{"title":"Effects of single-dose and co-supplementation of vitamin D and omega-3 on metabolic profile in women with polycystic ovary syndrome: An RCT.","authors":"Hadis Bahramian, Saeed Sherafatmanesh, Nasrin Asadi, Ali Bakhshi, Mohammad Hassan Eftekhari, Maryam Ekramzadeh Comma","doi":"10.18502/ijrm.v21i7.13889","DOIUrl":"https://doi.org/10.18502/ijrm.v21i7.13889","url":null,"abstract":"<p><strong>Background: </strong>Polycystic ovary syndrome (PCOS) is a heterogeneous medical condition with a cluster of metabolic and endocrine disorders including dyslipidemia, insulin resistance, and hyperandrogenism.</p><p><strong>Objective: </strong>The present study aimed to determine the effects of single-dose and co-supplementation of vitamin D (vit D) and omega-3 (O3) on anthropometric and several biochemical factors in women with PCOS.</p><p><strong>Materials and methods: </strong>In this double-blind, randomized clinical trial, 80 PCOS women referred to Shahid Motahhari Clinic, Shiraz, Iran, from April to October 2017 were studied in 4 groups (n = 20/each) for 8 wk. The placebo group received the placebo capsule (paraffin oil); 1 weekly and 2/daily; the vit D group received vit D (50,000 IU/weekly) + 2 placebo capsules daily, O3 group, 2, O3 capsules daily + 1 placebo capsule weekly, and vit D + O3 (50000 IU/weekly vit D + 2, O3 capsules daily). Before and after 8 wk of intervention, height, weight, body mass index, waist circumference, triglycerides, total cholesterol (TC), high-density lipoprotein cholesterol, low-density lipoprotein cholesterol, fasting blood sugar, homeostasis model of insulin resistance index, and sex hormone binding globulin were compared between groups.</p><p><strong>Results: </strong>The significant reduction was detected in serum triglyceride (p = 0.002), TC (p = 0.04), fasting blood sugar (p = 0.02), insulin (p = 0.001), and homeostasis model of insulin resistance index (p = 0.001) concentrations in all vit D, O3, and vit D + O3 supplemented groups compared to the placebo group. Furthermore, in comparison with the placebo group, a significant increase was observed in serum sex hormone binding globulin levels after vit D, O3, and vit D + O3 treatments. {"title":"Relation between sexual function, perceived social support, and adherence to treatment with infertility factor in women: A cross-sectional study.","authors":"Marzie Sheikhian, Marzeyeh Loripoor, Zohreh Ghorashi, Faranak Safdari-Dehcheshmeh","doi":"10.18502/ijrm.v21i7.13893","DOIUrl":"https://doi.org/10.18502/ijrm.v21i7.13893","url":null,"abstract":"<p><strong>Background: </strong>In some societies, childbearing is a part of women's identity and infertile women are under a great amount of pressure from others.</p><p><strong>Objective: </strong>This study aims to evaluate the relationship between infertility factors and sexual functioning, perceived social support, and adherence to treatment in infertile women.</p><p><strong>Materials and methods: </strong>In this cross-sectional study, 230 infertile women who referred to the infertility center of Shahrekord, Iran in 2022 were enrolled. Data were collected using demographic characteristics checklist, female sexual function index, multidimensional scale of perceived social support, and general adherence scale.</p><p><strong>Results: </strong>No significant relation was observed between the infertility factor and the mean score of sexual function, the mean score of perceived social support, and the mean score of adherence to treatment (p <math><mo>></mo></math> 0.05). Among the aspects of sexual functioning, only the mean score of lubrication in the group of male factors was significantly higher than the common factors for men and women (p = 0.07). Vulvar edema is a disease with various causes and frequent phenomena seen in physiological and pathologic conditions like pregnancy, inflammatory disorders, tumors, idiopathic reasons, and most importantly, in the severe form of OHSS.</p><p><strong>Case presentation: </strong>Here, we report a 26-yr-old woman with severe OHSS, recombinant follicle-stimulating hormone therapy. 8 days later, we observed a mild and asymmetrical swelling of the vulva with severe edema in the right labia. {"title":"The comparison of pregnancy outcomes in fresh and frozen embryo transfer: A cross-sectional study.","authors":"Ramesh Baradaran Bagheri, Mahshid Bazrafkan, Abbas Sabour, Mina Ataei, Bita Badehnoosh, Banafsheh Mashak, Bahareh Khakifirooz, Ramin Moghaddam","doi":"10.18502/ijrm.v21i7.13891","DOIUrl":"https://doi.org/10.18502/ijrm.v21i7.13891","url":null,"abstract":"<p><strong>Background: </strong>The benefits of frozen embryo transfer (FET) vs. fresh embryo transfer for in vitro fertilization (IVF) have been discussed in previous studies.</p><p><strong>Objective: </strong>To determine and compare the pregnancy outcomes following FET and frozen embryo transfer in women who underwent assisted reproductive techniques.</p><p><strong>Materials and methods: </strong>In this cross-sectional study, 233 women candidates for IVF/intra cytoplasmic sperm injection who referred to the Kamali Training Medical Center, Karaj, Iran during 2019-2020 were evaluated in 2 groups of fresh (n = 127) and frozen (n = 106) embryo transfers. The rates of pregnancy outcomes including chemical and clinical pregnancy, live birth, preeclampsia, ectopic pregnancy, still birth, and pregnancy loss were compared between groups in 3 age subgroups (<math><mo><</mo></math> 25, 25-35, and 35-40 yr old).</p><p><strong>Results: </strong>No significant difference in terms of chemical and clinical pregnancy and live birth rates were observed between groups in women aged <math><mo><</mo></math> 25 yr. Chemical and clinical pregnancy and live birth rates were significantly higher in the FET group compared to fresh group in 25-35-yr-old women (p = 0.01, p = 0.03, and p = 0.01, respectively). In 35-40-yr-old women, no significant differences were observed in terms of chemical and clinical pregnancy rates, but live birth rate was found to be significantly higher in the FET group (p = 0.02). It seems that a group of cytosolic receptors named nod-like receptor family, pyrin domain containing 3 (NLRP3) inflammasome, is activated and involved in the pathogenesis of varicocele.</p><p><strong>Objective: </strong>We aim to determine the time course of NLRP3 inflammasome expression in the testis tissue following varicocele induction.</p><p><strong>Materials and methods: </strong>In this experimental study, 36 adult Wistar rats (8 wk, 200-250 gr) were used. For the varicocele induction, the left renal vein was partially ligated. Sexual health is, therefore, a major concern for these women.</p><p><strong>Objective: </strong>This study aimed to assess the effects of the sensate focus technique and position changing on the sexual function of women with deep-infiltrating endometriosis 3-6 months after surgery.</p><p><strong>Materials and methods: </strong>This clinical trial study was performed on 80 women with deep endometriosis, aged 18-45 yr, who were referred to the endometriosis clinic of Avicenna fertility center, Tehran, Iran for follow-up after surgery from January to September 2021. They were divided randomly into 2 groups. In the intervention group, 2-hr virtual training sessions were held and the control group only completed the questionnaires without any intervention. Sexual function was evaluated after 4 and 8 wk.</p><p><strong>Results: </strong>8 wk after the intervention, the mean score of sexual function in the intervention group was significantly improved (p <math><mo><</mo></math> 0.001). The mean total score of sexual function in the pre-intervention period reached from 24.16 to 28.31 in 4 wk after the intervention and 29.85, 8 wk after the intervention. The existence of STIs such as human papillomavirus (HPV) might cause cervical cell changes leading to cervical cancer.</p><p><strong>Objective: </strong>This study aims to assess the association of STIs with cervical cytological abnormalities and genital warts among women in northeastern Iran.</p><p><strong>Materials and methods: </strong>This cross-sectional study was carried out on 190 women referred to the central laboratory of Academic Center for Education, Culture, and Research, Mashhad, Iran from March to July 2022. The presence of genital infections caused by <i>Chlamydia trachomatis</i>, <i>Neisseria gonorrhoeae</i>, <i>Mycoplasma genitalium</i>, and Herpes simplex viruses (1 and 2) were assessed using the real-time polymerase chain reaction method. HPV genital infection was detected based on the principles of reverse hybridization, and cellular changes in the cervix were examined by the liquid-based cytology technique.</p><p><strong>Results: </strong>The mean age of participants was 35.33 <math><mo>±</mo></math> 8.9 yr. 34 different HPV genotypes were detected in all HPV-positive cases, and the most common genotype was low-risk HPV6. No significant association was found between STIs and cervical cytology abnormalities. The prevalence rates of sexually transmitted pathogens among HPV-positive and HPV-negative individuals were 10.9 and 1.6%, respectively. The frequency of genital warts was significantly higher in cases with multiple infections of high- and low-risk HPV genotypes.</p><p><strong>Conclusion: </strong>High percentages of the participants with non-HPV STIs and HPV infection had normal cervical cytology. {"title":"Pregnancy outcomes in women with adenomyosis, undergoing artificial endometrial preparation with and without gonadotropin-releasing hormone agonist pretreatment in frozen embryo transfer cycles: An RCT.","authors":"Marzieh Eslami Moayed, Ashraf Moini, Ladan Kashani, Maryam Farid Mojtahedi, Tawoos Rezaee, Hamed Tabasizadeh, Khadije Maajani, Nazila Yamini","doi":"10.18502/ijrm.v21i6.13635","DOIUrl":"https://doi.org/10.18502/ijrm.v21i6.13635","url":null,"abstract":"<p><strong>Background: </strong>Selecting a suitable and preferable method for endometrial preparation in frozen embryo transfer (FET) cycles for women with adenomyosis is still challenging in infertility treatment.</p><p><strong>Objective: </strong>To compare 2 artificial endometrial preparation regimens with and without gonadotropin-releasing hormone agonist (GnRHa) pretreatment in women with adenomyosis undergoing FET cycles.</p><p><strong>Materials and methods: </strong>This randomized clinical trial study was conducted on 140 adenomyosis cases who underwent FET cycles at Arash Women's hospital, Tehran, Iran from May 2020 to March 2021. Participants were randomly allocated into hormonal replacement therapy (HRT) and HRT+GnRHa pretreatment groups (n = 70/each). Endometrial preparation with 2-6 mg daily estradiol was started in the HRT+GnRHa group, taking after down-regulation with the GnRHa. Within the HRT group, the same dose of estradiol was commenced within the early follicular stage. The main (chemical and clinical pregnancy rates) and auxiliary results (twin pregnancy, miscarriage, and live birth rates) were compared between groups.</p><p><strong>Results: </strong>The demographic characteristics and severity of adenomyosis, endometrial thickness, and pattern at starting progesterone administration were similar in the 2 groups, and triple-line endometrium was found to be the dominant pattern in both groups (p = 0.65). No significant differences were observed in chemical, clinical, and twin pregnancy rates as well as miscarriage and live birth rates between groups (p = 0.71, p = 0.81, p = 0.11, and p = 0.84, respectively). However, the total estrogen dose and duration of estrogen consumption were significantly higher in the pretreatment group (p = 0.001, and p = 0.003).</p><p><strong>Conclusion: </strong>These results indicated that the hormonal endometrial preparation with estrogen and progestin for FET cycles is as efficacious as a protocol involving preceding pituitary suppression with a GnRHa. Besides pathological confirmation, diagnostic laparoscopy has been internationally accepted as the standard method to identify the accurate mapping of endometriosis. Transvaginal sonography (TVS) is the first non-invasive imaging modality to estimate the severity of endometriosis.</p><p><strong>Objective: </strong>This study aimed to evaluate the accuracy of TVS in affected women compared with surgical findings.</p><p><strong>Materials and methods: </strong>This retrospective cross-sectional study surveyed 170 women with deep infiltrating endometriosis (DIE) referred to the endometriosis part of the Avicenna Infertility Center, Tehran, Iran and they underwent TVS followed by laparoscopy. Recorded data of individuals under study in the medical database system were reviewed. Finally, the agreement rate was calculated for ultrasound reports and intraoperative (IO) findings regarding ovarian endometrium, ovarian adhesion, involvement of cul-de-sac, rectovaginal septum, and bowel and ureter.</p><p><strong>Results: </strong>170 women with DIE entered the study. The agreement of TVS and IO findings were 86.76% for left ovarian endometriosis and 70.86% for right ovarian endometriosis, 93.90% for left ovarian adhesion, and 88.90% for right ovarian adhesion, 88.90% for a cul-de-sac, and 84.82% for bowel nodules. The findings, based on a laparoscopic assessment of the pelvic floor, were completely compatible with ultrasound reports (100%).</p><p><strong>Conclusion: </strong>TVS allows a preoperative evaluation in planning the surgical policy associated.
