diseases preventableby these vaccines. Majority of the children who do notreceive these vaccines live in the developing countries. Itis important to understand the underlying factors whichcauses non-immunisation of children and specifically, inBangladesh, in this review article.

The impact of vitamin D supplementation, physical activity, and diet counseling on the management of T2DM was measured in multivariable analysis by repeatedmeasure ANOVA.\u0000\u0000Results: The mean age of respondents was 46.4±9.6 years, majority were illiterate 21(34%) witha mean income 31,098.4 BDT; 63.9% have been suffering from T2DM for <5 years and 56% have a strong familyhistory of Diabetes. It was observed that reducing sedentary lifestyles and increasing physical activity increasedcalorie expenditure across timelines but not between groups (P>0.05). No significant differences were observed forphysical activity levels between groups (Intervention and placebo). Consuming less Carbohydrates and increasingphysical activity indicated a corresponding lowering of fasting blood glucose levels but not HbA1c. Compliancewith physical activity indicated higher total energy expenditure by the intervention group (2448 kcal) than placebo(2364 kcal). The biochemical profile indicated that vitamin D helps to reducing the fasting blood glucose (10.9 to 8.4mmol/L) levels along with diet restriction and increased physical activity.\u0000\u0000Conclusion: Advice on simple carbohydraterestriction and increased walking has an impact on lowering fasting blood glucose level but not HbA1C undergoingvitamin D supplementation.

Dental health problems due tothe poor oral health status of the tea workers are considered an emerging health problem.\u0000\u0000Methods: This study cross-sectional study was designed to assess the dentition status of the 372 tea garden workers. Dentition status was measuredby using ‘DMFT index score’.\u0000\u0000Results: The mean age of the workers was 35.2±8.3 years and one-third was from theage groups 28-37 years. Three-fourths of the workers (76.3%) had decayed teeth, 42.8% had missing teeth and fewer(5.9%) had filled teeth. The mean DMFT was found 3.15±3.481. The mean numbers of decayed missing and filledteeth per person were observed at 2.15±2.473, 0.97±1.629 and 0.13±0.621 respectively. The highest DMFT was foundin the 48-57 years group (6.64±4.382) and the mean decayed teeth were also highest in this group (3.94±3.464). Themean of decayed, missing and filled teeth observed in income group <5,000 taka were 2.24±2.541, 1.01±1.676 and0.14±0.645. Most of the workers’ dental health status was found poor (45.2%). The levels of dental health status wassignificant with the worker’s education, monthly family income, type of dental cleaning aids, frequency of daily toothcleaning, use of smokeless tobacco, use of pan-supari, alcohol consumption and visit a dentist (p<0.05).\u0000\u0000Conclusion: The study findings reveal that the dental health status was poor among the tea workers. Materials and methods: A cross-sectional study was conducted among 196 conveniently sampledprinting workers from 51 factories in September 2018. Data were collected by face-to-face interviews using a semi-structured questionnaire. A logistic regression model was applied to estimate the risk factors for dermatologicalproblems. Results: Among 196 respondents 74.0% experienced dermatological problems after joining a printingjob. Only 30.1% of respondents were found using personal protective equipment (PPE). No use of PPE, barriercream, hand washing more than 4 times a day, and duty in the printing process had a significant association with skinproblems (P<0.001). In the printing process, the workers who handled papers, chemicals, and machines and wereinvolved in cleaning machinery had a higher prevalence of dermatological problems. Logistic regression analysisshowed a significant association of skin problems among printing workers in Bangladesh with male (OR 0.23; 95%CI 0.06–0.98), higher monthly salaries (OR 17.98; 95% CI 3.14–103.08) and shorter daily work hours (OR 0.01;95% CI 0.01–0.56). Conclusion: This study revealed a high prevalence of dermatological problems among printingworkers in Bangladesh. PPE usage, avoiding excessive hand washing, and hand cream would be helpful in avoidingdermatological problems among printing workers in Bangladesh.

Pregnant women with autoimmune diseases, are likely toexperience more complications than are women without the disease. Pregnancies complicated by these disordershave a high clinical impact on both the pregnancy and the disease. Sjögren syndrome is an autoimmune disease witha high prevalence of anti-SS-A (anti-Ro) and anti-SS-B (anti-La) antibodies. Anti-SS-A antibodies are associatedwith congenital heart block. Women with Primary Sjögren syndrome require prenatal counseling explaining the risksinvolved and the need to control the disease well before conception. High-risk pregnancies can be optimally managedby a multidisciplinary team. Excessive fetal morbidity and mortality have been noted in patients with systemic lupuserythematosus (SLE). The influence of anti-SSA/Ro antibodies on fetal outcome in SLE patients has rarely beenreported, but its high association with congenital heart block or neonatal lupus syndrome is well known. As per the first nutrition survey afterindependence, “Nutrition Survey of Rural Bangladesh1975-76” carried out by the Institute of Nutrition and FoodScience (INFS), Dhaka University, revealed that 31% of12-23 months children and 28% of 24-35 months oldchildren were acutely malnourished; and 74% of children0-59 months old were stunted (moderate and severe). Nineout of ten families (i.e., 89%) were deficient in vitaminintake.\u0000\u0000The first nutrition survey in East Pakistan which is nowBangladesh, was conducted in 1962-64 but did not providemuch child-related data but found that over two-thirds ofthe daily diet consisted of rice, which was supplementedby curries, primarily of vegetables or fish, and Lentil.Milk and meat are consumed in small amounts, and fruitintake was only seasonal. Intake of cooking fats and oilswas scanty. The survey revealed that 46% of householdshad inadequate calorie intake. Carbohydrates constitutedabout 83% of the total calorie intake. Moreover, 85% ofthe people did not meet their recommended level of dailyprotein intake where almost 70% of protein was fromcereal proteins (plant sources) and only 14% was fromanimal sources. Eighty-four percent of all householdsconsumed less than the acceptable intake value of VitaminA (3,060 IU per person per day) and 40% of the populationhad deficient or low plasma concentration of Vitamin Aand carotene. Due to individual expectations two patients can provide different satisfactionscores after receiving the same care. It reflects how that care is supposed to be delivered as perceived by the patient. The individualized perception of a patient is influenced by patient’s individual values, personal beliefs, culturalbackground, age, education level, socioeconomic status, health literacy, and previous health care experiences. Patientsperceived low quality of physical care are extremely complicated to explain. Patients socio-demographic variables, especially age, sex, level of education, income, family background, ethnicity all can influence perceived satisfaction. The overall quality measure is either satisfying or dissatisfying. The health care administrators, health educatorsand other stakeholders and the community at large are in constant pressure to improve the patient perception ofquality health care. The increase in perceived care quality from the patients’ perspective are associated with improvedenvironmental conditions, care quality related to the health care personnel, information-communication andtechnological development. The high-tech environment may not improve patients’ perceptions towards quality careprovided by health care personnel if there is no empathy. Patient-centered service is recognized as a core componentof quality care. Patient-centered care is cost-effective and improves outcomes and patients are genuinely involved inhealthcare decisions and their preferences are considered, the result is better health, more involved patients and lowercosts.
