To develop a biomechanical model of the knee joint, including a detailed representation of the patellofemoral segment for the normal anatomy of bones, joints, ligaments and muscles, and study patellar movement during passive knee flexion.Materials and methods. The architecture of the biomechanical model was developed using an open source software system for biomechanical modeling OpenSim. Patellofemoral joint with 6 degrees of freedom, patellar stabilizers – medial patellofemoral ligament (MPFL), medial patellotibial ligament (MPTL), lateral retinaculum (LR), and patellar contact surfaces (facets) were included in the model. Gmsh and Paraview were used to generate the contact surfaces. Simulations of knee passive flexion with consistent patellar stabilizers exclusion were carried out to identify their influence on patellar movement.Results. The presented biomechanical model provides a detailed analysis of the normal dynamics of the patella and the role of different anatomical structures in its functioning and can be used for further experiments investigating of the patellar movement. The experiment involving all ligaments is consistent with the physiological norm. Disabling MPTL has minimal effects on patellar tilt and translation, which aligns with its small size. In contrast, deactivating MPFL results in increased lateral tilt and translation of the patella. Additionally, deactivation of LR components 1 and 2 induces more medial tilt and translation. Deactivating LR components 3 and 4 leads to further lateral translation and slight additional medial tilt.Conclusion. Computational results show that all ligaments contribute to the normal movement of the patella. To evaluate changes in the concentration of molecules that mark the neurodegenerative process, experimental spinal cord injuries (SCI) of various origins were studied.Materials and methods. SCI was modeled in six-month-old male Wistar rats by exposing the T10 vertebra to: carbon dioxide under a pressure of 2 N/cm2 (pneumocontusion); free-falling load of three weights of 1.12 N/cm2, 1.68 N/cm2, 1.96 N/cm2 (contusion injury); compression with forceps (compression injury); partial hemisection of the spinal cord; mechanical laminectomy using a mechanical drill. There were 6 rats in each group, including the intact control group. On the 28th day after a single application of SCI in rats, the concentrations of tumor necrosis factor-alpha (TNF-α), interleukin 6 (IL-6), albumin, thiobarbituric acid reactive substances (TBA-RS) and superoxide dismutase activity were assessed in the blood serum.Results. When modeling SCI of various origins in rats, the serum concentration of TNF-α increased (from 115.5% (p < 0.05) in mild contusion to 234.5% (p < 0.05) in compression trauma compared to intact control) as well as IL-6 (from 49.2% (p < 0.05) in mechanical laminectomy to 89.8% (p < 0.05) in hemisection compared with intact control), suggesting activation of inflammatory reactions. The concentration of albumin in the blood serum of rats with SCI was lower than that of intact animals, especially in the hemisection group – by 41.9% (p < 0.05). Animals with SCI had an increase in TBA-RS concentration ranging from 103.2% (p < 0.05) in mild contusion and compression to 135.5% (p < 0.05) in pneumocontusion, and a decrease in superoxide dismutase activity ranging from 26.3% (p < 0.05) in laminectomy to 31.7% (p < 0.05) in hemisection. At the same time, injuries caused by spinal compression and hemisection led to a more pronounced activation of the inflammatory process, as evidenced by the increased TNF-α content compared to other variants of SCI modeling.Conclusion. At the molecular, cellular and tissue levels, neuroinflammation serves as a sanogenetic response to a variety of injuries, including post-traumatic conditions and neurodegeneration. However, inflammatory changes in long-lived cells such as neurons inevitably trigger a range of adverse effects. As a result, prognosis often depends on the severity of neuroinflammation. In this work, we review the spectrum of biomarkers involved in two key mechanisms of neuroinflammation: immune-mediated and mechanical injury, that have a potential clinical application: cytokines, neurospecific proteins and their proteolysis products, markers of oxidative stress, matrix metalloproteinases and endocrine parameters. Over the past decades, a significant pool of data on neuroinflammation biomarkers has been accumulated and continues to grow, but no consensus has been reached on the actual gradation of their clinical significance. Their complete cure remains elusive, and existing therapies are often accompanied by side effects. Recent studies have shown a signifi cant role of disturbances in the composition of the microbiome in the development of autoimmune reactions. Moreover, modulation of the microbiome through various therapeutic interventions represents a promising direction in the framework of complex therapy of the underlying disease. Extracellular vesicles, in particular exosomes, transport biologically active substances between cells, and a number of studies have shown their therapeutic effect in autoimmune diseases. However, the role of extracellular vesicles in modulating the microbiome remains poorly understood, and further research is needed to better understand their impact on the pathogenesis of autoimmune diseases and associated microbiome changes, as well as to develop new treatment strategies. The presented literature review, based on a study of English-language sources, examines the importance of the microbiota of different loci of the human body (intestines, skin, oral cavity) in the development of autoimmune diseases such as multiple sclerosis, psoriasis and Sjögren’s disease. Only 46 cases of intracranial localization of primary EHE in adults have been published.Case report: A 47-year-old male patient developed pronounced speech disorder of sensory and amnestic aphasia type, right-sided pyramidal hemiparesis within 3–4 days. Magnetic resonance imaging revealed a delimited left sided brain islet neoplasm with signs of hemorrhage and small perifocal edema. Intraoperatively, the neoplasm was represented by a conglomerate of pathologic vessels, with involvement of the terminal branches of the M2 segment of the middle cerebral artery, which prevented radical resection of the neoplasm. Based on histopathological and immunohistochemistry analysis of the neoplasm (positive expression of CD31, CD34, proliferative activity index Ki-67 10%) the morphologic diagnosis of EHE was established. 3 months after surgery, continued growth of the residual part of the neoplasm was noted. Temozolomide chemotherapy was clinically ineffective. The patient died due to the development of dislocation syndrome after 9 months.Discussion. The malignant characteristics of EHE include invasive growth, recurrence, and metastasis, which is more common in intracranial localization of the neoplasm. To investigate the ST and bone health relationship between athletes of different training intensities and nonathletes of different ages.Materials and methods. A search of databases PubMed, Embase, and Pedro was conducted from January 2009 to August 2023. The full texts of all potentially relevant studies were obtained and evaluated by three independent reviewers for inclusion.Results. The comprehensive review of eight studies indicates a positive influence of ST on bone health. Sprinters show higher cortical and trabecular BMD in the tibia than controls, with a noted age-related decline in BMD. Short distance runners demonstrate significantly better BMD, counter-movement jump performance, and grip strength compared to long-distance runners. These benefits are consistent across various age groups, including older athletes, with minimal age-related changes in mid-tibial BMD. ST is also associated with a 21% increase in tibial stress-strain index, indicating sustained bone strength, and a reduction in fracture risk in the elderly through downregulation of fracture-related microRNAs.Conclusion. ST significantly enhances bone health, particularly in improving BMD and bone microarchitecture. Incorporating ST into exercise routines may benefit athletes and older individuals. To evaluate the long-term outcomes of IRP with melphalan under conditions of hyperthermia for treatment of locally advanced STS of the extremities.Materials and methods. We conducted a prospective cohort clinical study that included 42 patients with STS of the extremities. All patients underwent IRP with melphalan, in one case – in combination with tumor necrosis factor alpha. Additional lymph node dissection was performed in 8 cases (19%). The 5-year overall survival was estimated by the Kaplan-Meier method.Results. The median age of the patients was 50 years (from 24 to 80 years), women predominated – 32 (76%). STS of the upper limb was diagnosed in 14 patients (33%), of the lower limb in 28 (67%). G3 grade was verified in 34 (81%) patients, in the rest – G2. Most patients had a recurrent tumor – in 34 (81%) cases. All patients received previous treatment: surgery in combination with chemotherapy and / or radiation therapy – 36 (86%), chemotherapy alone – 2 (5%), surgery alone – 4 (9%) patients. Within 5 years, tumor recurrence developed in 20 (48%) patients, amputation was performed in 10 (24%) patients, distant metastases were diagnosed in 4 (10%) patients. The mean time to relapse after IPC was 9.3 ± 3.5 months. Overall survival at 1, 2, 3, 4, and 5 years was 98%, 88%, 86%, 74%, and 67%, respectively.Conclusion. To study the effectiveness of modified subtotal laryngectomy with the formation of a tracheopharyngeal shunt in patients with locally advanced cancer of the larynx and laryngopharynx.Materials and methods. We conducted a cohort study on the use of combined treatment (surgical and radiation/ polychemotherapy) in 25 patients: 24 men, 1 woman; median age – 56 years. The subtotal laryngectomy technique that we developed and used included the preservation of one arytenoid cartilage, the posterior third of the vocal fold, and the recurrent nerve on the side not affected by the tumor. We studied postoperative complications, the length and diameter of the tracheopharyngeal shunt, the functioning of the arytenoid cartilage, the frequency of voice recovery and its quality, and overall and relapse-free survival.Results. In the postoperative period, 8 (32%) patients developed a pharyngocutaneous fistula; tracheal stenosis after tracheostomy was observed in 8 (32%) cases. The length of the tracheopharyngeal shunt varied from 16.4 to 25.6 mm, in all cases its course was tortuous, the diameter of the preserved part of the vocal fold varied from 1.1 to 1.5 mm. 24 (96%) patients demonstrated the functioning of the arytenoid cartilage and its work as a valve. Voice function recovered in 24 (96%) patients. Fundamental frequency was 155.3 ± 20.9 Hz, maximum phonation time of the vowel “A” – 3.5 ± 1.0 s, duration of pauses in phrase – 0.24 ± 0.03 s, speech rate – 99.8 ± 8.4 words/ min, number of words in phrase 6.5 ± 0.3. The average value of the Acoustic Voice Quality Index was 6.74, which indicates dysphonia (for the Russian language, the threshold value for dysphonia is 4.86), but this did not interfere with patients’ communication at home and on the phone. Overall and relapse-free survival at 1, 2, and 3 years were 100%, 89%, 78% and 86%, 78%, 72%, respectively.Conclusion. However, risk factors for an adverse outcome of such operations have not yet been identified.Clinical cases. A 65-year-old man with primary chondrosarcoma of the V rib, underwent surgical resection of three ribs, plasty with local tissues, and an individual 3D printed titanium implant was installed on the sternum and IV-VI ribs. Follow up for 26 months showed no complaints or signs of recurrence. A 52-year-old woman with radiation-induced soft tissue sarcoma of the chest wall, that developed 9 years after radiation for breast cancer, underwent resection of four ribs and pectoral muscles and a 3D printed titanium implant was installed on the sternum and II-V ribs. The operation was complicated by the marginal necrosis of the soft tissue flap and infection of the endoprosthesis, which required removal of the metal structure and reconstruction using TRAM flap. After 9 months, a local recurrence of the tumor was diagnosed. Discussion. As a factor of a positive outcome in a man should be noted a thick layer of subcutaneous fat with muscle tissue, due to which plastic surgery was performed, covering the defect without tension. An extensive resection of the pectoral muscles in a woman created a tissue deficit. Another factor of an unfavorable outcome can be considered the radio-induced nature of the sarcoma. Type II high grade tumors are more common, accompanied by ascites, and are the main cause of cancer-related death in women. OC associated ascites is considered as valuable source of tumor material containing a wide range of dissolved components and cell populations. Over the past decades, the cellular and acellular components of ascites have been studied, but its effect on chemoresistance and the development of metastasis continues to be studied. This review describes the pathogenesis of ascites in OC, it’s cellular and acellular components, many of which are prognostic factors as well as markers of the effectiveness of anticancer therapy.
