The existing guidelines do not contain a clear algorithm for predicting the late recurrences of atrial fibrillation (AFib). Objectives. We have studied the predictors of late recurrence of arrhythmia in patients with persistent AFib after the restoration of sinus rhythm (SR). Research methods. A prospective single-center study included 120 hospitalized patients with persistent AFib who underwent successful cardioversion. The recurrence of AFib was assessed in the early period after cardioversion by recording 12-channel ECG, Holter ECG monitoring and event ECG monitoring. After a nine-month period follow-up visit was performed. Baseline demographic, clinical-functional features and comorbidities were compared in group with (n = 87) and without (n = 33) late recurrences of Afib. Results. There were no differences in demographic, anthropometric data and comorbidities. Patients with late recurrence of AFib had a lower heart rate (HR) after SR recovery (p <0.001). Early recurrences of AFib were found in 43 (49.4%) patients with late recurrences of arrhythmia and only in two (6.0%) without late recurrences (p <0.001). Patients with late recurrences of arrhythmia had a lower average daily HR (p <0.001), a higher number of single atrial extrasystoles (p = 0.001), atrial pairs and runs (p = 0.01) recorded by Holter ECG monitoring. Conclusions. Late recurrences of arrhythmia during the nine-month follow-up period were reported in 72.5% of patients. Groups of patients with and without late recurrence of arrhythmia had significant differences in the incidence of early recurrences, mean HR on SR, and the presence of atrial extrasystoles after rhythm recovery.","PeriodicalId":140699,"journal":{"name":"Proceedings of the Shevchenko Scientific Society. This year, the symposium is focused on the “Medical Imagining and Global Health”. The scientific event was held in Lviv on October 7–9, 2021. The format was mixed. The event was held with the support of Danylo Halytsky Lviv National Medical University, Medical Commission of the Shevchenko Scientific Society, Lviv City Council and Lviv Convention Bureau. The symposium was focused on providing a unique opportunity for young and experienced scientists and doctors working on the development of innovative technologies in medicine, to further cooperate in the field of science and integrate their knowledge and achievements into world science. The interesting and eventful agenda included over 20 lectures and poster presentations delivered by national and foreign lecturers, as well as a master class on “How to use the online Open Journal System to publish scientific articles in medical journals”. To focus the attention of young scientists, students and interns on topical issues of medicine, well-known scientists from different countries of Europe and America are annually invited to attend the symposium as speakers. During the two days of the conference, Ukrainian and foreign leading experts in the field of medicine – Leo Wolansky (USA), Sandor Szabo (USA), Vassyl Lonchyna (USA), Klaus Holzmann (Austria), Siegfried Knasmüller (Austria), Armen Gasparyan (Great Britain), Ivan Wolansky (USA), Yuriy Ivaniv (Ukraine), Nelya Oryshchyn (Ukraine), Andriy Netliukh (Ukraine), Yuriy Mylyan (Ukraine), Oksana Zayachkivska (Ukraine), Roman Plyatsko (Ukraine), Khrystyna Lishchuk-Yakymovych (Ukraine), Olena Zimba (Ukraine) – shared their experience and the latest achievements in the field of medicine. After a two-year break due to a COVID-19 pandemic, joint live discussions between young scientists – students, interns, post-graduate students – with leading scientists during poster presentations and panel discussions held at the symposium helped them rethink the need for systemic changes in medical education and the implementation of modern diagnostic methods utilizing real-time visualization with elements of artificial intelligence into curriculums. The paper describes the peculiarities of different immune cytopenia in CLL patients and the importance of individual prognostic markers in the course of the disease. Methods: We observed 62 patients with CLL complicated by immune cytopenia. Among these patients 30 had autoimmune hemolytic anemia (AIHA), 18 experienced immune thrombocytopenia (ITP), 10 had Fisher-Evans syndrome (FES), 3 were diagnosed with partial red cell aplasia (PRCA), and immune neutropenia (IN) was revealed in 1 patient. In addition to general examination and laboratory studies, the following examinations were performed: immunophenotyping of peripheral blood lymphocytes, flow cytometry (CD5; CD19; CD20; CD23; CD38; ZAP70), Coombs test, a molecular cytogenetic study of peripheral blood lymphocytes using the FISH method with TP53 and ATM probes, the level of ß2-microglobulin. Results: It was established that the overall survival of CLL patients with IC depends on the form of the latter. The median overall survival in patients with Fisher-Evans syndrome was the shortest (75 months), slightly better survival was observed in patients with AIHA (median 80 months), the best survival was found in patients with ITP (median not reached). Among unfavorable markers of CLL with IC, there is the presence of del 11q22.3. It has been assumed that generalized periodontitis (GP) adversely affects the qualitative and quantitative composition of plasma lipids and lipoproteins. On the other hand, periodontal treatment and reduction of general periodontal pocket infection in patients with GP are associated with a decrease in total cholesterol (TC), low-density lipoprotein-cholesterol (LDL-C) and triglycerides (TG). The aim of the study was to conduct a comparative assessment of the state of lipid metabolism in patients with coronary artery disease (CAD) depending on the GP severity. Methods. The study included 101 patients (mean±SD age - 58.2 ± 8.3 years) with CAD and GP, 58 men (57.4%) and 43 women (42.6%). According to the severity of GP, study participants were divided into 3 groups: group I - patients with CAD and GP stage I (initial periodontitis), group II - patients with CAD and GP stage II (moderate periodontitis) and group III - patients with CAD and GP stage III and IV (severe periodontitis). The control group included 9 patients with CAD and clinical gingival health on an intact periodontium (mean±SD age - 56.3 ± 6.1 years), 5 men (55.6%) and 4 women (44.4%). The diagnosis of CAD and the results of lipid metabolism were obtained by analyzing the inpatient medical records. Results. It was revealed that the values of TC, LDL-C, very low-density lipoprotein-cholesterol (VLDL-C), TG and atherogenic coefficient (AC) were lower in the group of patients with CAD and clinical gingival health compared to the level of the corresponding indicators in the groups of patients with CAD and GP (p<0.05). In the analysis of lipid profile in patients with CAD and GP of varying severity, it was found that the average levels of TC, LDL-C, and AC in the subjects of group I were significantly lower compared to the corresponding indicators of groups II and III (p<0.05). There was no statistical difference in the values of lipid metabolism in groups II and III (p>0.05). No significant difference was found in high-density lipoprotein-cholesterol (HDL-C) levels between the comparison groups including the patients with CAD and clinical gingival health (p>0.05), as well as the gender characteristics of the analyzed indicators in the examined patients. Conclusions. Periodontal tissue disease is one of the most common dental pathologies, which among young people occurs with a frequency of 60% to 99%. Therefore, the problem of finding new links in the pathogenesis, the reasons for the growing prevalence of periodontal disease, as well as effective methods for its early diagnosis and prevention, is relevant. Objectives. Establish the possibility of using individual stomatological and psychophysiological features to predict the development of periodontal disease. Materials and methods. 156 students aged 18-23 years old without systemic diseases were surveyed for some features of oral hygiene and nutrition. Also the study subjects underwent a dental examination, psychological testing and the assessment of individual typological features of higher nervous activity and autonomous regulation. The model for statistical prediction were designed using neural networks. Results. Two neural networks were designed with the best predictors among dental history and examination, psychological testing, parameters of higher nervous activity and heart rate variability analysis. The diagnostic sensitivity of the first prognostic model was 83.33 % and the specificity was 92.31 %. The second model was characterized by 90.00 % sensitivity and 78.57 % specificity. Conclusion. The method of modeling using neural networks based on the index assessment of the condition of teeth hard tissues, the level of oral hygiene and the evaluation of psychophysiological features can effectively predict the risk of periodontal disease development in young people","PeriodicalId":140699,"journal":{"name":"Proceedings of the Shevchenko Scientific Society. Acute respiratory infections are the most common infectious diseases worldwide among children of different age groups. Materials and methods. 59 children between the ages of 3 and 7 participated in the study. The first group included 22 patients with an acute respiratory viral infection, the second one consisted of 23 patients with acute respiratory viral infections associated with adenoid vegetation, and 14 apparently healthy children were included in the control group. Immunology research was conducted during the acute period of the disease. Statistical processing of received data was done with the standard statistical software EZR 1.41. Results. After the research, most of the patients with acute respiratory viral infections were identified an actual increase in CD3+, CD4+, CD8+, CD22+- cells and IgA, IgM in the blood serum. Simultaneously, in patients with acute respiratory viral infections associated with adenoid vegetation during the acute period, the increase in total lymphocytes was identified due to CD4+, CD8+, CD22+ cells and IgG. A comparative analysis of the study results of both groups of patients showed that children from the second group had a significantly higher level of CD3+- cells, while the CD22+- lymphocytes, IgA, IgM and IgG were significantly lower from the similar indicators of the first group. Conclusions. Medullary thyroid cancer (MTC) belongs to a class of rare neuroendocrine aggressive tumors and arises from parafollicular cells (C-cells). An important modern problem is the development of ways to predict the recurrence of this disease. The aim of the study is to determine the role of immunohistochemical tumor markers of medullary thyroid cancer in predicting recurrence or death. Materials and methods. The analysis of the prospective study included 22 patients with MTC, 5 of whom have developed a recurrence and 4 have died at the end of the 10-year (120 months) follow-up period. Immunohistochemical examinations were performed using monoclonal antibodies of tumor markers calcitonin, chromogranin A, vimentin and Ki-67. Results. The discrepancy between the data of histological and immunohistochemical examinations in MTC is 12.0%, which indicates the hyperdiagnosis of this nosology and argues the importance of performing immunohistochemical examinations to verify the diagnosis. Patients who had a recurrence of MTC had significantly (p <0.05) lower levels of calcitonin expression (5.00 [5.00; 5.00] points) compared with patients who did not relapse, where this figure was 6.00 [6.00; 7.00] points. In patients with MTC, an increase in calcitonin expression was significantly associated with an increase in chromogranin A expression (r = + 0.49, p = 0.02); a similar relationship was found for the proportions of immunopositive cells of these tumor markers: r = + 0.68, p = 0.001. At the same time, it was found that the increase in the level of calcitonin expression was apparently combined with the decrease in the level of Ki-67 expression (r = -0.52, p = 0.02). It was also found that the increase in the level of vimentin expression is combined with an increase in the expression (r = + 0.64, p = 0.001) and the proportion of immunopositive cells of chromogranin A (r = + 0.45, p = 0.038). Conclusions. Low levels of calcitonin expression are prognostically unfavorable markers for the recurrence of MTC. It is important to detect encephalitis, which develops a month or more after the manifestations of measles infection. These encephalitis are rare and occur mainly in people with immunodefi ciency. Multiple sclerosis is a chronic disease of the central nervous system for the treatment of which diseasemodifying therapy is used, namely monoclonal antibodies, that can lead to immunosuppression and immunodefi ciency. Nowadays, there is insuffi cient information about the course of postcortical encephalitis in patients with multiple sclerosis who are taking immunosuppressive drugs. The article presents data on the clinical classifi cation, diagnosis and treatment of measles encephalitis. A clinical case of measles inclusion body encephalitis in a thirty-threeyear-old patient with multiple sclerosis on the background of annual intake of monoclonal antibodies is presented. She also had viral-bacterial pneumonia and developed disseminated intravascular coagulation in the brain and lungs. These complications of measles infection led to the death of the person after a month and a half of intensive care. Thus, patients with multiple sclerosis who are taking drugs with immunosuppressive eff ects are among the risk group for measles inclusion body encephalitis. Measles inclusion body encephalitis in such patients can be severe, which complicates timely diagnosis, proper treatment and leads to death.","PeriodicalId":140699,"journal":{"name":"Proceedings of the Shevchenko Scientific Society. The aim was to determine the correlation between mean blood pressure and bilateral optic nerve sheath diameter, pulsatility index and mean flow rate of the middle cerebral artery in cases of preeclampsia/eclampsia. Methods: It was a prospective cohort study, including pregnant women diagnosed with preeclampsia/eclampsia who received medical attention in an Intensive Care Unit. The registered ultrasonographic measurements (optic nerve sheath diameter, pulsatility index and mean flow rate) were correlated against mean blood pressure at admission and after 24 and 72 hours. The recorded data were entered in an Excel sheet with subsequent use of the Social Science Statistics online web page. Results: The correlation analysis indicated that at admission, mean blood pressure had a significant negative correlation with mean flow rate (right: R2 = -0.52547, p = 0.04426 and left: R2 = -0.63628, p = 0.01077). In relation to optic nerve sheath diameter, the only positive and significant correlation was the one observed with mean blood pressure after 24 hours following admission (R2 = 0.56980, p = 02659). Conclusion: Mean flow rate measurements give more consistent and useful information than optic nerve sheath diameter in the hemodynamic evaluation of the preeclamptic women in the first 24 hours.","PeriodicalId":140699,"journal":{"name":"Proceedings of the Shevchenko Scientific Society. The use of in vitro fertilization and embryo transfer technologies determines the higher frequency of multiple pregnancies that enhances the risk of cesarean section, premature birth, low body weight in newborns, etc. in case of surrogacy. Genetic differences and immunogenetic reactions increase the risk of pre-eclampsia, placental pathology, abortion, intrauterine growth retardation. Hormonal disbalance under the effect of adverse factors during surrogate pregnancy may initiate prenatal reprogramming of morphogenesis through epigenetic mechanisms. This can affect the organogenesis and predispose to susceptibility to various diseases. Psychological factors and emotional link between a surrogate mother and a fetus are associated with increased levels of hormones of stress-releasing systems, which affect the formation of neurohumoral systems of the fetus and can modulate its mental development.","PeriodicalId":140699,"journal":{"name":"Proceedings of the Shevchenko Scientific Society.
