{"title":"Maternal outcome of Crigler-Najjar syndrome type-2: Case report","authors":"Amol Deore, Vijay Ahirrao","doi":"10.18231/J.IJPP.2021.015","DOIUrl":"https://doi.org/10.18231/J.IJPP.2021.015","url":null,"abstract":"Crigler-Najjar Syndrome type 2 [CN-2] is an uncommon inherited disorder characterized by non-hemolytic unconjugated hyperbilirubinemia. It is caused by mutations in one of the five exons of the UGT1A1 gene which codes for the enzyme hepatic uridine diphosphate glucoronosyl transferase-1, required for the conjugation and further excretion of bilirubin from the body via bile. Affected people are usually asymptomatic apart from jaundice and investigations reveal isolated indirect hyperbilirubinemia. It can be conveniently diagnosed by evaluating the response to phenobarbital in terms of reduction in bilirubin levels. Genetic testing confirms the finding of Crigler-Najjar syndrome. At least 20 different mutations of UGT1A1 have been associated with CN-2; all encode a bilirubin-uridine diphosphate glucuronosyl transferase-1 with markedly reduced but detectable enzymatic activity. Bilirubin concentrations are typically lower in CN-2, and plasma bilirubin levels can be reduced to 3 to 5 mg/dL by phenobarbital. Although uncommon in CN-2, bilirubin encephalopathy has occurred at all ages, typically associated with factors that temporarily\u0000elevation the plasma bilirubin concentration above baseline e.g. stress, prolonged fasting, an intercurrent illness like influenza. For this reason, phenobarbital therapy is often recommended. CN-2 is infrequent, and only a few pregnancies with this condition have been reported. The objective of the case study is to report a rare case of maternal CN-2 in the pregnancy and to evaluate whether pregnancy is safe in patients with the disease. A 29 years old mother with CN-2 had given birth to a baby girl by spontaneous vaginal delivery without complications. The newborn had mild unconjugated hyperbilirubinemia which was further treated by phototherapy and early morning sunlight. The aim of the present study was to evaluate therapeutic potential of ethanolic extract of Rosa damascene flowers (RDEE) in isoniazid induced hepatotoxicity in rats.\u0000Methods: Rats were given orally isoniazid (50 mg/kg) for 28 days. Silymarin (50 mg/kg) was administered, as standard drug. From 29th to 43rd day of the study, isoniazid was stopped and therapeutic agents such as normal saline, silymarin, RDEE 1.5g/kg and 3g/kg were given orally in different animal groups respectively. On 44th day, blood samples were collected for biochemical analysis and liver tissue was subjected to histopathological examination.\u0000Results: The rats administered RDEE showed significant reduction in the liver marker enzymes and total bilirubin treated as compared to negative control group. Though, efforts have taken to develop effective therapies for attenuation of DN, involvement of the multiple pathogenetic mechanisms and underlying oxidative stress (OS) makes it difficult to choose optimum therapeutic agent.\u0000Objective: The study is aimed at assessing the possible therapeutic effect of coenzyme Q10 (CoQ10) and N-acetylcysteine (NAC) individually or in combination on experimental DN in rats.\u0000Materials and Methods: The study was performed on adult male Sprague Dawley rats (n = 50; 200–250 g). Rats were made diabetic by alloxan (130 mg/kg; i.p.). The non-diabetic rats were randomly assigned to control, CoQ10 (10 mg/kg, p.o.), NAC (300 mg/kg, p.o.), and CoQ10+ NAC group. Whereas diabetic rats were grouped as DN control, DN+ CoQ10, DN+ NAC and DN+ CoQ10 +NAC groups. CoQ10 and/or NAC were administered to the diabetic rats for 8 weeks after confirmation of the DN. Levetiracetam is a novel antiseizure drug with better pharmacokinetic profile with less drug interactions and having broader therapeutic range.\u0000Aims: The aim of our study was to generate more evidence for better management of focal seizures with available anti-seizure monotherapy.\u0000Materials and Methods: It was a randomized, prospective, open label, comparative interventional study, conducted at Dr. R.P.G.M.C, Kangra at Tanda. 38 participants received tablet levetiracetam (LEV) 1000 mg/day and 36 participants received tablet controlled-release carbamazepine (CBZ-CR) 600 mg/day.\u0000Results: CBZ-CR group had significantly higher CNS depression as compared to LEV (p=0.027). Hematological adverse effects with CBZ-CR were reversible. Both the drugs were safe on kidney and liver. 22 (66.7%) patients in LEV group and 26 (78.8%) in CBZ-CR group remained seizure free. QOL: Both the drugs significantly improved QOL. Pharmacoeconomics: CBZ-CR costed significantly lower than LEV (p\u0000Conclusion: Both the drugs are well tolerated and equally efficacious in controlling focal seizures. Quality of- life has improved significantly in both the groups. Sodium-Glucose Co-Transporter Type 2 inhibitors (SGLT2i) are the latest class of oral anti-hyperglycemic agents that have been approved for the treatment of type II diabetes mellitus.\u0000Aim: In this review, we have comprehended the evidence for antidiabetic and extra-glycaemic effects of SGLT2i.\u0000Materials and Methods: The information is collected based on safety trials, randomized controlled trials (RCT), meta-analyses and real-world data.\u0000Results: SGLT2i are rapidly establishing their role in the treatment of diabetes mellitus. The beneficial effects extend beyond glycaemic control, impressive cardio protective and Reno protective effects and included improvement in blood pressure, uric acid concentrations, body weight and oxidative stress.\u0000Conclusion: Especially in patients with type 2 diabetes, SGLT2 inhibitors may be another option in those patients requiring additional glucose lowering and in those with acceptable risk factors.
