Wenn von «komplizierten Mehrlingen» die Rede ist, dann sind v. a. die monochorialen Schwangerschaften gemeint. Die adaptiven Prozesse der Plazenta welche – nicht wie ursprunglich geplant fur die Bedurfnisse eines Kindes aufkommen muss – plotzlich zwei Feten versorgen muss, sind komplex und noch wenig verstanden. Eine ungleiche Verteilung der Plazentamasse und/oder eine unausgewogene Angio-architektur zwischen den Feten konnen zu charakteristischen Komplikationen fuhren welche unbehandelt mit einer hohen Morbiditat und Mortalitat assoziiert sind. Die moderne Betreuung von monochorialen Mehrlingen fokussiert auf das zeitnahe Erkennung von Hinweisen abnormer intrafetaler Interaktionen. Zentral dabei ist ein spezialisiertes pathophysiologisches Verstandnis um diese Prozesse sonographisch erfassen und interpretieren zu konnen. The revision of the VZV (Verkehrszulassungsverordnung) included in this program will come in to force on 1.July 2016. On that account the legal medical requirements for driver will be renewed. In particular, the requirements for vision (visual acuity, visual field) will be adjusted to international standards. Due to demographic changes the number of elderly drivers with old age (85 – 90+) with eye associated diseases increases. Therefore, questions concerning traffic ophthalmological problems have to be increasingly considered within traffic medical assessments. The driver's vision in traffic's safety must enable him to perceive relevant information, process information quickly and perform an adequate reaction in time, even if visibility is limited (e. g. due to rain, night, darkness) or in the presence of physical or psychical constraints.","PeriodicalId":137209,"journal":{"name":"Therapeutische Umschau. Haufigste Ausloser sind Medikamente, Nahrungsmittel und Insektenstiche. Neben Schocklagerung und Sauerstoffgabe besteht die arztliche Erstversorgung aus Adrenalin intramuskular und Volumensubstitution. Erst anschliesend sind H1-, H2-Blocker und Kortikosteroide intravenos zu verabreichen. Nach diesen Masnahmen ist der Patient zu hospitalisieren und intensiv zu uberwachen. Ebenso wichtig sind anschliesend die folgenden praventiven Masnahmen: Abgabe von Notfallmedikamenten, Anmeldung zur allergologischen Abklarung, Instruktion zur Allergenvermeidung, Angeben von Alternativpraparaten, Ausstellen des Allergiepasses. The costs for this treatment are enormous. Guidelines for managing the patients are changing for the last 20 years. Haematoma rates of 10% have been shown with CT scans in prospective studies for patients with GCS 14/15 and normal neurological examination. One out of ten of these patients had to undergo craniotomy. CT scans have shown to reduce costs if done on all patients with mild head injury and discharged with normal findings. Patients with skull fractures and age over 65 years are at higher risk, but not patients with loss of consciousness and post traumatic amnesia. We suggest CT scans on all patients. If a CT scan is not available we recommend to observe the patient for 24 hours in the hospital. If a patient with GCS 15 is to be discharged, an information leaflet with instructions for surveillance at home should be given to the patients and to the care taker.","PeriodicalId":137209,"journal":{"name":"Therapeutische Umschau.
