It is a rare condition. We report the case of an old woman with dyspnea and abdominal pain, which investigation was consistent with pseudo-Meigs’ Syndrome, however definitive diagnosis was not able to be determined. A palliative treatment was decided. We discuss the investigation of pleural effusion and ascites, the management of pseudo-Meigs’ Syndrome as palliative options when surgery is not an option.","PeriodicalId":12806,"journal":{"name":"Gazzetta Medica Italiana Archivio per le Scienze Mediche","volume":"2013 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-09-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"134995394","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

After several episodes of chest / epigastric pain that led her to the emergency room, she start treatment with dual antiplatelet therapy because of what was assumed has a myocardial infarction without ST elevation. Despite the hemodynamic stability and slight increase in cardiac enzymes, due to recurrent symptoms, hipotension and electrocardiographic dynamic changes, she was transferred for an emergent catheterization. Before procedure, transthoracic echocardiogram revealed the presence of a large thrombus in the right ventricle so angio- CT was performed. It confirmed not only a clot but contrasting leakeaged too, allowing inferolateral free wall rupture diagnosis. The patient underwent emergent surgical treatment, that took place without any complications. From the subsequent etiologic study, it stood out an angiography without coronary disease but the finding of an inferolateral infarct scar in the magnetic resonance imaging that suggested previous ischemic event.","PeriodicalId":12806,"journal":{"name":"Gazzetta Medica Italiana Archivio per le Scienze Mediche","volume":"5 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-09-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"134995231","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

It suppresses the excessive or dysfunctional activities of immunity and prevents autoimmune diseases. In animal models vitamin D increases ACE2 on the cell surface, so its protection may not apply to COVID-19 since SARS-CoV-2 enters human cells by binding ACE2. On the other hand, ACE2 might make COVID-19 less severe, as it protects against lung damage. Observational and ecological studies are controversial regarding the association vit D levels - COVID-19 severity. Contradictory are the results of the way it influences the rate and duration of hospitalizations, ICU admissions and mortality.","PeriodicalId":12806,"journal":{"name":"Gazzetta Medica Italiana Archivio per le Scienze Mediche","volume":"27 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-09-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"134995385","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Different forms of pneumothorax can be distinguished: spontaneous, post-traumatic, iatrogenic (associated with invasive maneuvers). Spontaneous pneumothorax is distinguished in: primary (in the absence of a pulmonary pathology), secondary (with a pulmonary pathology known as cystic fibrosis, chronic obstructive pulmonary disease, pulmonary tuberculosis) and catamenial. The latter affects the female sex and it is associated with the menstrual cycle. Catamenial pneumothorax is part of a more complex syndrome: thoracic endometriotic syndrome. It affects women between 30 and 40 years, the right lung is most involved and it occurs within 24-72 hours after the onset of menstruation. The pathogenesis is that of endometriosis, i.e. there is a dissemination of endometrial tissue outside the uterine cavity. In catamenial pneumothorax, endometriotic tissue is located at the level of the pleurae, the lung or the diaphragm. The most accepted theory is that endometrial tissue follows the circulation of peritoneal fluid. This theory explains why the endometrial lesions located at the diaphragmatic level concentrate mainly in the right hemi-diaphragm and from there they reach the pleura. From the clinical point of view the symptoms of the various forms of pneumothorax are: dyspnea, cough, chest pain, tachycardia, tachypnea and subcutaneous emphysema. The diagnosis is based on the physical examination, on the execution of an arterial blood gas that shows hypoxemia and on instrumental investigations. Today the ultrasound is important, already performed by the emergency doctor, which highlights the typical signs of the pathology (absence of gliding and identification of the lung point). The other instrumental tests are chest X-ray and CT scan of the chest. There are mild or severe forms of pneumothorax, in the former the treatment is based on clinical observation and evaluation of the evolution of pneumothorax; in severe forms the treatment is the placement of a drainage. The treatment of the various forms of pneumothorax is represented by: pleurodesis, sub-total pleurectomy and talcage which offer the best results in terms of prevention of recurrences. In addition, in the catamenial form the patient can undergo hormonal therapy. In this clinical case we describe a case of catamenial pneumothorax, which affects a 37-year-old woman who previously had a spontaneous pneumothorax. It is important when diagnosing a pneumothorax in the female sex to think of the catamenial form, which may not be a rare occurrence.","PeriodicalId":12806,"journal":{"name":"Gazzetta Medica Italiana Archivio per le Scienze Mediche","volume":"39 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-09-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"134995389","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The objectives of our study were first to isolate the brain regions in RRMS patients and networks disrupted in non-fatigued and fatigue subgroups and correlate rsfMRI with neuropsychology scores using seed based and graph theory.METHODS: Resting fMRI analysis was done on a total of 10 RRMS patients and 10 healthy subjects and based on MFIS were subdivided as fatigued and non-fatigued. We then correlated the SDMT and MFIS scores using seed based and graph measures of rsfMRI data.RESULTS: Seed-based techniques showed abnormalities in the DMN, other cognitive networks, increased networks in left brain in non-fatigued and migration to right brain in fatigued as part of neuro plasticity. Areas of hypoconnectivity were extensive and indicated more severity of disease. Compensatory areas of hyperconnectivity in the association areas, cerebellum and striatal hubs were noted. Graph analysis showed tendency of brain networks towards lack of integration and over segregation of networks. Clustering coefficient was reduced and, randomness, path length, inter hemispheric connections was increased in RRMS. Frontal lobe and thalamus showed a significant decrease in rich club network with a weak but increased interhemispheric connectivity in non-fatigued and intrahemispheric connectivity in fatigued groups respectively.CONCLUSIONS: In neuroeconomic terms, the alteration of these connectivity and graph metrics indicate increased metabolic demand between nodes to carry out the normal functions.","PeriodicalId":12806,"journal":{"name":"Gazzetta Medica Italiana Archivio per le Scienze Mediche","volume":"88 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-09-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"134995391","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Previous studies identified only knee position at initial contact and peak flexion during landing as risk factors for ACL injury. Establishing further assessments to detect deficits in knee neuromuscular control is useful for more effective determination of ACL injury risk in female athletes. The purpose of this study was to examine gender differences in basketball players regarding knee valgus angle and dynamic knee wobbling.METHODS: Ten participants were recruited from intercollegiate basketball teams. Two 2D-videocameras were used to capture the participants performing 3 trials of controlled single-leg squat. An incident of knee wobbling was defined as when the valgus angular velocity crossed zero. Frontal plane projection angle (FPPA), valgus excursion range, and frontal angular velocity divided by sagittal angular velocity (F/S Ratio) during wobbling were compared between genders.RESULTS: The valgus excursion angle was significantly larger in females than males (0.8° vs. 0.1°, P=0.011), and the F/S Ratio was significantly larger in females than males (212.1% vs. 16.7%, P=0.011). There was no difference in the FPPA or the frequency of knee wobbling between males and females.CONCLUSIONS: Greater knee excursion angle and knee wobbling during a single-leg squat test were observed in female basketball players. To determine the risk of ACL injury in female athletes during screening, knee wobbling and F/S Ratio during the eccentric phase of single-leg squats can be more useful than static angular assessments.","PeriodicalId":12806,"journal":{"name":"Gazzetta Medica Italiana Archivio per le Scienze Mediche","volume":"27 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-09-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"134995396","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Optimal length of latissimus dorsi and teres major muscles is prerequisite to allow full elevation of scapula and lateral rotation of humerus. Repetitive overhead throwing with the shortened muscles would cause undue stress in shoulder joint to accomplish the task.METHODS: Seventy-five overhead asymptomatic athletes from different sports were included. Player was in crook lying and smartphone clinometer was used to measure shoulder flexion. To measure latissimus dorsi length Pressure biofeedback was placed at the lower back to monitor pelvic tilt and medial rotation of the humerus was observed during flexion. For teres major length, scapular abduction, and medial rotation of the humerus was observed. The measurement was done on the non-throwing arm as well.RESULTS: Shoulder flexion on the throwing arm was 143.7° (IQR-141.7°, 149°, P value <0.001) and on non-throwing arm was 158° (IQR-154.3°,161.7°, P value <0.001) for latissimus dorsi. Meanwhile, flexion on throwing arm was 144.7° (IQR-143.3°, 146.7°, P value <0.001) and on non-throwing arm was 151.3° (IQR-149.7°, 153.3°, P value <0.001) for teres major.CONCLUSIONS: The present study has concluded that there was a significant reduction in throwing arm shoulder flexion due to shortened latissimus dorsi and teres major muscles in asymptomatic overhead throwers.","PeriodicalId":12806,"journal":{"name":"Gazzetta Medica Italiana Archivio per le Scienze Mediche","volume":"27 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-09-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"134995232","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
