Through a specific process involving 10 repeated interactive transits incurred via autohemotherapy, the blood is “fused” with 200 ml of ozone gas respectively brought to a concentration of 70 mcg/ml, thus providing a cumulative ozone intake of 140,000 mcg. \u0000The evolution of the ozonotherapeutic treatment promoted by Lahodny would then consist, in a way, in breaking the restrictive rules relating to pragmatic dosage which tended to prevail until now with regard to a usual protocol which still remains well anchored in traditional customs and habits. Indeed, any former procedure usually involved a single injection of ozonated autologous blood therapy containing 200 ml of ozone at a concentration of 40 mcg, after which the session would end. This conceptual limit was based on an arbitrary credo that exceeding \u0000this dose boundary—in terms of both quantity and concentration administered—would be likely to result in cell bursting by blood dissolution, a supputive risk whose consequences had previously been considered hypothetically lethal. \u0000In 2014, he discovered and refined a new dosage—L1—that he believes would activate the growth of stem cells and the production of adenosine triphosphate (ATP). Its application can be summed up as a major autologous blood therapy combined with massive doses of ozone injections both in terms of concentration — up to 70 mcg/ml in ozon IV or even 80 mcg/ml in rectal insufflation - and quantity: which generally implied 140,000 mcg, but sometimes up to 280,000 mcg in the treatment of more complex conditions. Partial correction of symptoms cannot balance multiple systemic dysfunctions and mitochondrial activity. In this context,there is a risk of neurodegenerative developments, which are announced by the occurrence of partial or total resistance to pharmacological therapies. \u0000Ozone is a factor with antioxidant, antiapoptotic action and which improves autophagy, a phenomenon that can improve mitochondrial dysfunction. The diabetic patient often gets complications, through an inadequate lifestyle or by giving up treatment, most often due to polypragmasia. Oxygen-ozone treatment has remarkable effects in patients with diabetic foot (DFN or DFU). \u0000A retrospective study of 50 patients with type 2 diabetes (25 with DFU and 25 with DFN), treated with oxygenozone mixture for a minimum duration of 3 months, showed the positive effect of treatment with accelerated wound healing, 70% of them having a skin lesion for more than one year. Decreased glycosylated hemoglobin, blood pressure regulation and increased distal skin sensitivity were added to the healing of skin lesions, using only oxygen-ozone therapy (systemic and local) combining personalized diet, physical activity and the correction of vitamin D levels. Out of all the 45 cases that have been studied in the timeline 2020-2022, some of them who were incredibly severe with little hope for improvement, had a tremendous impact on me and I will try to describe them in a few words and raise your awareness about this not yet explored field that has real potential in aiding those who suffer. A female patient of 49 years old had diabetes mellitus, multiple necrotic lesions, post peritoneal dialysis with skin calcifications of both legs, chronic kidney disease, renal osteodystrophy, calciphylaxis with secondary cutaneous necrosis in remission, renal lithiasis, secondary arterial hypertension, ischemic cardiac disease, paroxistic atrial fibrillation, polycystic kidney disease, hyperuricemia, calcium metabolism alterations, hyperphosphatemia, moderate proteic malnutrition. The first ozone therapy session was done according to ozone therapy protocol for the diabetic foot, after debridement, disinfection of the wounds and disposal of the necrotizing tissue. Drainage from the wound was examined and revealed an E. coli infection. After 30 sessions, the result was spectacular. A 51-year-old patient presented to the clinic with an open wound after amputation of his left leg. A year after, he came back to the clinic with an infected wet gangrene of his right leg, who also caught the first 3 toes. His medical history included type II diabetes mellitus, end stage renal disease with a double kidney transplant until the age of 28. In both cases debridement of the wound was done, exposing healthy tissue, antibiotics were administered according to antibiogram and the entire procedure of ozone therapy was done as follows: washing the wound with oxygenated water, disinfection of the wound, local infiltrations with ozone, major autohemotherapy and ozonized bag. The result: the left leg had a rapid favorable outcome, and after only 20 ozone sessions, the wound closed. The right leg had a total of 45 ozone therapy sessions done, with a 100% improvement. After 5 months, the patient came to the clinic with a postblockage necrotizing fistula on his right second finger, which healed after 6 ozone therapy sessions. Among those 45 patients included in the study, 10 of those who had major amputations presented to the clinic for a primary intervention and only 2 of them for a reintervention. Nevertheless, most of the minor amputations were done as a primary intervention. In many cases though, to obtain the healing of the patients, reinterventions were necessary. Analyzing these data, the necessity of applying a new method of correctly assessing the severity of the lesions to avoid an undervaluation of it, which would unnecessarily prolong the hospitalization and more importantly their suffering. I have encountered 4 cases of extensive wet gangrene associated with severe sepsis. The mortality in this category of patients was 50%. Arterial hypertension (AH) is a particularly important public health issue in Mongolia which has one of the highest mortality rates from hemorrhagic stroke in the world. Antihypertensive pharmacological therapy combined by other non-medication therapy is well-studied in patients with mild and moderate AH. However, studies that have investigated combination of low intensity laser irradiation and ozone therapy in patients with severe AH are lacking. The objective of the present work was to develop methods using low intensity laser irradiation therapy (LLLT) combined with major autohemotherapy (MAH) for the patients presenting with AH. \u0000Studies have shown that LLLT is capable of inducing a photobiological response within the cells which modifies the micro and macrovascular response; this accompanies evidence showing the systemic effects even using in nasal laser irradiation (NLI). \u0000Oxygen-ozone therapy exerts a positive effect on the platelet aggregation, cell remodeling, cytoskeletal elements organization and mitochondria structure. Due to O2-O3 therapy is an effective method in hypertension, and it diminishes the hypoxia state of various tissues. \u0000In recent years, scientists have made great progress in the area of pharmacotherapy for patients with AH. Drugs help to achieve the target level of arterial blood pressure, particularly in patients with mild and moderate AH. With a higher degree of AH (severe arterial hypertension – the 3rd degree of arterial hypertension) the effectiveness of pharmacological therapy decreases and to achieve the target level of arterial blood pressure it is necessary to use a combination of all major groups of antihypertensive medicines. \u0000 \u0000Method and Result: The study was held at Wellness Med Hospital Mongolia since the beginning of 2021 until the end of year. The study included a total of 60 patients of any age (63% of men and 37% of women) having AH associated with coronary heart disease and abnormal blood lipid spectrum. Initially, clinical-instrumental and laboratory examination, blood pressure registration and daily arterial blood pressure monitoring were carried out. Suitable drugs were selected for all the patients. 3 months after starting drug treatment the patients were divided in 2 groups each comprising 30 patients matched for age, sex, severity of clinical conditions, and character. The patients of the first group were prescribed combination with NLI and ozone therapy (MAH) in addition to drugs. For ozone therapy used Ozomed smartline ozone generator by Hansler Medical, Germany. The total dose of ozone to be given at each session will vary according to the general condition of the patient. Intervals from daily to weekly and twice a week. Generally, chondropathies are first treated with drugs and physiotherapy, but their progress often requires the implantation of a prosthesis. \u0000The combined treatment with ozone, hyaluronic acid and platelet rich plasma (PRP) I set up allows achieving excellent results in terms of joint functional restoration and benefit duration. In particular, using an appropriate protocol, the local application of oxygen-ozone (intra- and extra-articular), low-molecular weight hyaluronic acid and PRP induces the stabilization of the degenerative process, likely due to the synergistic interaction of the three therapeutic substances. In addition, the local combined therapy is always associated with both a systemic treatment with oxygen-ozone (via the venous or rectal route) and a rehabilitation therapy. As a whole, this therapeutic approach leads to a gradual disappearance of pain and to the resumption of the joint functional activity, with a consequent improvement of the patient’s quality of life. \u0000In my personal case histories, the success rate of this treatment for chondropathies is very high, also thanks to the possibility of maintaining for long time the benefits by means of boosters (maintenance therapy). This means that it will have different effects on multiple pharmacological targets, which have been identified in hundreds of studies (experimental and clinical) on disease models and patients. These targets are closely connected to each other and can occur in different diseases. A good example is the “cytokine storm” in rheumatoid arthritis as well as in COVID-19/SARS-CoV-2. Therefore, the aim of this presentation is to highlight the therapeutic targets of medical ozone in different diseases, their interrelationship, and ozone application in pathological processes with an oxidative etiology. \u0000In animal models and in patients with rheumatoid arthritis, medical ozone has been shown to reduce proinflammatory cytokines (TNF-? and IL-1?) that contribute to “cytokine storm,” to synovitis, cartilage- and bone damaging autoantibodies and oxidative stress. Vascular complications in patients with diabetes were reduced when superoxide radical scavengers increased, mediated by medical ozone, restoring normal metabolic pathway functions and pancreatic integrity. Marcvart","doi":"10.7203/jo3t.6.7.2022.25992","DOIUrl":"https://doi.org/10.7203/jo3t.6.7.2022.25992","url":null,"abstract":"This work connects extensive literature research of the last three decades on medical ozone and the personal clinical experience (more than ten years) of ozone therapy. \u0000Classical and more recent concepts were considered: hormesis and lowdose concept, ozone action through messenger-molecules – reactive species (well-studied H?O? and 4-HNE), their implication in the redox homeostasis and in cellular signalling and the consecutive cellular response by adaptation or apoptosis. Another issue in the therapy is a correct diagnosis – based on molecular mechanisms. Many current diseases are sharing commune etiopathogenesis – redox homeostasis alteration, mitochondrial dysfunction, chronic inflammation, endothelial disturbances, early cellular senescence, accelerated apoptosis. As such, a new therapeutic concept aimed to adjust these mechanisms appeared: “Many diseases - One therapy”, and the ozone fits perfectly to this pattern. \u0000The mentioned diseases and their complications have a tremendous emotional and social impact on the patients and their families and a negative economic effect on society. If ozone therapy is correctly applied – proper diagnosis, low/medium doses, standard and safe procedures, complementary to the specific treatment/diet of undergone conditions – it will act on numerous diseases, improving the clinical status, stimulating the healing process, practically without side effects. Despite the O3 clinical efficacy, its potential toxicity as a strong oxidant is a cause for concern. This occurs when the doctor does not correctly evaluate the oxidant status of the patient, while he is not aware of the disease etiopathogenesis and/or when he applies inadvertent ozone operational procedures. Systemically administered ozone in the low concentration range (10 to 40 ?g/ml) in the form of autohemotherapy (MAH) or rectal insufflation (RI) works synergistically inducing bioregulation. \u0000Chronic subclinical inflammations often have their origin in teeth and jawbone structures.Toxins, like the products of bacterial metabolism, may cause inflammations. Ozozated water has the potential to degrade toxins and to disinfect the surgical site from local infections. Additionally there must be taken mechanical cleansing efforts, to remove biofilms and toxins in all fields of dentistry. It is associated with widespread pain in the muscles and bones, areas of tenderness, and general fatigue. \u0000Symptoms like these are considered subjective, meaning they cannot be determined or measured by tests. Because its symptoms are subjective and there is no clear known cause, fibromyalgia is often misdiagnosed. Antinuclear Antibodies (ANA’s) directed against a variety of nuclear antigens have been detected in the serum of patients with many rheumatic and non rheumatic diseases. The lack of reproducible, objective tests for this disorder plays a role in some doctors questioning the disorder all together.
