We present here the case of Unilateral left pulmonary artery agenesis with pulmonary hypertension. Case Report: A 33-year young male patient presented to our tertiary care hospital in the out-patient department with complaints of shortness of breath on exertion for 7 months and bilateral feet swelling for 6 months. Routine investigations were within normal limits except 2 dimensional ECHO showed right atrium and right ventricle dilatation with severe pulmonary artery hypertension and 60% left ventricular ejection fraction. Patients with isolated right pulmonary artery agenesis commonly survive into adulthood with minimal or no symptoms, which makes their identification challenging. But our patient had left pulmonary artery agenesis which has significant association of life threatening cardiovascular malformations with left-sided UAPA and surgical repair is often required during the first year of life. None of which is present in our patient which is unsusual. Conclusion: Clinicians should be aware that recurrent respiratory infections may be presenting feature of UAPA. Initial investigation is usually a chest radiograph. Echocardiography is required for evaluation of possible pulmonary hypertension. Confirmation of the diagnosis and anatomic details can be discerned by CT scanning and MRI. Angiography is reserved for patients requiring embolization or revascularization surgery. Present case demonstrates a rare presentation of UAPA as right heart failure in first visit.","PeriodicalId":13918,"journal":{"name":"International Journal of Contemporary Medical Research [IJCMR]","volume":"20 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2019-10-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"79366457","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

More than 75% of Indian children under 36 months of age are found to be anemic. Iron-deficiency anemia (IDA) has been found to be correlated with decreased cognitive performance in many studies, hence prompt diagnosis and treatment are necessary. Pallor is a clinical sign in the diagnosis of anemia. Current research aimed to study about the predictive value of pallor in various sites and its relation to anemia. Material and methods: 500 children attending the outpatient department of pediatrics were evaluated for clinical pallor and Hemoglobin was estomated by cyanmethemoglobin method. Results: When the various parameters were compared between conjunctival, palmar, mucosal and nail bed pallor; it has been observed that sensitivity was highest for nail bed pallor in the evaluation of anemia. Specificity was highest for conjunctival pallor. The positive predictive value and negative predictive value were both highest for conjunctival pallor and least for palmar pallor. However, when all the evaluated parameters were compared in patients with severe anemia, highest sensitivity and specificity was observed for palmar pallor. The positive predictive value was highest for palmar pallor and least for nail bed pallor. The negative predictive value was highest for conjunctival pallor and least for nail bed pallor. Conclusion: Clinical pallor of the nailbeds had highest sensitivity for detection of moderate anemia while palmar pallor had the highest sensitivity for the detection of severe anemia. These are simple clinical signs which can be read even by non doctors and can thus be taught to various personnel like health workers and school teachers for early detection and prompt management of anemia in children below 5 years.","PeriodicalId":13918,"journal":{"name":"International Journal of Contemporary Medical Research [IJCMR]","volume":"8 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2019-10-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"83661287","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

CSOM can severely impact quality of life (QoL) of patients. QoL is the latest indicator of health care. The aim was to assess the quality of life in CSOM (Chronic suppurative otitis media) patients in a tertiary care centre. Materials and methods: It was a descriptive epidemiological study and sample size is approximately 60 using a Questionnaire by interview method the CES questionnaire (Activity Restriction, Symptom and Medical Resource). Results: In the end we came to know QoL is not affected in CSOM patients (p<.05) in terms of age, gender with activity restriction, symptom and medical resource. Conclusion: Hence concluded that quality of life is not affected in csom patients and though the health seeking behavior has increased, the knowledge about the disease is minimum. We have to promote health education about the disease. Further studies with larger sample size necessary to get significant correlation in QoL of csom patients.","PeriodicalId":13918,"journal":{"name":"International Journal of Contemporary Medical Research [IJCMR]","volume":"8 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2019-10-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"89950711","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Vijayakumar, S. Bhandari","doi":"10.21276/ijcmr.2019.6.10.32","DOIUrl":"https://doi.org/10.21276/ijcmr.2019.6.10.32","url":null,"abstract":"Introduction: Use of autopolymerising resin for direct provisionalization is one of the commonest methods employed by clinicians. However, these materials exhibit an exothermic reaction and are prone to cause pulpal damage. Thus, a combination of cooling technique and matrix material to reduce the quantum of heat transferred to a healthy vital pulpal tissue may be employed for minimal tissue damage. An ex-vivo study was thus envisaged to compare the efficacy of various matrix materials and cooling techniques on time related temperature changes in the pulp chamber during the fabrication of provisional FPD restorations using direct technique. Material and Methods: A total of 100 provisional restorations were fabricated and were grouped on the basis of three different matrices: Vacuum template, Putty index and Alginate index and further subgrouped on the basis of various cooling techniques employed. Thermal changes in the pulp chamber was evaluated using Cr/ Al thermocouple placed in the pulp chamber of a prepared tooth and connected to a digital thermometer. Intrapulpal temperature variations were recorded, tabulated and statistically analysed. Results: The inter-group and intra-group statistical comparisons of continuous variables were done using analysis of variance with Bonferroni’s correction for multiple group comparisons considering the independent study groups. The underlying normality assumption was tested before subjecting each variable to t test and ANOVA. In the entire study, the p-values less than 0.05 were considered to be statistically significant. All the hypotheses were formulated using two tailed alternatives against each null hypothesis (hypothesis of no difference). The entire data was statistically analysed using Statistical Package for Social Sciences (SPSS ver 21.0, IBM Corporation, USA) for MS Windows. Conclusion: The order of heat dissipation for the different matrices used is as follows: Irreversible hydrocolloid matrix > PVS putty matrix > vacuum form template. It was concluded that intrapulpal temperature rise during the direct fabrication of provisional restorations can be limited by employing different cooling techniques. “Cooling” of PVS putty and alginate impression index for 2 minutes in refrigerator (Ambient temperature 10-120C) was found to be most effective “cooling” method for preventing temperature rise in pulp.","PeriodicalId":13918,"journal":{"name":"International Journal of Contemporary Medical Research [IJCMR]","volume":"55 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2019-10-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"79427680","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
