{"title":"Lowering the legal alcohol limits for driving: expected changes in the prevalence of positive tests","authors":"Manuela Alcañiz, Montserrat Guillen, Miguel Santolino","doi":"10.1016/j.gaceta.2025.102463","DOIUrl":"10.1016/j.gaceta.2025.102463","url":null,"abstract":"<div><h3>Objective</h3><div>To estimate the increase in the prevalence rate of drivers testing positive for alcohol when lowering the breath alcohol content (BrAC) limit from 0.25 to 0.10<!--> <!-->mg/l.</div></div><div><h3>Method</h3><div>A cross-sectional study using a sample of 8320 breath tests from a mandatory roadside random survey conducted on interurban roads in Catalonia in 2019.</div></div><div><h3>Results</h3><div>The percentage of drivers with a BrAC<!--> <!-->>0.10<!--> <!-->mg/L was 3.8% (95%CI: 3.2-4.4), more than twice the percentage with a BrAC<!--> <!-->>0.25<!--> <!-->mg/L (1.6%, 95%CI: 1.2-2.0). The main study variables were having experienced sexual violence and reporting unhealthy behaviours or health problems (low mood, daily tobacco consumption and alcohol and cannabis risky consumption). Prevalence rates were estimated for all variables, and Poisson regression models with robust variance were used to analyse potential associations with independent variables.</div></div><div><h3>Results</h3><div>The prevalence of experienced sexual violence was 30%, with differences between boys (14.1%; 95%CI: 12.8-15.4) and girls (43.3%; 95%CI: 41.6-45.0). Having a minority sexual orientation, early onset of sexual relations, being in a higher academic course, having experienced and exercised bullying, and poor relationship with the parents were associated with experienced sexual violence. Experiencing sexual violence was associated with low mood, daily tobacco consumption and risky alcohol and cannabis consumption for both sexes.</div></div><div><h3>Conclusions</h3><div>Experiencing sexual violence in adolescence is prevalent, particularly among girls and sexual minorities, and is associated with mental health issues and substance use. The study highlights the need for gender-sensitive healthcare policies that address mental health and social support systems. The investigation revealed environmental exposure in a mobile recreational installation that was neither registered nor subjected to health inspections. Active case finding was conducted through public health surveillance networks and interregional collaboration. Subsequently, <em>Legionella</em> was detected in reused components at other cities along the tour route. The absence of prior inspections and health declarations for such events limited the ability to anticipate and respond to the outbreak. This experience highlights the lack of specific regulation for temporary activities with aerosolization risks and suggests their inclusion in regional registries of high-risk installations. This tool is based on an interactive platform that allows the visualisation of 257 indicators in six areas. It also incorporates new functionalities for the interpretation of the results and data download. These indicators are shown from a dual perspective of analysis: territorial (according to the person's residence) and health care centre (according to the place where the care has been provided); also including their evolution over time (from 2017 to 2023). The data are visualised through tables, graphs and maps. They are accompanied by fact sheets and reports that summarise and contextualise the results, and also provide navigation guides and a search engine for indicators. Cities, due to their physical and social environments, are particularly vulnerable to the effects of health crises. However, there are no specific preparedness and response plans for health crises in cities. Four years after the onset of the pandemic, it is crucial to identify the achievements and areas for improvement in Barcelona. For this, we used four elements: 1) crisis recognition, 2) crisis management, 3) communication and policy, and 4) prevention of future crises. Although the crisis was acknowledged, there was also a lack of preparedness to effectively address and respond to it, primarily due to the historical underfunding of public health resources. However, throughout the three years of the pandemic, notable efforts have been made in key aspects of management: the development of new technological platforms for information systems and the improvement of intersectoral coordination. New communication formats have also been developed for both internal and external channels. These elements are essential tools that require improvement to face future pandemics.
