It is a rich source of iron, magnesium, alkaloids, saponins, flavonoids and steroids. Recent researches have highly explored its antidiabetic, anticarcinogenic, hypocholesteremic, anti-oxidant, and immunological actions. Seeing the volume and trend of research on the said herb, least work has been done with reference to this valuable plant in Unani medicine. It is a sincere effort of authors to accurately present the invaluable benefits mentioned in Unani classical literature.

Although various bone augmentation procedure like ridge augmentation, sinus lift is in practice but it may lead to the morbidity of donor’s site. Sometimes patient is not willing for such extensive surgical procedures. In such cases Cortical/basal implants is a viable treatment option. Basal implants gives support from the basal bone area which usually remains free from the infection and less prone to resorption. This study was undertaken to assess clinically and radiographically the bone levels in rehabilitation of immediate loading and delayed loading in opposite arch in an occlusion for a period of 1 year. After placing the implants, immediate loading was done within 72 hours for basal implants and conventional implants were placed in the lower partially edentulous sites.

The periodontal status of even young patients has been altered so it is of utmost importance that orthodontic patients has to be ruled out suffering from plaque induced gingivitis and compromised periodontal status. It is the role of the orthodontist to screen the disease, make provisional diagnosis and refer to a periodontist for immediate treatment as without periodontal therapy the severity and extent of the disease may progress. So, the orthodontist has a great role in bringing up awareness not only before starting the appliance therapy, but also during and after the active mechanotherapy. The understanding of metabolic and hormonal abnormalities in milder forms of renal dysfunction is expanding, but the knowledge of thyroid dysfunction in people with CKD is still limited. The growth, differentiation, and regulation of physiological mechanisms in all tissues, including the kidney, depend on thyroid hormones. They are essential for maintaining the balance of electrolytes and water. Prevalence of hypothyroidism in end stage renal disease (ESRD) has been estimated to be in the range of 0 to 9%. Patients with ESRD also have a higher prevalence of goitre. Although there are several indicators that can predict both overall mortality and the severity of renal impairment, thyroid dysfunction is one of the major ones. In order to improve the outcome, it is wise for the internist and treating physician to be aware of thyroid dysfunction. The importance of knowing the prevalence of thyroid dysfunction in CKD patients also lies in the fact that it adds to the already high cardiovascular mortality risk in this patient group. \u0000Objectives \u0000 \u0000To find the proportion of thyroid dysfunction in patients with chronic kidney disease. \u0000To study the correlation between thyroid dysfunction and severity of renal diseases. \u0000 \u0000Methodology \u0000A single center cross sectional study conducted in Departments of General Medicine and Nephrology in a tertiary care hospital in Calicut, Kerala. In the study period of 12 months, among patients admitted in Medical Ward after applying inclusion and exclusion criteria, 100 patients were included in the study. Patients who fulfilled the criteria for CKD and who are on conservative management and haemodialysis were taken up for the study. Thyroid profile was done in all patients who fulfilled the criteria. The prevalence of thyroid dysfunction in chronic kidney disease was described and analyzed in terms of percentages and averages. One way ANOVA test was used to analyze various parameters like T3, T4, Blood urea and Serum creatinine in relation to various grades of renal failure. \u0000Results \u0000Low T3 levels were seen in 65% and subclinical hypothyroidism was seen in 17% of the study subjects. Low T4 was seen in only 6% of the study subjects. There was no significant association observed between CKD stages and T3, T4, TSH values categorized as low, high and normal. \u0000Conclusion \u0000Low T3 syndrome was the commonest abnormality detected. This may be viewed as protective mechanism to conserve protein in chronic kidney disease patients. Subclinical hypothyroidism was the second most common abnormality detected and the number of patients with subclinical hypothyroidism progressively increased with the severity of chronic kidney disease. As subclinical hypothyroidism is associated with increased cardiovascular mortality in CKD patients, adult patients with CKD should be routinely screened for subclinical hypothyroidism and further studies ar

Several pleiotropic effects of Vitamin D have been studied like regulation of cell proliferation, suppression of cell mediated immunity, Glial cell line-derived neurotrophic factor, stimulation of nerve growth factor, suppression of Renin-angiotensin-aldosterone system (RAAS), reduction of albuminuria, immunomodulatory effects, and anti-inflammatory effects.(1)Thus, vitamin D is implicated in the pathogenesis of retinopathy, neuropathy and nephropathy. \u0000Materials and Methods: A Cross-sectional case-control study of 206 patients (>30 years), who met the American Diabetes Association, 2011 criteria (2) for type 2 DM, was conducted. Subjects were evaluated for the presence of microvascular complications by clinical evaluation, urine examination, fundus examination, clinical neuropathy scoring, and various biochemical tests. 25-OH cholecalciferol levels were done and cut off level for vitamin D deficiency was 30 ng/ml. 72 Vitamin D sufficient and 134 Vitamin D deficient cases were studied. \u0000Results: A total of 134 T2DM cases with Vitamin D deficiency were studied. 72 age matched T2DM patients with sufficient Vitamin D levels served as controls. The mean age of cases under study was 47.58 ± 9.63 years compared to 51.24 ± 8.75 years of controls. 61.2% were females, whereas 38.8% were males in the case group while in the control group, 69.4% were males, and 30.5% were females. The mean duration of diabetes in the cases studied was 57.08 ± 18.13 months. It was 52.69 ± 17.98 months in the Vitamin D sufficient controls. Significant statistical associations were established using Chi square test between Vitamin D status and development of microvascular complications like Diabetic nephropathy, retinopathy and neuropathy in Type 2 Diabetes Mellitus at p < 0.05. \u0000Conclusion and Implication: Vitamin D is an important factor in modifying the risk of Type 2 DM and its microvascular complications, especially Diabetic retinopathy, neuropathy and nephropathy. Administration of vitamin D supplements may prove to be a beneficial adjuvant therapy in mitigating microvascular complications. Frequent Monitoring of Vitamin D status can decrease the burden of co-morbidities in T2DM and thus can decrease the mortality rate associated with T2DM. However; the exact role of vitamin D in these processes requires further investigations. \u0000Keywords: \u0000Diabetes, Nephropathy, Neuropathy, Retinopathy, Vitamin D
