Abdou","doi":"10.21608/ha.2019.12639.1044","DOIUrl":"https://doi.org/10.21608/ha.2019.12639.1044","url":null,"abstract":"Background: Female genital mutilation/cutting (FGM/C) is still a wide spread practice in Egypt in spite of its criminalization in 2008.  Objective: This cross sectional study was carried out to assess female sexuality in a large cohort of Egyptian women exposed to FGM/C compared to those who were not exposed to it.  Patients and Methods: A large sample of married Egyptian women participated in this study. They were divided into 2 equal groups (1000 women each). Group A contained women who were not exposed to FGM/C while group B contained women exposed to FGM/C. The tool used was a self-report questionnaire, designed by the authors, and anonymously filled by the participants. The questionnaire involved questions that mainly covered demographic data and sexual activity of the participating women.  Results: By comparing all age groups, our results reflect a retreat in FGM/C practice. A significant difference was observed in all sexual domains in favor of women not exposed to FGM/C including spontaneous desire, lubrication, orgasm and overall satisfaction with sexual life. In addition, dyspareunia was more common in genitally cut women.  Conclusion: Although FGM/C is on the retreat, still it has negative consequences on female sexuality. Since infertility causes personal, familial, and social problems, it considered a devastating health problem. Infertile males experience considerable psychological stress, with low self esteem, isolation, loss of control, sexual inadequacy and depression.Objective: To assess impact of male infertility on self-esteem and determine relationship between clinical, demographic characteristics and self-esteem of infertile males.Patients and Methods: A non experimental (case-control) research design used to conduct this study. A purposive sample was used. The current study included 150 subjects, 100 was studied group & 50 was control group. Its effect on different body systems is well known. Sexual dysfunctions (SD) in men and women may be related to DM. However, SD is more difficult to diagnose and treat in women than in men because of the intricacy of the female sexual response. Also, the literature is limited addressing female sexual dysfunction in DM, and this aspect of female health, is often ignored in clinical practice in women with DM.Objective: : To determine the frequency and types of SD and its causative factors among the diabetic women.Patients and methods: This cross sectional study used a self-report questionnaire taken from the FSFI with addition of questions suiting the purpose of the study. Participants of this study were 407 married women with regular sexual activity who attended the outpatient clinics at the National Institute of Diabetes Mellitus in Cairo during the period from September to December 2018.Results: The commonest age group of women was 40-49 years, most of them were obese and most of them can only read and write. Most participants (42%) suffered from diabetes for more than 10 years and 70.7% were of type II diabetes. Diabetic complications included peripheral neuropathy, hypertension and coronary artery disease. At least one SD was present in 66.3% of participants including dyspareunia followed by sexual dissatisfaction followed by low libido. Duration and control of diabetes, BMI, age and diabetic complications were determining factors for the occurrence of SD.Conclusion: SD are common in diabetic women. Also, acceptability of both partners to this method will be assessed.  Materials and Methods: A total of 200 married women who experienced male condom use by their husbands were enrolled in this work. The tool used in this study was a self-report questionnaire written in English and translated into Arabic to ensure it is understood by all subjects regardless of their level of education.  Results: With condom use, desire level prior to sexual relationship was moderate in 38% of participants. Half of the respondents had moderate degree of lubrication while only 36% kept lubrication during the whole coitus on using condom. Condom had a negative effect on orgasm frequency in 28% of respondents. Most participants thought that their sexual life would be better without condom use.Conclusion: A majority of women disliked male condoms. It is characterized by prolonged penile erection for more than 4h not related to sexual excitation. It is classified into three main types: venoocclusive (ischemic), arterial (nonischemic) and stuttering priapism. Many causes can predispose to ischemic priapism including thalassemias, sickle cell anemia, hypercoagulable status, neoplastic syndromes, pelviabdominal tumors and use of some drugs and medications. However, in the majority of cases of ischaemic priapism no cause could be identified. Chronic myeloid leukemia (CML) is a chronic myeloprolifertative tumor that is characterized by a variety of vague clinical manifestations. Priapism as a complication of CML is a rare clinical presentation. It may cause priapism as a result of hypercoagulability due to hyperleukocytosis. Here, in this report, the case of a known CML male patient who presented with venoocclusive priapism for about 16 h after abrupt discontinuation of his cytoreductive therapy is described. Hyaluronic acid (HA) is a natural and safe compound that has been widely used not only in the aesthetic medicine clinic but also for the treatment of osteoarthritis.  Objective: To show the effectiveness of a HA-based procedure for treatment of PE.  Patients and methods: A total of 20 male patients were treated with HA injections in the deep dermis of their glans penis to increase the volume and the circumference of their penis to prevent male PE and improve the patients’ and their partners’ sexual satisfaction.  Results: At 6-month follow-up, intravaginal latency time was still significantly higher when compared with baseline values. The maximal glandular circumference was significantly increased at 6-month follow-up. At baseline, patients’ self-rated satisfaction with sexual intercourse was 1.5±0.07, and partners’ self-rated satisfaction with sexual intercourse was 1.48±0.06. At 6-month follow-up, patients’ self-rated satisfaction with sexual intercourse was 6.2±0.08, and partners’ self-rated satisfaction with sexual intercourse was 5.5±0.07. Mostafa","doi":"10.21608/ha.2019.12272.1043","DOIUrl":"https://doi.org/10.21608/ha.2019.12272.1043","url":null,"abstract":"Introduction: Sexual satisfaction is a multidimensional experience involving thoughts, feelings, personal and socio-cultural attitudes and beliefs, combined with biological factors.  Aim of the Study: The present study aimed to assess the degree of sexual satisfaction and its predictors in a convenience sample of married Egyptian women.  Materials and Methods: 302 married women were recruited for this study. The tool used was a self-report questionnaire designed by the authors with the help of the COSECON survey. The questionnaire included questions about demographic data, sexual satisfaction, sexual activities e.g. frequency of coitus, obtaining lubrication and orgasm.  Results: The degree of sexual satisfaction was as follows: 57.9% highly satisfied, 22.2%%moderately satisfied and 19.9% dissatisfied. The main predictors of satisfaction were: age, coital frequency, spontaneous desire, lubrication and the ability to reach orgasm. Foreplay duration and method, duration of marriage, residence and educational level were all related to sexual satisfaction.Conclusion: Sexual medicine counselors may benefit from the present work in dealing with their female clients. There are several reasons for this, which include a lack of general clinical awareness, the nonspecificity of its symptoms, biochemical tests that are not always easy to interpret, concerns over the safety of testosterone replacement therapy especially in older men, and the false perception that testosterone is a sex hormone that has no other specific health benefits. Tiredness is a common symptom of hypogonadism, which can be profound, but there are many medical practitioners who do not include the assessment of testosterone levels in the clinical workup of this symptom. Many etiological factors are involved, among which urogenital bacterial infections play an important role.Materials and Methods:Semen analysis, bacteriological culture, and sensitivity analyses were carried out to investigate the effect of genitourinary infections on semen parameters of infertile men.Results: Staphylococcus aureus was the most common isolated pathogen (46.2%) followed by urogenic gram-negative pathogens (24.1%). The isolated microorganisms are highly sensitive to piperacillin/tazobactam, imipenem, meropenem, gentamicin, doxycycline, amikacin, and nitrofurantoin. These antibiotics could be used empirically while awaiting the results of semen culture.Conclusion: Semen culture is an important diagnostic tool in all patients undergoing fertility investigations to detect genitourinary infections, pyospermia, and bacteriospermia. Moreover, early treatment should be considered according to the results of culture whenever possible. The former has the advantage of being watertight thereby avoiding hematoma formation, and the disadvantages are the risk of puncturing the implant cylinders and the process is time consuming.Aim: The technique herein provides watertight closure of the corporotomies with short operative time as an alternative.Patients and Methods: Before insertion of the implant cylinders, a continuous suture line is preplaced in the form of an inverted U around the corporotomies on both corpora, using Vicryl 20/ suture material. Following insertion of the implant, the ends of the suture lines are pulled tight and tied as a purse string. The technique was applied in 18 cases of three-piece inflatable implants.Results:Overall, the purse-string technique closed the corporotomies successfully in 18100%( 18/) cases of the three-piece inflatable penile prostheses.
