Recurrent epistaxis is common in children. Although epistaxis in children usually occurred due to different benign conditions, it may be one of the important presenting symptoms of some inherited bleeding disorder. Whereas most bleeding disorders can be diagnosed through different standard hematologic assessments, diagnosing rare platelet function disorders may be challenging. In this article we describe one case report of platelet function disorders on Glanzmann’s thrombasthenia (GT). Our patient was a 10-year old girl who presented to us with history of recurrent severe epistaxis. She had a bruise on her abdomen and many scattered petechiae in different parts of the body. Her previous investigations revealed no demonstrable haemostatic anomalies. Taking bone marrow biopsy alongside aspirate is still the most preferred practice for precise diagnosis and evaluation of various haematological and non- haematological disorders. Aims and objective: This study was carried out to evaluate the importance of this procedure in the diagnosis of various haematological and non-haematological disorders especially when bone marrow aspirates alone are non-diagnostic and to assess the prognostic significance of haematological malignancy. Materials and Methods: This was a retrospective study using the trephine biopsy and aspiration reports extracted from hospital records of Delta Medical College Hospital, Dhaka, over an 8years period from May 2009 to December 2016.The patient's profiles along with corresponding diagnoses and the necessary investigation reports were analysed. Result: Eighteen (26.47%) patients had bone marrow involvement for non-Hodgkin's lymphoma, three (4.41%) patients for Hodgkin's lymphoma, and acute lymphoblastic leukaemia was diagnosed in 18 (13.43%) patients, metastatic deposits in 6 (4.5%) patients, acute promyelocytic leukaemia in 3 (2.2%) cases, aplastic anaemia in 7 (5.2%) cases, chronic lymphocytic leukaemia in 1 (0.75%) case, multiple myeloma in 3 (2.2%) cases, myelofibrosis in 6 (4.5%) cases and chronic myeloid leukaemia and immune thrombocytopenic purpura were found in less than 1% cases. Total 37 patients (54.41 %) were reported as normocellular marrow with normal maturation among all the cases of lymphomas (N=68). One trephine biopsy was carried out to assess remission after induction chemotherapy in ALL. Conclusion: Trephine biopsy is an invaluable diagnostic tool in case of diagnostic dilemma and for follow up of patients undergoing chemotherapy and bone marrow transplantation. Therefore, prompt administration of appropriate antimicrobial drug therapy is often essential to prevent mortality and morbidity in such patients. Spectrum of the infection causing organisms is continuously changing throughout the world along with their susceptibility to antimicrobials drugs. Aims: The aims of the study were to identify the bacterial pattern and their susceptibility to antimicrobials in cancer patients with infection and to help develop a guideline for the rational and appropriate use of antimicrobial in these patients. Materials and methods: It was an observational study conducted from January 2013 to December 2014 at the National Institute of Cancer Research and Hospital (NICRH), Mohakhali, Dhaka. We evaluated 517 blood and other specimens from cancer patients of both sexes and all ages under anticancer therapy suspected of having infection. Result: Blood culture was done in 53 patients of them 8 (15.09%) were proved to be culture positive. In case of urine and wound culture the rates of positivity were 37.79% & 73.98% respectively. In suspected cases culture for anaerobic organism was done but no such organism was found. Major isolated organisms in different cultures were E. coli, Staphylococcus aureus, Pseudomonas and Proteus. Imipenem, gentamicin, cotrimoxazole were the most sensitive antibiotics. However, in most cases amoxicillin, cephalexin and nalidixic acid were mostly resistant. It may be primary or secondary. The latest classification criteria (Sydney 2006) recognize just three tests to define this syndrome- lupus anticoagulant, anticardiolipin antibodies and anti-?2-glycoprotein-1 antibodies. Treatment of thrombotic events involves lifelong anticoagulation with vitamin K antagonist warfarin. Antiphospholipid antibody syndrome (APS) with only pregnancy morbidity is treated with thromboprophylaxis with heparin during pregnancy and postpartum for 6 weeks. It is a rare entity and most frequently associated with acute myeloid leukaemia (AML) but can occur with other myeloproliferative disorders. It may occur at any site, leading to very varied clinical presentations. Although it can occur in a number of areas of the body, the involvement of the breast is uncommon. We present a rare case of MS in a 14?year?old boy with a non-tender lump in the left breast that had been apparent for five months. Available diagnostic techniques, including ultrasound and magnetic resonance imaging were systematically performed. After mastectomy, biopsy and immunohistochemistry was done. Immuno-histochemical stains were positive for CD45 (haematological marker) and myeloid markers, such as myeloperoxidase (MPO), and CD68, CD43 suggesting the diagnosis of MS. Although there are established guidelines for the use of empirical therapy in febrile neutropenia, local microbiological pattern and antibiotic susceptibility should be considered for better outcome. There is paucity of data regarding the organism isolated and sensitivity in febrile neutropenic patients in Dhaka Medical College Hospital (DMCH). Objectives: The current study aimed to find out causative organisms for febrile neutropenia and to observe their antimicrobial sensitivity pattern. Materials and method:  This was a prospective observational study carried out in department of haematology and BMT of Dhaka Medical college hospital during the period of October, 2014 to July, 2015. Total 104 patients experienced febrile neutropenia and both febrile blood and urine samples were collected and sent for culture. Results: Out of 104 patients male were 65 and female were 39 (M:F ratio 1.6:1) with a mean age of patients were 31.3 (±15.43) years. Majority (86.5%) of febrile neutropenic patients were post-chemotherapy of acute leukaemia. Out of 104 case 29 were positive growth in both blood and urine samples. Out of 25 blood culture positive patients’ organisms isolated were predominantly gram negative (78.6%) whereas 21.4% were gram negative.  However, in urine culture equal number of organisms (4 vs 4) from both gram positive and gram negative were isolated. Gram negative microorganisms were resistance to antibiotic such as amoxicillin, ceftazidime, ceftriaxone, cefixime, cotrimoxazole and gentamicin and sensitive to antibiotic like meropenem, amikacin, ciprofloxacin and colistin. Gram positive microorganisms were almost resistance to azithromycin, cefixime, ciprofloxacin, cotrimoxazole, gentamicin and moxifloxacin but were sensitive to chloramphenicol, clindamycin and linezolid. Conclusions: Gram negative organisms were the predominant organisms in febrile neutropenic patients at our institute. Although surgical resection is the foundation of curative therapy, with the use of effective neoadjuvant and adjuvant chemotherapy the 5 years overall survival of patient with hepatoblastoma had recently reached up to 80% to 90 %. But the frequency and severity of haematological toxicity is one of the major concerns of intensive chemotherapy. Objective: To evaluate the frequency and severity of haematological toxicities following treatment of childhood hepatoblastoma with two different regimens of multi-agent chemotherapy. Methodology:  This was a comparative observational study conducted at Bangabandhu Sheikh Mujib Medical university in 24 childhood hepatoblastoma patients who received either cisplatin/carboplatin /doxorubicin (Group A; n=14) or cisplatin/ vincristine/5- fluorouracil (Group B; n= 10) to between March 2010 and July 2014. Results: Grade 3 or 4 anaemia and thrombocytopenia, and Grade 4 neutropenia were observed in 64.3% vs 60%, 35.7% vs 20% and 57.1% vs 20% respectively in group A and Group B. Febrile neutropenia occurred in 100% vs 70% and septicaemia developed in 92.9% vs 70%. Red cell concentrates and Platelet transfusions were required in 35.7% vs 50% and 14.3% vs 10% respectively in two groups. Deaths due to haematological toxicities occurred in 21.4% in Group A and 21% in Group B.
