While the infection appears to be mild in most patients, considering its high rate of transmission, a large number of people are at risk of developing severe to critical illness in total which makes prognosis studies a priority.The aim of the present study was to evaluate red blood cell distribution width (RDW) as a predictive factor for diagnosing severe cases of coronavirus disease 2019 (COVID-19).Materials and Methods: A total number of 204 inpatients diagnosed with COVID-19 including 122 men and 82 women (Mean age: 58.83±15.93 years old) treated at Imam Reza Hospital, Mashhad, Iran were included in the study. Patients were divided into severe and moderate groups according to their clinical signs and examinations and pulmonary imaging features. Demographic Data, laboratory test results, treatments, patients’ complications and outcome were recorded. Mann-Whitney U test and spearman correlation coefficient (r) were performed to assess RDW correlation with severity and serious complications in patients including intensive care unit (ICU) admission, shock, secondary infections, intubation, length of hospitalization and death. Receiver operating characteristics (ROC) curves analysis was carried out to define the reliability of RDW as a predictive indicator in severe COVID-19.Results: The results showed statistical significant correlations between high levels of RDW and developing secondary infections and longer hospitalization (P values ≤0.001). The optimal cutoff for RDW to predict the length of hospitalization (≤ 7 days or more than 7 days) was estimated to be 14.65% with 94% sensitivity and 71.3% specificity. The area under curve was calculated to be 0.895 through Roc curve analysis.Conclusion: High predictive value of RDW, a routine blood test parameter, could be used in diagnosing COVID-19 patients at higher risk for developing secondary infections and longer hospital stay which in turn helps with better management of the disease.","PeriodicalId":131413,"journal":{"name":"journal of cardio-thoracic medicine","volume":"38 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2021-03-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"127109528","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Needs long-term care and etc. Activation of the deep inflammatory response dependent on the blood response and the cardiopulmonary pump circuit is one possible reason for this. Therefore, in this study, two anti-inflammatory drugs, dexamethasone and methylprednisolone, were used in Prime solution in cardiac surgery patients and then the occurrence of delirium was evaluated Methods and Materials: In this clinical trial study with a sample volume size 43 patients in open-heart surgery by on-pump method, after random assignment to two groups including dexamethasone and methylprednisolone that were added to prime solution. Inclusion criteria were EF more than 30%, candidate for mitral valve surgery, absence of cognitive impairment and preoperative delirium and age range 30-65 years. In the first group with 21 patients received dexamethasone at a dose of 0.1 mg/ kg and in the second group with 22 patients at a dose of 0.4 mg/Kg in prime solution. Then delirium examined with standard CAM-ICU instruments after extubation and then the results were analyzed with SPSS 21 version. Results: The results showed that only 18% of patients who received methylprednisolone primer solution and 28% of patients who received dexamethasone primer solution had delirium, and this difference was non-significant (P. value: 0.448). Conclusion: In this study, the anti-inflammatory drug methylprednisolone were not superior to each other and had a similar effect on delirium","PeriodicalId":131413,"journal":{"name":"journal of cardio-thoracic medicine","volume":"9 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2021-03-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"130192870","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

We present an 11-year-old male patient, with a syncope event and normal ECG in the first medical assessment. Eight months later, presented a new syncope event, but now with 1st degree block that evolved to complete AV block in 24 hrs in ECG. In Tertiary Medical Care Hospital showed sinus arrhythmia with nodal rescues. The echocardiographic assessment showed dilatation and global hypokinesia in the Right Ventricle.  The MR showed in apex and lateral wall late Gadolinium enhancement indicating fibrosis greater than 20%. The electrophysiologic assessment showed a low-voltage zone in the RVOT, normal stimulation between sinus and AV nodes. Was implanted ICD which identified events of ventricular tachycardia (185 bpm), giving discharges between 21 to 41 Jules, which were not perceived for the patient. Despite the well-known pulmonary manifestations of COPD, the extrapulmonary complications are mainly demonstrated in clinical studies, and the exact mechanisms behind the relationship between COPD and some extrapulmonary manifestations are still unclear. Many case-control studies reported osteoporosis as a common complication of COPD. However, the possible causality between COPD and osteoporosis is not clearly understood, and a lack of follow-up studies on COPD patients is an important issue addressed in the literature. In the present review, we discussed the common molecular pathways in COPD and osteoporosis and summarized the follow-up studies on COPD patients evaluating the possible changes in bone mass density or the development of fractures in COPD patients.","PeriodicalId":131413,"journal":{"name":"journal of cardio-thoracic medicine","volume":"60 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2020-12-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"133208952","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Restless Legs Syndrome is associated with many systemic diseases. RLS is found in many patients with COPD. There is paucity of data on this topic from India.Aim: The aim is to find the prevalence of RLS in COPD patients and its distribution in study population with respect to various demographic and clinical characteristics. Correlation between severity of RLS and COPD severity was also aimed to be evaluated.Setting and design:Prospective, observational study conducted in a tertiary care institute over a period of 12 months. Materials and methods:After fulfilling the inclusion criteria the patients were categorised according to the severity of COPD using the GOLD guidelines. Diagnosis and severity of RLS was evaluated using IRLSSG criteria. Prevalence and association of RLS with COPD and its demographic and clinical characteristics was evaluated. Results:A total of 294 patients were taken which included 229(77.9%) males and 65 females (22.1%).Mean age was 62.05 ± 10.32 years. Smoking was a more common risk factor among the patients (85.7%). 110 patients were diagnosed with RLS. A statistical significant association was found between duration of COPD and RLS (p value =0.001) and also between severity of COPD and RLS (p value=0.001). A significant positive correlation (r=0.395, p value= 0.001) between severity of COPD and RLS. Forty two (87.5%) patients with definite RLS had stage D COPD. Degree of obstruction of airways showed a statistical significant association with prevalence of RLS (p value=0.001). Conclusion: RLS showed increased prevalence in COPD patients making it crucial to screen all the COPD patients for symptoms of RLS and if required to treat the symptoms, thus improving the quality of life.","PeriodicalId":131413,"journal":{"name":"journal of cardio-thoracic medicine","volume":"149 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2020-10-21","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"125227770","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
