Its pathophysiology is uncertain, but hypothalamic dysfunction is hypothesized. It is usually a primary headache, but it can have secondary causes. \u0000Objectives: We aimed was to review articles published in the last 34 years on hypnic headache and analyze secondary cases. \u0000Methods: Based on a literature search in the major medical databases and using the descriptor “hypnic headache” we included articles published between 1988 and 2020. Of the 359 patients found, only 18 met the inclusion criteria and were analyzed. \u0000Results: We found 18 patients (6 men and 12 women) with secondary hypnic headache. The mean age of patients was 58.7±15.0 years, ranging from 20 to 84 years. Dessa forma, a maioria da população acaba excluída do debate científico. Nesse contexto, faz-se importante a popularização da ciência como uma forma de democratizar o conhecimento e minimizar a disseminação de desinformação. Esse processo, no entanto, requer estratégias de comunicação para estabelecer um diálogo eficiente entre doutos e leigos. Tendo isso em vista, o papel das Universidades é essencial e exigiu, ao longo do tempo, adaptações nos modelos de ensino dentro delas, incluindo o ensino médico. Dentre essas adaptações, destaca-se a ascendente importância da extensão, por meio da qual os estudantes estabelecem um vínculo entre o meio científico e a comunidade. Com base nesses princípios, o Núcleo de Estudos em Cefaleias e Algias Cranianas do Oeste do Paraná (NECEFAC) foi criado no curso de medicina da Universidade Federal do Paraná - campus Toledo. The main symptoms are headache, vision disturbances, and pulsatile tinnitus. Definitive diagnosis requires the presence of clinical and/or radiological signs of intracranial hypertension, high opening pressure on lumbar puncture (LP), and normal CSF constitution. Several studies have evaluated new contributions of CSF in the clinical evaluation and the in understanding of the pathophysiology of pseudotumor cerebri. Such studies have included the analysis of inflammatory biomarkers, adipokines, proteomic analysis, and CSF flow studies. In this review, we present the main results obtained so far and critically discuss the present status and the potential role of research involving the CSF in this condition. Based on current knowledge, it is possible to conclude that CSF research with new biomarkers has not yet provided information that can be employed in clinical practice at this moment. É multifatorial, sendo a insônia um fator de predisposição presente em quase 50% dos casos. A fisiopatologia da migrânea é complexa e envolve alterações em diversas áreas do SNC e que encontram vias comuns com os distúrbios do sono. Entre elas, conexões hipotalâmicas, hormônios e neuropeptídeos, como orexias e monoaminérgicos. Além disso, outros distúrbios do sono específicos se relacionam com a migrânea. Atualmente, o tratamento é individualizado e além de medidas medicamentosas, é proposto mudanças nos hábitos de vida e adequações do sono. Siega","doi":"10.48208/headachemed.2022.26","DOIUrl":"https://doi.org/10.48208/headachemed.2022.26","url":null,"abstract":"O uso de derivados da Cannabis sp. no tratamento de certas condições médicas, como dor neuropática, epilepsia e esclerose múltipla já é bem conhecido. Porém, estudos sobre seu benefício em cefaleias ainda são de baixa abrangência, sendo constituídos, em sua maior parte, por relatos e séries de casos. Apresentamos o caso de paciente masculino de 74 anos, portador de enxaqueca de alta frequência (2 a 3 vezes/semana) há vários anos, com importante limitação funcional associada a diversos gatilhos, fazendo uso apenas de triptanos durante as crises. Apresentou intolerância a 2 medicamentos profiláticos (topiramato e propranolol). Realizou 2 aplicações de Erenumabe 70 mg, havendo considerável melhora inicial seguida de novo aumento na frequência das crises após a segunda aplicação, com interrupção do tratamento. Fez uso de nutracêuticos por 6 meses, sem grande melhora. Após introdução de óleo de canabidiol, as crises reduziram em frequência (1 episódio/mês) e intensidade, com intervalo recorde de 30 dias seguidos sem dor, associado a eliminação de gatilhos. Estudos pré-clínicos avaliando a relação entre o sistema endocanabinoide e a fisiopatologia da migrânea apontam para os derivados da Cannabis sp. como potenciais armas para o tratamento dessa condição. Thus, in addition to these conditions, neuropathic pain secondary to cases of novel coronavirus infection have been seen in the literature and have raised questions about the pathophysiology arising from this situation, prognostic factors, and best indications for treatment. Among the neuropathic pain reported, occipital neuralgia is one of the complications faced, characterized by a burning and paroxysmal pain of variable duration. Many are admitted withdiminished conscience status (coma) and focal neurological deficits. In the evaluation ofthese patients, neuroimaging is indispensable in order to identify the type of lesion andthe location of the brain where it is located.In the case of stroke, we can subdivide it into hemorrhagic and ischemic. Among hemorrhagic hemorrhages, we can mention (1) spontaneous intracerebral hematomasand (2) hemorrhages due to rupture of an intracranial aneurysm, with subarachnoidhemorrhage leading.Patients with head trauma are critical; even those who arrive at the hospital alert andoriented can decrease their level of consciousness in a few hours due to an intracranialhematoma, edema, or cerebral contusion.Thus, the availability of performing neuroimaging evaluations, using computed tomography and magnetic resonance imaging, or even digital angiography, is vital for continuoussupervision of this type of patient. The exams often require repetition several times due tothe rate of evolution of vascular lesions and after head trauma.A warning sign in these types of patients is headache. In the intracranial aneurysmal rupture, we classically have the thunderclap headache, an explosive, sudden pain mentionedas the worst pain the individual has suffered in his or her life. The pericranium and someintracranial structures are sensitive to nociceptive stimuli, such as the dura mater, largearteries, and venous sinuses. The brain is relatively insensitive to painful stimuli.This narrative review aims to inform the importance of neuroimaging assessment of patients with stroke and traumatic brain injury in an emergency department.
