Case Summary: A 72-year-old female patient was brought to the psychiatry outpatient unit with symptoms such as sleeplessness, irrelevant talking, irritability, weakness of memory, lack of personal hygiene, and wandering away from home. The case was diagnosed as unspecified dementia, assessed with mini-mental state examination (MMSE), and treated with Ignatia 200. MMSE score of 10 (severe cognitive impairment) at baseline gradually improved to 24 (no cognitive impairment) within 6 months and was maintained up to 12 months. Clinical improvement was also observed in cognitive functions, behaviour as well as the general condition of the patient. Causal attribution to changes after homeopathic intervention is evaluated through Modified Naranjo Criteria for homoeopathy. It is also called musculoskeletal disease which is portrayed by pain and a gradual diminishing in the range of motion and function, in one or more areas of the musculoskeletal structure. In a few specific diseases, there are indications of inflammation, swelling, redness and warmth in the influenced zones like in osteoarthritis (OA), rheumatoid arthritis, etc. Cases Summary: Four cases of Osteoarthritis were treated with Gettysburg water (Get.) as a potentised homoeopathic medicine. Each case was followed up with clinical observation and reported according to the criteria of the HOM-CASE guidelines. Assessment of homoeopathic treatment and its effect was carried out using the visual analogue scale (VAS) during every visit. Improvement in all cases was evident from the reduction of pain in the affected joints and relief in other associated symptoms within 2–6 months of treatment. VAS scores for the cases ranged from 8 to 4 in all cases. Complaints associated with chronic OA were relieved by potentised Get. This case series has shown promising outcomes and a further high-quality clinical trial is needed for the establishment of effectiveness data for the said medicine. Abstract OA is a chronic degenerative disease, which progresses because of multifactorial causation presented by the loss of articular cartilage, hypertrophy of bone at the edges, subchondral bone sclerosis, and scope of biochemical and morphological alterations of the synovial membrane and joint capsule clinically. The condition is portrayed by joint pain, tenderness, limitation of movement, crepitus, and occasional effusion along with variable degrees of local inflammation. [1,2] Although OA can involve any joint containing hyaline cartilage, troublesome symptoms most often occur in the weight-bearing joints of the lower extremities. Fibrocystic disease of the breast is the most common and benign lesion among women of reproductive age group seeking medical advice for breast problems during a lifetime. It is characterised by hyperproliferation of connective tissue or fibrosis, which is followed by facultative epithelial proliferation in the breast tissues. Case Summary: A case of a left endometriotic ovarian cyst with multiple cystic lesions in the right breast in a 36-year-old patient successfully treated with individualised homoeopathic medicine is reported. She had severe cyclic lower abdominal pain for more than 7 years, and conventional analgesics and even surgical procedures could not afford relief. Homoeopathic treatment was started with Silicea based on the totality of symptoms and repertorisation and later switched to Conium maculatum as an anti-miasmatic remedy, which showed disappearance of the left endometriotic cyst and reduction in the size of cystic lesion in the right breast with general improvement. The Modified Naranjo Criteria were used to assess causal attribution to the prescribed medicine in this case. Assessment of pain was done using a visual analogue scale. This evidence-based case reported according to the HOM-CASE guidelines suggested that individualised homoeopathic medicines can be used as a useful, safe and non-invasive mode of treatment for ovarian endometriotic cyst, associated with fibrocystic disease of the breast. Abstract Endometriosis is an estrogen-dependent chronic inflammatory condition of women seen in the reproductive period, identified by the presence of endometrial glandular epithelium and stroma outside the uterine cavity. [1] The risk of endometriosis seems to increase in women aged 25–35 years. [2] It affects 20% of infertile women and 15% of women with chronic pelvic pain. [3] Ultrasonograph y is a readily available, user-dependent and inexpensive tool for the diagnosis of endometriotic lesions. [4] Management depends on location, size and extent of the lesion, age of the patient, desire for fertility and results of previous therapy. Conventional therapy comprises medical, surgical and a combination of both depending on the severity and response of medical treatment. [5] Symptoms of endometriosis are alleviated in the conventional medicine system by hormonal medications and gonadotropin-releasing hormone agonists or antagonists, whereas surgical treatment mostly amounts to laparoscopy, be it diagnostic or therapeutic. Ovarian reserve, follicular density and fecundity rate may be affected by surgical excision of the cyst. Infertility may result from any underlying pathology or unexplained causes and can cause severe emotional disturbances in both partners. The complexity and cost of conventional treatment may not be affordable for a majority of people. Case Summary: Three cases of infertility with an underlying pathology successfully treated with standalone homoeopathic treatment are reported. These cases presented with a structural deformity as a cause of infertility. The patients partners were also given homoeopathic medicines in all the cases. The first case showed a long liquefaction time on semen analysis and the female partner had a unilateral tubal block. The second case investigation reported ipsilateral varicocele and small-sized testes with oligospermia. In the third case, the female had polycystic ovarian syndrome with a sub-septate uterus and multinodular goitre. All three cases were treated with individualised homoeopathic medicine. All these cases were followed up regularly and they conceived within 6 months of treatment. [2] The overall prevalence of primary infertility in women of reproductive-age group is 8.9% in the urban population of Central India. Their presence, irrespective of size, is considered an indication of surgical removal. Case Summary: A 44-year-old hypertensive and diabetic male presented with severe pain in the left lower back for 1 month. Abdominal USG revealed the presence of a 4-mm size gallbladder polyp, mild hydronephrosis of the left kidney, two left renal microliths, 5.2-mm calculus in the left vesicoureteric junction, Grade-I fatty liver and Grade-I prostatomegaly. The patient was advised for cholecystectomy, but he opted for homoeopathic treatment to avoid surgery. At the first visit, homoeopathic medicine Calcarea fluorica was prescribed and the patient was followed up periodically for 1 year. His back pain reduced within 15 days. The patient was assessed for subjective improvement through the ‘outcome related to impact on daily living’ scale. USG after 10 months of treatment revealed that the gallbladder polyp was not present and his liver, kidney and ureteric functions returned to normal as per investigations. Modified Naranjo Criteria for Homoeopathy score (+8 on the ‘−6–+13’ scale) assesses if the improvement of the patient can be attributed to the homoeopathic treatment provided. This case report is presented to document the scope of individualized homoeopathy in the treatment of gallbladder polyp with multiple comorbidities pathologies. It can affect both genders, all ages and races. Solar lentigines are especially prevalent in fair-skinned adults. Sunburn may be one of the causes of lentigo. Modern medicine treats these conditions by either melanin-specific high-energy lasers or external applications. Case Summary: A 56-year-old female visited outpatient department with a complaint of lentigo on the right cheek for the past 15 years. The patient was treated with Sepia officinalis in increasing potencies (200, 1M and 10M). The treatment got completed over 9 months with significant improvement and complete disappearance of lentigo. The cases were documented with proper photographs at onset and during treatment. It can be suggested from this case study that high dilutions of homoeopathic medicines are useful for patients with lentigo. Abstract Lentigo is small, sharply, circumscribed, macular, pigmented and mucocutaneous lesions which measure 1–5 mm, but not larger than 1 cm, surrounded by normal skin. Histologically, hyperplasia of the epidermis may be observed as well as increased pigmentation of the basal layer and a variable number of melanocytes. [1] “Black solar lentigo” is a melanotic macula, that may evolve slowly over several years or they may be eruptive, appearing rather suddenly. [2,3] commonly found in fair-skinned individuals on sun-exposed areas of the body. Clinically, the term “ink spot Lentigo” is used and it is a dark pigmented spot [4] and the colour ranges from brown to brown and black to black and the pigmentation may be homogeneous or variegated. However, the case reported here is typically an ink spot lentigo which is a unique type that appears among several other common solar lentigines. [5] Post-inflammatory hyperpigmentation, solar lentigos and melasma come under the category of hyperpigmentation and occur widely in the human population and are thus of broad interest for control. Pigmentation of the skin is primarily related to melanocyte functionality, but the surrounding keratinocytes and extracellular matrix proteins and fibroblasts in the underlying dermal compartment also actively contribute to cutaneous homeostasis. [6] There are potentially hundreds of proteins and other effectors involved in pigmentation based on a genomic and proteomic understanding of the melanocyte and melanogenesis. Potdar","doi":"10.53945/2320-7094.1063","DOIUrl":"https://doi.org/10.53945/2320-7094.1063","url":null,"abstract":"Abstract IntroductionIntroduction: Idiopathic Granulomatous Mastitis (IGM) is a rare, debilitating, chronic inflammatory disease of the breast, occurring in women of the child-bearing age, which can clinically, and radiologically mimic abscess, tubercular infection or breast cancer. Homoeopathy can treat the disease by addressing its multifactorial origin, given its holistic approach. The paper presents a case series of 11 patients of IGM treated with classical homoeopathy in place of conventional methods. Case Summary Case Summary: After exclusion of differential diagnosis of inflammatory breast lesions by radiology, and biopsy, the patients were given individualised homoeopathic treatment. Common symptoms were pain, single or multiple lumps, abscesses, sinuses, ulcers and discharge in various patients. Descriptive statistics, clinical observation and patient’s feedback were used for analysis. 11patients were followed up for a median period of 24 months. All 11 patients experienced subsidence of the lumps with no recurrence and general improvement in health, without any conventional medication or surgical intervention. The initial experience of resolution of IGM with homoeopathic treatment is encouraging. For an evidence-based evaluation of the results, larger numbers of case studies are required. Homoeopathic approach of treating the disease based on symptom totality is a relevant alternative to often unsatisfactory conventional medicine in cases of LA. Case Summary: A 35-yearold male who presented with LA was treated with homoeopathic medicine based on the totality of symptoms and significant improvement was seen with resolution of abscess in the liver along with relief in other presenting symptoms. ObjectivesObjectives: Primary objective was to evaluate change in shoulder pain after individualised homoeopathy treatment for over 2 months. Secondary objective was to assess the change in the QoL and outcome related to impact on daily living (ORIDL). MethodsMethods: A single-arm, pretest-posttest, clinical study on AC was conducted on 40 participants recruited from the outpatient clinic of rheumatological disorders at Clinical Research Unit (Homoeopathy), Siliguri, West Bengal, India. Medicines were prescribed on the basis of the totality of symptoms. Changes in shoulder pain over 2 months were evaluated using the shoulder pain and disability index (SPADI). QoL was evaluated using SF-12v2 and ORIDL (participants and physician assessed), respectively. ResultsResults: Thirty-six participants completed the study and four participants dropped out. A protocol compliant sample of n = 36 was analysed. There was a statistically significant reduction of SPADI score (91.92 ± 10.22 vs. 34.14 ± 24.43; mean reduction 57.78, 95% CI 49.41–66.14, P < 0.001) and statistically significant increase in SF-12 v2 score (44.39 ± 9.70 vs. 72.27 ± 10.97; mean increase 27.87, 95% CI 23.89–31.85, P < 0.001). The Spearman’s correlation between the changes in physician assessment ORIDL scores and participants assessment ORIDL scores over 2 months suggested a statistically significant correlation (rs = 0.998, P < 0.01). ConclusionConclusion: The findings showed symptom alleviation, and improvement in the QoL after homoeopathic treatment. Randomised controlled trials are further warranted.
