Diagnosis is often delayed due to minimal symptoms and signs. Elevated serum amylase or lipase levels on routine pre-operative laboratory investigations makes pancreatic injury highly likely. Contrast Enhanced CT scan is usually confirmatory.\u0000We are reporting on two patients who presented with blunt abdominal trauma, haemodynamically stable, and with epigastric tenderness. Pre-admission biochemical investigations revealed elevated serum amylase.  CECT demonstrated Grade III and Grade IV injuries which were treated with spleen-preserving distal pancreatectomy and pancreaticoduodenectomy, respectively. In genetically predisposed people, an environmental contaminant is thought to induce an immune-mediated attack against liver antigens. A wide range of clinical manifestations, from chronic indolent illness to fulminant hepatic failure, can be detected, and the diagnosis necessitates the exclusion of other causes of liver disease. Corticosteroid therapy must be started early and tailored to the person. The variety of clinical symptoms, uncertainty regarding the natural history, developing notions about therapeutic end targets, and the existence of many alternative immunosuppressive drugs can confound therapeutic decisions. The ultimate aim of the desired therapy is to achieve normal liver tests and tissue; however, this must be balanced against the danger of side effects. Early liver transplantation may be beneficial for decompensated patients. When therapy is started quickly and aggressively, the long-term prognosis is excellent. Isolated extrathoracic sarcoidosis has been reported in under 10% of the patients, and so isolated spleen sarcoidosis is extremely rare.\u0000A 68-year-old woman presented to our hospital with 6-month history of anorexia, left hypochondrial discomfort and early satiety, aggravated with high fever, fatigue and generalized myalgia two days before coming to the hospital. The physical examination detected splenomegaly. Laboratory analysis revealed pancytopenia with hemolysis and an elevated angiotensin converting enzyme. Image studies showed splenomegaly with multiple hypoechoic lesions. Infectious and autoimmune etiology was excluded. Bone marrow biopsy excluded lymphoproliferative disease. Patient underwent ultrasonography-guided spleen biopsy finding noncaseating epithelioid cell granulomas. The patient was started on corticosteroids with symptom improvement. Early pelvic fixation provides stability and should diminish ongoing hemorrhage. Materials and Methods: We undertook a retrospective and descriptive study of all cases of unstable open pelvic fractures treated with external fixators in the orthopedic and trauma surgery department of the Army Training Hospital Principal in Dakar over 4 years ranging from 1 January 2018 to 31 December 2022. Results: 12 patients were included in this study. The mean age of the patients was 39 years with extremes of 21 and 56 years.  The series consisted of 10 men (83.3%) and 2 women (16.7%), representing a sex ratio of 5. The etiology of the open fractures was road traffic accidents (61%), followed by public road accidents (33%), and finally falls from a high place (6%). The injury mechanisms were direct impact in 9 cases and 3 cases of indirect impact by shearing. Bone injuries were Tile B in 75% and Tile C in 25%. Soft tissue lesions were located in the perineum in 7 patients, in the inguinal area in 2 patients, and the buttock in 1 patient. Pelvic exofixation was performed in all patients with an average delay of 12 hours, with extremes of 4 and 24 hours. One case of infection was observed in the medium-term outcome. Three deaths were recorded. Radiographically, the reduction was satisfactory in 75% of cases (N=9) (Figs. 6 and 7), tolerable in 16.7% (N=2), and poor in 8.3% (N=1). Two patients had a residual disjunction of more than 5mm. Conclusion:  The external fixator in unstable open fractures of the pelvis appears to be a good alternative that finds its place within the limits of emergency internal osteosynthesis (hemodynamic instability, hemorrhagic and infectious risk). The pain of prostate biopsy is of immense challenge. Many factors have been ascribed to it. Identifying such risk factors will assist in mitigating the pain associated with this procedure. This study therefore aims to assess the role of histopathological outcome on pain of TRUS guided prostate biopsy. Methods: The study was a prospective randomized study carried out in University of Benin Teaching Hospital over a 1year period between 2017 and 2018. Consecutive patients who met indications for biopsy were randomized into Group A: intrarectal xylocaine gel group and Group B: periprostatic block group. Pain was assessed during probe insertion, biopsy and one hour post biopsy using an 11-point visual analogue scale. Association between mean pain scores and histological diagnosis in both groups was assessed using the independent t- test, association between use of intrarectal xylocain gel, periprostatic block was done using the independent t-test. Level of significance set at p <0.05. Results: There was no statistically significant difference in mean pain score during probe insertion, biopsy and post biopsy (p=0.3888), (p=0.089) and (p=0.584) respectively between benign and malignant histological diagnosis for Group A, while there was also no statistically significant difference in mean pain score during probe insertion, biopsy and post biopsy (p=0.266), (p=0.506) and (p=0.522) respectively between benign and malignant histological diagnosis in Group B. Cancer detection rate for Group A and Group B was 64.3% and 59.1% respectively, which was not statistically significant p=0.662. Conclusions: The study demonstrated that pain of TRUS guided prostate biopsy is not influenced by histopathological outcome irrespective of mode of anaesthesia. Objective of present study is to investigate CS rates according to ROBSON classification system and to determine area of concern to decrease overall C-section rates. Methods: This study is a retrospective descriptive cohort study done with all caesarean sections delivered in a tertiary teaching hospital in the west coast of Gujarat, India from January 2022 to December 2022. Results: In current study, total CS rate was 28.47%. The major contributor to total CS rate was ROBSON I group (34.09%) followed by ROBSON V (26.34%). Major indications for ROBSON I group include fetal distress due to meconium-stained liquor (25%), Cephalo-pelvic disproportion (CPD) stage1(18%). Major indications for ROBSON group V includes CPD 1(31%), scar tendereness(23%) and previous 2 LSCS(14%). Conclusion: ROBSON I and V being major contributors to total CS suggest to focus on these groups to reduce total CS rate. The condition is considered an emergency and needs urgent recognition and management before the affected patient develops peritonitis or bowel infarction. Case presentation: We report a case of a 70-year-old man with quadriplegia who experienced abdominal pain and constipation for 2 days. Abdominal radiography showed distended colon, and computed tomography (CT) revealed closed-loop obstruction with signs of bowel ischemia. Albeit rare, many complications are reported and extend from cough and dyspnea to sudden cardiac death. The case presented herein is the first large and localized hemorrhagic pericardial cyst mimicking an acute coronary syndrome (ACS) with intense chest pain, precordial ST-segment elevation, and elevated troponin.\u0000Following thorough investigations, this cyst appeared to be associated with moderate pericardial effusion explaining myopericarditis as a cause of the findings listed. It has allowed surgeons to perform more procedures with a remarkable time grain and to reduce the cost. The objective of our study is to evaluate the interest of this technique in the management of upper limb trauma in the emergency room. Results: The study included 36 patients, 14 fractures and 22 wounds. The average waiting time in the emergency room was 82.9 minutes, the average time between anesthesia and incision was 37 minutes, and the average duration of the procedure was 37 minutes. Three patients whose procedure was prolonged required additional anesthetic, and no patient required sedation or general anesthesia. None of the patients required the use of a tourniquet. No complications were reported in these patients. 92% of patients were satisfied. Discussion: WALANT makes it possible to perform surgical procedures using only lidocaine and adrenaline in combination with bicarbonate and saline. The long-standing dogma about the involvement of adrenaline in ischemic necrosis of the fingers has been reconsidered by several authors. Several advantages have been reported in the various publications. This concerns the patient, the surgeon and the health care system. Thus, it allows to improve the result in tendon repairs and to verify the stability of the osteosynthesis intraoperatively. Osman","doi":"10.23958/ijirms/vol08-i04/1642","DOIUrl":"https://doi.org/10.23958/ijirms/vol08-i04/1642","url":null,"abstract":"Background: Warfarin is a common anticoagulant drug that has a narrow therapeutic index; a higher dose causes excessive bleeding and a lower dose leads to cerebrovascular clotting and stroke in patients. The management of warfarin therapy is challenging because there is variability in patient response due to many factors. Genetic factors that are most relevant, such as CYP2C9, and andVKOR, are the target site for warfarin. The study aimed to investigate the association between CYP2C9*2, VKORC1 (-1639 G>A), and GGCX T>G polymorphisms with warfarin daily dose in Sudanese patients on warfarin treatment referred to anti-coagulation clinics in Khartoum State, Sudan. Method: A cross-sectional descriptive study was conducted on randomly selected 107 patients on warfarin with different clinical indications. Their genotype was analyzed by Polymerase Chain Reaction (PCR) and Restriction Fragment Length Polymorphism (RFLP) to determine the polymorphisms. Result: The study revealed that CYP2C9 genotype wild type was more frequent than heterozygote. For VKORC1 the frequency of the A allele was 61.8% and for the G allele was 38.2%, the GGCX genotype was observed only in wild-type and homozygous genotypes (89.7%& 10.3%, respectively). The t allele of GGCX was higher than the G alleles. Conclusion: The most common polymorphisms that revealed high significance on warfarin dose determination were VKORC1A/giving evidence to new guidelines dose requirements according to the patient genotype.
