The main goal was to compare the NSA in intertrochanteric fracture of femur immediately after the surgical fixation using either Dynamic Hip Screw or Proximal Femoral Nail. In the elderly among all of the hip fractures approximately half are intertrochanteric fractures and out of which approximately 50% of fractures are unstable. Method: X-ray of pelvis with both hips was taken prior to fixation and after fixation, done either with PFN or DHS and morphometric measurements were taken. Result: The mean neck shaft angle restored after fixation with PFN (131.10±7.24 degree) was almost similar to that of normal side (129.58±5.79degree) whereas it was more with DHS (141.65±9.78 degree). There are a variety of associated injuries that occur along with the complex fractures in cases of trauma. Materials and Methods: The study was conducted after institutional research board clearance and after approval from the human ethics committee. All patients fulfilling the inclusion and exclusion criteria admitted at A.J.I.M.S. will be considered for the study after obtaining informed written consent. Once the patient was fit for surgery patient were operated by senior staff. Preoperatively the urine was sent for detection of fat globulin and Postoperatively the urine was sent for detection of fat globulin evaluation within 3 hours of surgery. The collected data was analysed by SPSS 17.0 version software. Observations and Results: Males were 88% p =0.008, left side 52% p =0.078. 33% were admitted within 4-5 Among all bones tibia was the commonest of the bone fracture involved 67 cases, combination of tibia and fibula was the commonest seen in 44.00%.The examination of urine for fat globulin post operatively was negative in all 100 of the cases. The examination of urine for fat globulin pre operatively was positive in 2% of the cases. Conclusion: 1 The present study we found that urine fat globulins in fractures involving the long bone are present in two percent of cases. In both cases the patients had sustained multiple bone fractures. There was a delay in presentation to the hospital of more than 4 hours in both cases. Even after an adequate and rigid fracture fixation early rehabilitation is necessary to drastically improve the outcome and reduce stiffness. Lack of effective pain control postoperatively can significantly reduce participation in therapy regimens and thus contribute to reaccumulation of fibrotic tissue in the joint after artholysis. Hence, epidural anaesthesia through a lumbar epidural catheter is an established procedure to reduce post operative pain. 1 gm preservative free morphine and 2ml of 0.5 % Bupivacaine diluted with normal saline to make a total of 10 ml is the most commonly used protocol in top up after surgery for post operative analgesia. MATERIALS AND METHODS:This is a prospective study. A total of 40 patients with peri-articular of distal femur and proximal tibia were included in the study. Patients were randomised into 2 groups. Group A was given spinal anaesthesia for surgery and Group B was given combined spinal plus epidural anaesthesia with a lumbar epidural catheter. The grafts harvested are corticocancellous chips which we prefer to name as 'Drill dust'. This drill dust usually goes wasted as surgeons tend to ignore to collect the drilled out bone which is usually washed out during or after the procedure. We decided to collect these drilled out bone in a meticulous and systematic manner and found that they form a significant amount of bone to be used as a graft at the fracture site which can potentially enhance fracture union. We did a study utilising our technique assessing clinical and radiological union in a subset of patients undergoing upper limb fracture plate osteosynthesis between July 2018 to Jun 2019. We found that there was an enhanced radiological and clinical union in all 28 of our cases where this technique was used. Material and Methods: 15 patients were analyzed having unilateral babhulkar and pande stage 2, 3 and 4 hip tuberculosis and with regular antituberculosis treatment for more than 4 weeks before surgery treated by total hip replacement. Result: There was a consistent rise in the mean value of the Harris Hip Score which was 33.47 at the time of preoperative assessment and at the 6 months it got raised to 87.93, the comparison between the mean values of the intra group also came up with a significant P value of 0.01 for each and every segment and the mean values of Hip Harris Score was also noticed to be significant as well along with the competition of the same across the intra group. Conclusion: Finally, it can be concluded from the study that total hip replacement for arthritis following the tuberculosis of the hip acts as a proper method for the patients who suffer from tuberculosis and restores them back to a good function that helps them to get back to their daily life. But a proper intraoperative workup is also very much necessary for ruling out if any residual disease is left, with proper follow up of the patience to identify if there is any recurrence. Sometimes a chance for recurrence remains during the first-year post completion of the surgery despite complete resolution of the radiological sign. Methods: The present study consists of 20 cases of Monteggia fracture dislocation in adults treated by 3.5mm Dynamic compression plate/Locking Compression plates/Buttress plates and screws in the Department of Orthopaedics, SSG Hospital, Govt. Medical College, Baroda over the period of 1 year (2019-2020). There were fifteen male patients and five female patients who underwent surgery. Results: In our study patients are of age group between 19-59 years. Majority of our patients belonged to 21-40 age group. The ratio between Male to Female is 3:1. In our study labourers (manual & farm) were 13 (65%). Most of patients had sustained fracture with fall. 16 (80%) patients had Monteggia fracture with anterior radial head dislocation (Type 1). 1(5%) patient had Monteggia fracture with posterior radial head dislocation (Type 2). 1(5%) patient had Monteggia fracture with lateral radial head dislocation (Type 3). 2(10%) patients had Monteggia fracture with upper third radius fracture (Type 4). Both infections were superficial and healed eventually. 1(5%) patient went under non-union. Majority of patients are having excellent results 10 (50%). 9 patients had good result (45%). 1 patient had poor result (5%). Conclusion: The treatment plan in Monteggia Fracture dislocation depends on fracture pattern, soft tissue injury, patient co-morbidity, fixation resources. Fracture pattern, condition of soft tissue are important factors to be considered before selection of fixation method.
