Pontoriero","doi":"10.5301/GTND.2017.17765","DOIUrl":"https://doi.org/10.5301/GTND.2017.17765","url":null,"abstract":"Uremia is a systemic inflammatory condition characterized by a combination of oxidative burst, uremic toxicity, uremic dyslipidemia, endothelial dysfunction and oxidative stress resulting from dysfunctional mitochondrial electron transfer generating reactive oxygen species. These compounds induce oxidative modifications of carbohydrates (AGEs), proteins, lipids and DNA, in turn recognized as DAMPs by Toll-like receptors (TLRs), which in end-stage renal disease (ESRD) are upregulated in many cell types including macrophages and neutrophils. All the inflammatory derangements typical of ESRD lead to an increased cardiovascular risk in dialysis patients. Unfortunately, the technological innovations in dialysis techniques have not given rise to a better prognosis for patients with ESRD, probably because they have had no favorable effect on inflammation in the uremic milieu. Sarti","doi":"10.5301/GTND.2017.17758","DOIUrl":"https://doi.org/10.5301/GTND.2017.17758","url":null,"abstract":"A nursing research project including a series of qualitative surveys was set up to investigate how a care-team culture of hospitality and the use of some relational tools could, through a structured approach, have positive effects on patients starting the dialysis treatment pathway. A structured hospitality approach addressed to new-entry patients seems to activate important prerequisites for patient care perception. The research showed that the lack of a personally addressed hospitality approach would not be limited to neutral effects but might have harmful effects on patients' self-care attitude as well, producing different patient outcomes, both clinically and in their interaction with the dialysis setting. The will to stimulate self-empowerment and self-esteem seems to be clearly perceived and appreciated by the patients, and this was confirmed by the collected quality data. The measures of association evaluate the direction and magnitude of this relationship. Relative risk (RR) and odds ratio (OR) are the measures used most when the strength of the association between an exposure and an outcome is measured in relative terms. RR is calculated as the ratio of the incidence rate of a certain outcome among individuals exposed to a particular factor and the incidence rate among those not exposed to the same risk factor. OR is calculated as the ratio of the odds that an exposed person develops disease and the odds that an unexposed person develops disease. RR and OR higher than 1 mean that exposure is a risk factor for the outcome. RR and OR lower than 1 mean that exposure is a protective factor for the outcome. The mechanisms regulating the dissemination of facts and information online are complex and partially unknown. For some of them, such as confirmation bias and polarization, specific mathematical models have been provided. Although a lot of work still needs to be done, it is already possible to develop strategic behaviors aimed at countering disinformation. The concept of NBM has been developed over the last decades to bring the biomedical aspects of illness and treatment closer to the psychosocial aspects and life experiences related to illness. Purpose To identify and summarize the evidence and concepts of NBM in nephrology. Methods Review of studies based on a literature search in 6 databases. Results Thirty-five papers were included in the review, none of which were quantitative. Discussion The narrative approach applied to a nephrology context seeks to shed light on the life experiences related to kidney disease by collecting, through structured and semistructured interviews, stories from patients, caregivers and health care providers. Some of the main topics emerging in the first article are the lack of a “disease identity” that allows patients to identify with the specific status of kidney disease; uncertainty as a dominant feature of living with an illness characterized by continuous progression and regression; the “unspeakable”, referring to all those aspects for which there is no room for expression in the care relationship, such as living with the risk of dying, experiences that are difficult to put into words, and everything related to mystery, faith and the inexplicable. Conclusions This review shows the importance of exploring the disturbing aspects of life for patients with chronic renal failure. Knowledge of these aspects can help health care providers improve the quality and effectiveness of the therapeutic relationship and ensure that the patient will not feel isolated but at the center of the care process. Narrative medicine, as will be highlighted in the second article, is also aimed at health care providers in order to investigate their experiences, which are often characterized by situations of suffering and discomfort.
