Discussion This child has cleidocranial dysplasia (CCD). Is an autosomal dominant disease caused by an heterozygous loss-of-function mutation in the RUNX2 gene, encoding transcription factor CBFA1, on chromosome 6p21, which plays an important role in osteogenesis and differentiation of osteoblasts. The main clinical features include persistently open skull sutures, hypoplasia or aplasia of the clavicles permitting abnormal facility in opposing the shoulders, short middle phalanx of the fifth fingers, dental anomalies (supernumerary teeth, failure of exfoliation of the primary dentition and malocclusion), pes planus, upper respiratory complications, recurrent otitis media, hearing loss and often vertebral malformation (scoliosis). Conclusion The clinical diagnosis of CCD was made during pregnancy, through the detection of aplasia of the clavicles in fetu, in association with the mother having short stature (1.32 meters), persistence of primary teeth and open anterior fontanelle. With 10 years of age, he underwent mendilioma study which revealed the mutation in the RUNX2 gene (c. 747 del (p.(Arg251 Alafs*7))) classified as a pathogenic variant.","PeriodicalId":13694,"journal":{"name":"International Journal of Medical Reviews and Case Reports","volume":"34 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2023-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"87154628","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

One of the latest variants, namely the Omicron variant has proved to possess novel characteristics including a shift in its entry pathway. Studies have shown that the Omicron variant has adapted to enter host tissue more efficiently through endosomal pathway rather than cell fusion as shown in its predecessors. As far as coronaviruses are concerned, there are several modes of entry for endosomal pathway including the caveolae formation pathway which is mediated by proteins called caveolins. One of the caveolins in particular is found abundantly in cardiomyocytes, thus posing as a higher risk for the Omicron variant to directly invade the heart tissue.","PeriodicalId":13694,"journal":{"name":"International Journal of Medical Reviews and Case Reports","volume":"50 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2023-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"81117077","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Blood cell composition testing is an important monitoring technique in the typical diagnostic examination. Objectives: The present study has been designed to evaluate Ionizing radiation's impact on the hematological parameters of occupational exposure in a cardiac catheterization lab at Azadi teaching hospital in Duhok city of Iraq. Methods: The study involved 40 hospital employees working in medical radiation centers who have been exposed to lower doses of ionizing radiation during Radiotherapy or diagnostic and the 32 unexposed workers as control. The correlation between the total dose and the hematological indices of exposed workers is examined using a bivariate regression analysis program. Results: The results looked to be significantly different with (p<0.05) compared to the respective controls in all blood parameters. Nevertheless, there is no significant difference (p>0.05) in Mean corpuscular volume, Mean Corpuscular Hemoglobin, Mean Corpuscular Hemoglobin Concentration, White blood cells, and Lymphocytes values obtained as compared with the control. Conclusions: The study found that almost some hematological parameters are affected by radiation. Further research should incorporate a variety of other independent factors such as (chromosomal aberrations and genetic polymorphisms) to study further low-dose radiation's long-term effects on personnel who were exposed to it.","PeriodicalId":13694,"journal":{"name":"International Journal of Medical Reviews and Case Reports","volume":"32 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2023-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"81335012","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

They represent an out of tolerance window zones and can negatively affect receiving, processing, and integration of stimuli. They are debilitating clusters of symptoms that reduce quality of life and significantly interfere with the individual’s daily functioning. Moreover, chronic significant hyperarousal- hypoarousal shifts can interfere with patient’s engagement in psychotherapy. Addressing these out of tolerance window states may reduce patient’s distress and improve his/ her quality of life and psychotherapy outcome. Case presentation This report describes a clinical case of psychotherapy- resistant PTSD key symptoms in a 32-year-old female patient. Stand-alone psychotherapy in the form of Eye Movement Desensitization and Reprocessing (EMDR) failed to lessen the PTSD symptoms. Persistence of PTSD hypo- and hyper-arousal symptoms significantly interfered with the patient’s daily functions and limited the progress and effectiveness psychotherapy. Given the nature and severity of her illness, the patient had been reassessed. Re-assessment included a clinical interview, blood tests, vital signs, ECG, anthropometric characteristics, assessment for attention deficit hyperactivity disorder (ADHD) and borderline personality disorder (BPD). Diagnosis of comorbid ADHD has been confirmed and the patient has been commenced on methylphenidate (MPH) with adjunct propranolol for associated paroxysmal sinus tachycardia. The effect and side effects of the treatment were monitored during a follow-up period of 7 weeks. Main parameters assessed were flashbacks, nightmares, dissociation and fainting, and hypervigilance with paranoid symptoms. Within one week of commencing the treatment the patient reported improvement in all psychotherapy- resistant PTSD symptoms. Remission of symptoms remained until the last follow up review (7 weeks post-treatment). Conclusion PTSD- related hypoarousal and hyperarousal symptoms reduce patient’s quality of life, impair his/her daily functioning, and could hinder the effective progress of psychotherapy, a corner stone in the treatment of PTSD. Pre-psychotherapy diagnosis and treatment of PTSD comorbidities such as ADHD may decrease PTSD symptoms severity. MPH seems to play a clinically meaningful role in the treatment of PTSD-related hyperarousal- hypoarousal symptoms and thereby reduces patient distress and improves patient’s quality of life and may facilitate successful psychotherapy outcome.","PeriodicalId":13694,"journal":{"name":"International Journal of Medical Reviews and Case Reports","volume":"41 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2023-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"82618051","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

This variant is rarely considered when investigating DVT, especially in patients with other risk factors. MTS requires careful workup and management to avoid complications such as chronic venous insufficiency, post-thrombotic syndrome, and primary lymphedema. The authors present a 63-year-old woman admitted to the emergency department with a 24-hour history of left lower limb pain and edema. Initial assessment, with computed tomography of the lower left limb and pelvis, showed compression of the left common iliac vein with an extensive thrombus of the left common iliac and external iliac veins, as well as femoral and popliteal left veins, consistent with MTS. Therapeutic enoxaparin initially implemented, was then switched to oral anticoagulation. At 6 months reevaluation, venous duplex ultrasound showed only partial recanalization of the femoral vein. Investigation for May-Thurner Syndrome in patients with left lower limb venous thrombotic events, regardless of risk factors, should always be present since the establishment of an effective therapeutic plan and eventually, endovascular treatment can minimize the long-term sequelae of deep vein thrombosis.","PeriodicalId":13694,"journal":{"name":"International Journal of Medical Reviews and Case Reports","volume":"72 4","pages":""},"PeriodicalIF":0.0,"publicationDate":"2023-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"72634039","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The objective of this study was to assess the effectiveness and long-term results of pyeloplasty in children with poorly functioning kidneys. Method: Patients who underwent pyeloplasty at Department of Urology, Rajendra Institute of Medical Science, Ranchi for last twelve years were the subject of a retrospective data study. Postoperative outcome was the parameter under study. Success was measured by either symptom improvement or better drainage on postoperative Tc-99m DTPA renography, which was performed after two months and then yearly after that. Results: 60 individuals were enrolled in the study, with a mean age of 8.5 years (3 months to 12 years), a male-to-female ratio of 2:1, and poor function on the isotope renogram (31%). The ratio of left to right sides was 1.2:1. All patients had dismembered pyeloplasty procedures. 37 patients (66.8%) underwent laparoscopic pyeloplasty, whereas 23 (33.2%) underwent open surgery. 35 patients had follow-up renograms available; of these, 30 showed improved drainage and no further decline in renal function, while 5 displayed an obstructed pattern; of these, 3 patients had noticeably worsened split function, and the other two underwent a second open pyeloplasty. A mean follow-up of 21.5 months revealed a 91% overall success rate. Conclusion: In the pediatric age group, pyeloplasty provides satisfactory intermediate results in kidneys with inadequate function.","PeriodicalId":13694,"journal":{"name":"International Journal of Medical Reviews and Case Reports","volume":"7 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2023-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"75958522","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
