Various factors contribute to the recurrence of LDH, including patient-related, surgical, and biomechanical factors. Despite attempts to determine the best surgical techniques and patient selection criteria, there is inconsistent evidence that some factors increase the likelihood of recurrence. Repeat discectomy has been shown to be a safe and effective surgical option for recurrent LDH cases that do not respond to non-operative management. However, selecting the most suitable surgical intervention, such as endoscopic or conventional discectomy with or without instrumented fusion, can be difficult. Factors such as presenting symptoms, previous surgeries or re-herniations, radiographic instability, sagittal or coronal deformity, and surgeon experience should all be considered when making this decision. The importance of an unbiased and protected Peer Review System is codified in the Health Care Quality Improvement Act of 1986, HCQIA. However, the peer review process may go wrong when in the new landscape of healthcare which is dominated by large hospital organizations and the big business of medicine, the peer review system may be misused for reasons other than to ensure compliance to the highest standards of professionalism in the interest of the public and the profession. In those instances, due to the immunity protection, which is afforded by HCQIA, contrived allegations of incompetence or disruptive behavior may be used to retaliate against physicians. Most physicians are not familiar with these complex issues which can affect their careers. Main advantages of ultrasound technique include a direct view of nerves; local anaesthetic (LA) spreading during injection; reduced volume of LA; blood vessels and other structures injury is significantly reduced. It is especially useful in patients with serious comorbidities, in which possible peri-operative consequences and risks of general anaesthesia can be avoided. This ectopic endometrial tissue exhibits cyclic bleeding, inflammation, fibrosis and leads to formation of adhesions. Endometriosis affects about 10% to 15% of women between 15 to 45 years of age. Clinical presentations vary from infertility, dysmenorrhoea, chronic pelvic pain, deep dyspareunia and even bleeding at external sites like the umbilicus. Besides involving fallopian tubes, bowel, liver, thorax, pericardium, pleura etc, the most commonly affected areas in the gastrointestinal tract are the descending colon, rectosigmoid, appendix, and ileo-caecum in descending order of frequency. This case series highlights some unusual presentations of endometriosis along with co-existence of other ovarian pathologies which are not frequently encountered in clinical practice. A thorough understanding of relationship between gastrointestinal (GI) disorders and skin diseases can alert the clinician to an underlying occult GI disorder. The aim of the study was to review the cutaneous changes in patients with co-existent, often masked GI diseases and to observe the underlying pathophysiology of such lesions. This was a retrospective, record based observational study. The archived histopathology reports & medical records of patients over a period of 5 years were searched and all patients who underwent skin biopsy with co-existent GI diseases were included. Out of a total of 218 patients of GI diseases who had dermatological manifestations, only 44 were biopsied. Pruritis (n=175), koilonychia (n=52) and erythematous rash (n=35) were the most common clinical features. When respective injuries are characterized as unstable, surgical reduction and stabilization is necessary. Generally, these operations are conducted under regional or general anesthesia to avoid pain and tourniquet for bloodless surgical fields. Wide Awake local anesthesia no tourniquet (WALANT) procedure is described as a local injection of anesthetic agent and epinephrine without tourniquet application, with scope to allow local anesthesia without other sedation and at the same time hemostasis. The purpose of the study is to compare the functional assessment of the hand function during a period of three months after surgical intervention for unstable metacarpal and phalangeal fractures under WALANT versus Biers block. Material &Methods: Between May 2021 to October 2022, 43 patients with unstable metacarpal and phalangeal fractures underwent open reduction and internal fixation, 21 cases with Biers block(BB) anesthesia and rest with WALANT technique. Metacarpal fractures appeared in 29 cases and phalangeal fractures in 14 patients. Patients were operated within the first week from the injury. According to implants they employed locking plates, screws and Kirschner wires in two groups. Results: At three months postoperatively, Quick Dash Score is better in WALANT group while patients’ satisfaction does not differ between two groups. In WALANT group immediate postoperative median VA Score appeared 2/10, while in the Biers block group appeared 6/10. Patients which underwent WALANT technique were discharged on the same day after operation, while the patients with regional anesthesia the next postoperative day. Mean duration of analgesic use was sorter in WALANT group (2,3 days) than in BB group (5,7 days). Conclusion: Οbjective of surgical treatment of unstable hand fractures (metacarpal-phalangeal) is early mobilization and enhance hand functionality to avoid stiffness and tendon adhesions. Wide Awake local anesthesia no tourniquet technique is an alternative sedative procedure which immediately offers postoperative numerous advantages (low-cost technique, no complication from tourniquet, decrease sensation of intra or postoperative pain) compared with Bier’s block anesthesia. The increase of parathyroid hormones can be primary hyperparathyroidism (HPT) and secondary HPT resulting from renal failure and vitamin D deficiency. Case presentation: A 26-year-old Thai female visited the clinic because of right leg pain and swelling for three weeks. Plain radiography revealed a solitary osteolytic lesion with an impending fracture at the anterior cortex of the right proximal tibia. The patient underwent a core needle biopsy. The initial histopathology report depicted a giant cell lesion. In addition, the patient had elevated serum parathyroid hormones. A parathyroid adenoma was discovered on a further investigation with contrast-enhanced computed tomography of the neck. The immunochemistry and molecular study report were consistent with a brown tumor. The patient underwent parathyroidectomy for her adenoma and was on weight-bearing restrictions for her leg. After three months of follow-up, she could bear weight on her right leg without pain, and radiographic evaluation showed restoration of the tibial cortex. Conclusion: Conservative management has a role in managing brown tumors of the lower extremities with impending pathologic fracture. Methods: A total of 387 women, 234 of whom were non-obese and 153 obese, residing in the central district of İzmir in 2022 were subjected to the study. A questionnaire including demographic information form, Digital Literacy Scale and Attitude Scale towards Healthy Eating was applied to the participants. Results: Age, economic status, chronic disease, malnutrition and family obesity history distributions were significantly different between obese and non-obese groups (p<0.05). Body Mass Index (BMI) and Malnutrition (MN) means were higher in non-obese group, and digital literacy, Information on Nutrition (IN), Emotion for Nutrition (EN), Positive Nutrition (PN) and Healthy Nutrition Behavior (HNB) total scores were higher in obese group. The digital literacy level difference was significant between patient groups (p<0.05). Digital literacy was positively correlated with IN (r=0.184; p<0.01), and negatively correlated with positive nutrition (r=-0.326; p<0.01) and malnutrition (r=-0.142; p<0.05) in non-obese group. Digital literacy was negatively correlated with malnutrition (r=-0.255; p<0.05) in obese group. Digital literacy level has significant and negative correlation with HNB (r=-0.121; p<0.05). Conclusion: The level of digital literacy significantly affects the level of health knowledge, positive nutrition and malnutrition in non-obese women. Yan, Tian Meng","doi":"10.23958/ijirms/vol08-i03/1632","DOIUrl":"https://doi.org/10.23958/ijirms/vol08-i03/1632","url":null,"abstract":"Objective: It was to investigate the relationship between high thyrotropin, thyroid stimulating hormone (TSH) degree and changes of low-density lipoprotein cholesterol (LDL-C) in Yunnan Plateau pregnant patients’ serum in China; Methods: All pregnant woman were collected from the Kunming Angel Women's and Children's Hospital from January 2021 to October 2022, who were diagnosed by the endocrinology department, during their pregnancy. Then it analyzed their biochemical indicators levels and blood liquid parameters changes in their serum. Additionally, we investigated their epidemiological data, including the treatment history of hypothyroidism; Results: High-TSH degree and increasing of LDL-C level were both risk factors leading to hypertension, cardiovascular symptoms, symptoms of digestive tract, pregnancy ultrasound indicating small-sized fetus (P<0.05). Importantly, they reinforce each other on patients. Their increasing caused gestational diabetes mellitus, preterm birth history and autoimmune diseases in Yunnan plateau (China). TSH≥4.3 m IU/L and DL-C≥3.6 mmol/L in the pregnant patients’ serum, which were approved by obstetric professional committee of that Hospital. It contributes significantly to fetal and maternal mortality especially in the developing countries. Proper antenatal care and prompt intervention is necessary to forestall adverse and improve outcome.</p><p><strong>Objective: </strong>To determine the prevalence, sociodemographic characteristics, risk factors, fetomaternal outcome of pregnancies with antepartum haemorrhage.</p><p><strong>Methods: </strong>The case files of the patients were retrieved from the medical records department. The total number of deliveries within the study period was obtained from the labour ward records. The feto-maternal-outcome-measures were; prevalence of caesarean-section, postpartum-haemorrhage, hysterectomy, need for blood-transfusion, maternal-death, prematurity, need for admission in intensive-care-unit and still births. The data was analysed using SPSS version 21. Chi-square was used to test for significance.</p><p><strong>Results: </strong>Within the 5-year period under review, out of a total of 6974 deliveries, 234 had antepartum-haemorrhage (3.4% prevalence rate). Abruptio-placentae was the commonest cause and accounted for 69.5% of the cases (prevalence of 2.1%) while placenta praevia accounted for 28.2% of the cases (prevalence rate of 0.9%). The mean age of the women was 31.8±5.3 years. The mean parity was 3.4±1.7 and majority (63.8%) of the women were unbooked. The commonest identifiable risk factors were multiparity and advanced maternal age. One-hundred-and sixty-six (77.9%) women were delivered through the abdominal route. Postpartum-haemorrhage occurred in 22.1% (47) of the cases while prematurity was the commonest fetal complications. Maternal mortality was 0.47% (1) while still birth was 44.1% (94).</p><p><strong>Conclusion: </strong>There is high prevalence of antepartum-haemorrhage in our environment. Abruptio-placentae was the commonest cause and associated with significant adverse fetomaternal-outcome when compared with placenta-praevia.
