The goal is to develop systems that could support early discharge of cardiac patients and enable their home based recuperation. Such systems could act as a virtual extension of the ICU/CCU systems in the hospital, enabling physicians to monitor patients at home, and giving them the ability to discharge patients earlier while continuing to monitor their recuperation progress. This concept could result in direct health care cost savings and improvement in the quality of medical care, especially in rural areas with limited access to cardiologists. Our first step towards this goal is a system that can send cardiac data from the patient to the physician via the Internet. The physician can perform data analysis and send reports back to the patient over the Internet. The demand for wireless technologies has been absolutely exploding. In the 1980s, many analog cellular networks were implemented. These networks are already reaching capacity limits in some service areas. Several digital technologies were introduced to increase spectral efficiency and enhance wireless communications by adding features and services such as facsimile and data transmission and new call handling features. In other words, wireless communication technology has evolved from first generation analog systems for business applications to second generation digital systems with rich features and services for residential and business applications. As the turn of the century approaches, a new vision of ubiquitous personal communication systems starts to emerge. For the new century, many new wireless communication services and capabilities using the wideband CDMA technology are being defined by the industry worldwide. Specifically, the specifications for the future global wireless access system, the International Mobile Telecommunications-2000 (IMT-2000) are currently being developed within the International Telecommunication Union (ITU). With the higher data transmission speed and the new services such as simultaneous voice and data, multimedia and location services, applications of the new wireless technologies to the biomedical field become practical in the near future. Among the different existing algorithms, Kohonen's self organizing feature map (SOFM) is one of the well known methods for VQ. It allows efficient codebook design with interesting topological properties. Furthermore, use of VQ for compression gives, in the same process, basic information on the image content. But in order to preserve the diagnostic accuracy in medical applications, the block size is restricted to small values (3/spl times/3, 4/spl times/4), which limits the compression rate. We propose to improve the compression performance by using several codebooks containing codewords of different size, according to the quadtree decomposition of the images. Results are compared to those provided by the standard JPEG image compression algorithm. Finally we explain how it is possible to generate characteristic signature maps of images using compression information. The paper represents an extension of the work presented by G. Cauguel et al. The set-up consists of the supercomputer and the graphical workstation connected through an ATM data link. The main advantages of the described system, i.e. the combination of the huge computing power of the supercomputer and the convenience of control and display enabled by a graphical workstation are aimed at efficacy of development of medical image processing procedures. It is illustrated by a pilot registration of pictures captured by a confocal laser microscope. Thomas, 1991). A digital imaging network that eliminates the use of X-ray film makes military medicine requirements simpler. X-ray film requirements include storage of new, unexposed films, storage and use of chemicals and water for processing, and disposal of chemicals. In some deployed situations, the chemical discharge needs to be collected and shipped out of the area. Therefore, the ability to implement electronic imaging, and eliminate or greatly reduce the dependence on film, chemicals, and water are intrinsically important to military medicine. In December 1995, the US government began deployment of 20000 US troops to Bosnia-Herzegovina as a part of NATO's peace keeping implementation force (IFOR) operation. A full complement of military medical support facilities was established in Bosnia. An army base in Hungary was the location from which the deployment was staged. The project to deploy telemedicine and teleradiology capabilities to the medical treatment facilities (MTF) in Bosnia and Hungary became known as PrimeTime III. NIH participates in the national Computing, Information, and Communications Research and Development (CIC R&D) program by applying these new technologies to problems in biomedicine. We describe a high speed networking example, the Radiology Consultation WorkStation, and a high end computing example, positron emission tomographic (PET) image reconstruction. The NIH Cancer Genome Anatomy Project (CGAP) provides an example of a major biomedical research effort that will require the creative use of information technology in the future. Because of the increase of telemedicine in the last years, telediagnosis is becoming more and more frequent. There are no studies for telediagnosis in burns. We are testing a PC platform for that application, using a digital photographic camera for data acquisition, the standard JPEG for image compression and the telephone network as communication media. Ambulance personnel, who usually are the first to handle emergency situations, do not have the required theoretical knowledge and experience. Financial and practical reasons do not allow the participation of specialized physicians on ambulance vehicles. Within the framework of the health telematics programme, we have developed a portable medical device that allows telediagnosis, long distance support and teleconsultation of mobile health care providers by specialized physicians. The device allows the transmission of vital biosignals and still images of the patient from the incident place to the hospital. The transmission is performed through the GSM mobile telephony network. The device can telematically \"bring\" a specialist doctor to the site of the medical emergency, allow him to evaluate patient data and issue directions to the emergency personnel on treatment procedures until the patient is brought to hospital. Due to the need for storing and archiving data being interchanged during the telemedicine sessions, we have equipped the consultation site with a multimedia database able to store and manage the data collected by the \"AMBULANCE\" system. In both cases, the source schemata are generally assumed to be static a reasonable assumption because of the massive undertaking required to perform a schema evolution. Since mapping a source to the warehouse is considered to be a rare event, little effort is made to reduce the work involved.
