The three-dimensional (3D) imaging during laparoscopic procedures can improve the quality of that surgeries. There is a shortage of publications about the potential benefits of 3D navigation in laparoscopic surgery with urological diseases. Radical prostatectomy (RPE) is known as the gold standard of treatment of localized prostate cancer (PC), and investigation of imaging technologies in laparoscopic surgery in PC patients is a hot topic. Aim. To compare the perioperative outcomes of laparoscopic RPE performed with 3D and two-dimensional (2D) imaging. Materials and methods. We performed retrospective analysis of perioperative outcomes in 146 patients who had undergone radical surgery with localized PC. All the patients were divided into 4 groups by the surgery features: 1) 2D imaging with the technique for neurovascular bundles preservation (TNVBP) (n=52); 2) 2D without TNVBP (n=46); 3) 3D with TNVBP (n=23); 4) 3D without TNVBP (n=25). We assessed operative time, intraoperative blood loss volume (IBLV), duration of the bladder drainage, positive surgical margin (PSM) detection rate, duration of the postoperative inpatient period, urinary continence recovery rate, erectile function recovery (EFR) rate. Results and discussion. In groups 1, 2, 3, 4 the operative time was 171,4±21,1, 168,3±23,2, 98,7±17,3, 92,2±22,2 min, and the IBLV was 294,2±62,1, 281,2±53,2, 144,2±31,7, 148,5±33,0 mL, respectively. PSM detection rate was 1,92±0,11%, 2,17±0,04% in groups 1, 2, while PSM had not been detected in groups 3, 4. In all the participants, duration of the bladder drainage was 5–7 days, and the full recovery of urinary continence was detected at both 6 and 12 months after the surgery. The postoperative inpatient period was 8–10 days in groups 1, 2, and 8–9 days in groups 3, 4. The EFR at 3 months after the surgery was detected in 38,4%, 28,3%, 34.8%, 28.0% of patients, while at 12 months it was detected in 59,6%, 41,3%, 82,6%, 56,0% of patients in groups 1, 2, 3, 4, respectively. Conclusion. We revealed the following features of perioperative period of laparoscopic RPE performed with 3D imaging compared to 2D: 1) the operative time was reduced by 42–45% (р<0,05); 2) the IBLV was reduced by 47–51% (р<0,05); 3) the PSM had not been detected; 4) there was the tendency to the shorter postoperative inpatient period; 5) the EFR rate was increased by 1,3–1,4 times (р<0,05), and the best EFR outcomes were obtained via 3D imaging together with TNVBP. Currently, there is a renaissance of phenazopyridine in world urology. The aim of this work was to study the analgesic and antidysuric effects of phenazopyridine, as well as to evaluate the efficacy and tolerability of phenazopyridine in combination with fosfomycin for the treatment of acute uncomplicated cystitis in women of working age. Materials and methods. In 2020–2022 a multicenter randomized open study was conducted. This study involved 224 women aged 18 to 60 years with acute uncomplicated cystitis. They were randomized into two groups of 112 people each. In the first group, patients received oral phenazopyridine (Urinalgin F, earlier Fenazalgin) 200 mg 3 times a day for 2 days (total dose 1200 mg) and fosfomycin trometamol (Monural) at a dose of 3 g once. In the 2nd group, women received orally once 3 g of fosfomycin trometamol and drotaverine hydrochloride (No-spa) 80 mg 3 times a day for 2 days. The visual analogue scale (VAS) of pain, the assessment of symptoms of cystitis according to the ACSS scale, urinalysis by microscopy, and bacteriological examination of urine were used. The results were evaluated after 6, 12, 24 and 48 hours, 3 and 6 days. Results. In the 1st group, a strong uroanalgesic and antidysuric effect of phenazopyridine was revealed. According to VAS, the severity of pain decreased from the initial 7.3±0.5 points after 6 hours to 3.7±0.4 points, after 12 hours to 1.6 points, after 24 hours to 0.4 points, and after 48 hours the pain disappeared in all 112 patients. The sum of the characteristic symptoms of cystitis on the ACSS scale from the initial 12.2±0.5 points decreased to 2.1±0.2 points after 3 days, and to 0.27±0.04 points after 6 days. In patients of the 2nd group, for all indicators, a less pronounced symptomatic effect was obtained with relief of pain and dysuria. The combined treatment of cystitis with fosfomycin and phenazopyridine turned out to be more effective than the combination of fosfomycin and drotaverine hydrochloride: in the 1st group, 97.3% of patients recovered, bacteriological efficiency was 96.8%, leukocyturia disappeared at an earlier date, by 30% reduced treatment time. An undesirable effect of phenazopyridine (nausea) was detected in 1 (0.9%) patient. Сonclusion. Phenazopyridine has a pronounced uroanalgesic and antidysuric effect and is an effective and safe agent for the symptomatic treatment of patients with acute uncomplicated cystitis. Chronic kidney disease (CKD) is a worldwide problem with an increasing general and widespread incidence affecting people of all age groups and races. All stages of CKD are at risk of some somatic diseases, reduced quality of life and mortality. Material and methods. The study included 30 patients (16 men and 14 women) who underwent surgical treatment with intraoperative kidney biopsy to identify possible morphological predictors of CKD development in the Urology Department of the Krasnoyarsk Regional Clinical Hospital. Operations were performed laparoscopically for kidney cysts, stenosis of the pyelo-ureteral segment, and kidney neoplasms. All patients from the anamnesis data did not have glomerular lesions. The age of the patients ranged from 21 to 79 years. Directly obtained kidney tissues were performed using puncture processing of a 16G guillotine needle intraoperatively under visual control. Tissue columns were obtained, separated for light-optical microscopy, immunofluorescence reaction, and electron microscopy. Posting samples for morphological studies was carried out according to the standard method. Results. Based on the results of 30 nephrobiopsies obtained during laparoscopic operations on the kidney, latent glomerular lesions were diagnosed for the first time in 6.7% and moderate or severe manifestations of tubulo-interstitial nephritis in 33.3%. In addition, mesangial proliferation and tubular atrophy are reduced in 63.3% of biopsy specimens. Conclusion. Chronic prostatitis (СР) is an actual problem of modern urology as it leads to significant deterioration of life quality of able-bodied male population. Frequent relapses of the disease in spite of timely prescribed therapy leads to the necessity to search for alternative drugs. Objective of the study. To evaluate efficacy and safety of Adenoprosin® in patients with CP/chronic pelvic pain syndrome (CРРS) of IIIA category according to USA National Health Institute classification. Materials and methods. The prospective observational study of efficacy and safety of Adenoprosin® was carried out from 10.2021 till 10.2022 on the basis of the Center of Urology and Andrology of the State Research Center named after Burnazyan, Federal Medical and Biological Center of the Russian Federation. Men aged 35 to 60 with diagnosed CKD (IIIA according to the US National Institutes of Health classification) took part in the study. Patients were divided into 2 comparable groups. The control group was treated with an alfa1-adrenoblocker (Tamsulosin 0.4 mg 1 once daily), antibacterial agent (Levofloxacin 500 mg 1 once daily) for 30 days. In the main group, in addition to the standard therapy, Adenoprosin® 150 mg, one suppository a day was administered rectally for 30 days. The study requirements were included 4 Visits: Visit 0-1 – the baseline visit; Visit 2 on Day 14 from the therapy start; Visit 3 on Day 30 from the therapy start; Visit 4 on Day 90 from the therapy start. Clinical monitoring was performed with standardized questionnaires «International system of summarizing prostate disease symptoms» (IPSS), «Chronic prostatitis symptoms index (NIH-CPSI)», WBC counting in prostate secretion. Results. Statistical analysis of NIH-CPSI, IPSS questionnaires, as well as WBC count in the prostate secretion before and after the beginning of treatment, showed that the therapy was significantly effective in both groups (p <0,05). In the main group (Adenoprosin® group) the results were statistically better (p<0,05) if all questionnaires and WBC count in the prostate secretion were evaluated after 90 days from the therapy start as compared to the control group. Conclusions. Hypercalciuria is one of the main factors in the formation of the most common calcium-containing stones. Urolithiasis (UL) is more common in men than in women, which indicates the existence of gender lithogenic factors. In this work, the effect of varying degrees of calciuria in stone formers (men and women) on metabolic parameters and the frequency of detection of urinary stones of various chemical composition was investigated. Materials and methods. A total of 982 patients with UL (439 men and 543 women aged 18 to 79 years) were examined. To estimate lithogenic activity of calciuria in patients, the values of calcium excretion (mmol/day) were ranked in ascending order and divided into 10 equal parts (ten 10%-х percentilesей). In each of the calciuria ranges кальциурии, the percentage distribution of urinary stones types and biochemical blood and urine parameters were determined. Results. As the degree of calciuria increased from minimum to maximum values, the proportion of oxalate stones in men increased by 1.4-2 times, compared with women (χ2 test for trend p<0.02), and in women there was an increase in the proportion of carbonatapatite stones compared with men by 2-2.9 times (p<0.025, χ2 test). Increasing calciuria was accompanied by an increase in excretion, uric acid, phosphates and magnesium, which was more pronounced in men than in women (p<0.0001) and positively correlated in patients of both sexes with an increase in the frequency of detection of oxalate stones (p<0.05). The frequency of detection of carbonatapatite stones in men was directly dependent on calcium excretion (r=0.6783, p=0.0314). There was no such dependence in women, despite their high proportion of these stones, unlike men. Oxalate stones are more common in men in all age groups (20-70 years). In women, the greatest predominance of carbonatapatite stones is observed in the period from 20 to 40 years. Conclusion. Gender features of lithogenesis are clearly revealed at calciuria, which increases from minimum to maximum values. At the same time, the frequency of oxalate stones formation increases in men. Women have a high percentage of carbonatapatite stones, which, however, did not depend on the severity of calciuria. Symptoms of lower urinary tract dysfunction (LUTS) in older men are most often associated with benign prostatic hyperplasia (BPH). A low level of testosterone leads to various changes in the organs and systems of the body, including causing structural changes in the tissues of the genitourinary system, which negatively affects their function. The aim of this work was to study the effect of testosterone replacement therapy on LUTS with androgen deficiency in patients after surgical treatment of BPH. Materials and methods. The analysis of observation and treatment of LUTS in 101 patients operated on for BPH was performed. The patients are divided into three groups. Group A (n=48) included patients with normal testosterone levels receiving conservative therapy for moderate LUTS. Groups B (n=38) and C (n=21) included patients with SNIP and low testosterone levels. In order to correct the level of testosterone in patients in group C, after surgical treatment, androgen replacement therapy was prescribed. For primary diagnosis and evaluation of the effectiveness of the treatment vinitial and during periods of 1, 3, 6 and 12 months, general clinical examination methods, the International Pancreatic Symptom Scale (IPSS) questionnaire, urination diaries (DM), uroflowmetry, and ultrasound examination of the pancreas and bladder were used. Results. According to the IPSS questionnaire, patients in group B had more pronounced manifestations of LUTS compared with patients in groups A and C due to accumulation symptoms. At the same time, by prescribing androgen replacement therapy, it was possible to achieve an increase in the level of total testosterone in patients of group C to «physiological values», corresponding to patients in group A. According to the results of the analysis of DM data, it was found that in patients in all observation groups, the number of urination per day decreased (p<0.001) with an increase in the functional capacity of the bladder. The highest values of the maximum micturition volume, at the end of the observation period, were recorded in patients in group A, the minimum micturition volume and the most pronounced decrease in episodes of nocturia and nocturia index – in patients in group C. Temporal values of the accumulative bladder function also indicate a more pronounced decrease LUTS in group C. Conclusions. It is believed that the «target» of the hexane extract of Serenoa repens (HESR) is the prostate gland itself. However, there is evidence in the literature that the therapeutic effect of HESR can be carried out at the level of the bladder. We hypothesized about the direct effect of HESR on the obstructive bladder, preventing the development of its dysfunction. Material and methods. The study was conducted on 30 white mongrel female rats. Bladder outlet obstruction (BOO) was modeled by applying a ligature to the urethra on 18G catheter. The animals were divided into groups: experimental – HESR (n = 10); sham/placebo (n = 15) and control (n = 5). From the day following the operation, rats of the experimental series daily received aqueous alcohol suspension of HESR (27.6 mg/kg) intragastrically through a probe, and the sham group received the same volume of 3.5% alcohol solution. After 2 weeks, the survival rate of the animals, the functional state of the bladder, daily diuresis, biochemical changes in blood and urine characterizing the functional state of the kidneys were evaluated. Bladder function was assessed according to infusion cystometry. After examination, the animals were euthanized and the bladders was removed to determine its mass and histological examination. Results. Within 2 weeks in the sham group 10 (67%) rats died due to renal failure, while in the HESR group – only 3 (30%). An obvious overfilling of the bladder in the main and sham groups was revealed. Bladder volume was larger in the HESR group compared to the sham (p<0.05) one. The daily diuresis of the animals of the main group was also higher compared to the sham group, and the decrease in diuresis in the sham group was lower compared to the control (p=0.045). The functional state of the bladder was assessed by filling cystometry. In the sham group, an increase in intravesical pressure occurred immediately after the start of fluid infusion and its growth was accompanied by irregular fluctuations, which is a manifestation of secondary detrusor hyperactivity, while in the HESR group, a gradual increase in pressure without fluctuations up to large volumes of fluid was observed. The dynamics of the volume/pressure index in the main group was close to normal, which indicates better compliance of the wall of the hypertrophied bladder during HESR therapy. The use of HESR contributed to better preservation of kidney function. In terms of daily diuresis, glomerular filtration rate, tubular reabsorption of sodium, daily excretion of urea and creatinine in the urine, statistically significant differences were established between the main and sham groups. In the sham group, a more pronounced activity of creatine phosphokinase was noted compared to the HESR group (p<0.05), which indicates a significant damage to kidney and bladder cells (cytolysis) during BOO. To facilitate access under ultrasound (US) guidance, it is customary to use a needle guide which is fixed to the US-probe. Taking into account the sanctions of the equipment manufacturing countries, Russian specialists may potentially find themselves in conditions of a shortage of components and particularly needle guide for ultrasound probes. In this regard, it was decided to develop the possibility of printing a needle guide for an ultrasound probe using 3Dprinting technology. Materials and methods. Using a handheld 3D laser scanner, a metal needle guide for the convex ultrasound probe was scanned, the necessary changes were made for use, and a plastic needle guide was created using 3D printing. Results. The needle, when passed into the grooves with preset angles of the printed needle guide, exactly matches the dotted aim on the monitor of the ultrasound machine, similar to the original probe. Conclusion. Chronic pyelonephritis is the most common kidney disease. In accordance with the chronic nature of the infection, an important aspect of the treatment of recurrent uncomplicated urinary tract infections is antibiotic resistance. The aim of this work is to evaluate dynamics of the number of relapses and severity of symptoms of chronic pyelonephritis against the background of the complex drug «Nephrobac»®. Materials and methods. The study included 60 patients aged 24 to 56 years with complaints of pain in the lumbar area, fever, and dysuria. Based on the examination, the patients were diagnosed with chronic pyelonephritis. The patients were randomized into two groups: the first group received only antibacterial therapy, the second group received antibacterial therapy in combination with the biologically active complex «Nephrobac»® 1 capsule 2 times a day for 1 month. Patients of both groups were observed for 6 months from the start of therapy. Results. Fewer cases of leukocyturia were observed in the group receiving the antibiotic in combination with «Nephrobac»®, 45% and 75% after 3 and 6 months, respectively. In addition, in the same group, there were significantly fewer patients with bacteriuria after 3 visits in the main group compared with the control group receiving antibiotic therapy. There were no relapses registered 6 months after the start of the study in the group receiving combination therapy, including «Nephrobac»®, there were three patients with 1 relapse each in the control group. Over the past decade, there has been a progressive increase in the number of patients with chronic kidney disease (CKD). Early diagnosis of renal insufficiency allows to prevent irreversible consequences leading to disability and significant medical and economic costs. Purpose: to analyze modern markers for determining the functional ability of the kidneys. Materials and methods. A search was carried out in PubMed and Library, according to the keywords creatinine, urea, glomerular filtration rate (GFR), clearance, formulas for calculating creatinine clearance, cystatin C, β2-microglobulin, β-bound protein, NGAL, KIM-1, chronic kidney disease, chronic renal failure, urolithiasis. We found 389 sources no older than 10 years (published after 2012) that were relevant to the topic of the review. 80 articles were selected directly for citation in the review. Results. GFR assessment and detection of albuminuria are the most effective methods, but biomarkers used to calculate GFR are subject to a number of factors and do not allow assessing kidney function in real time. For early prediction of acute renal injury, the most suitable biomarkers are considered to be: NGAL and KIM-1 and IL-18. Equations combining cystatin C and creatinine r work better than equations using only cystatin C or creatinine, especially in situations where it is necessary to confirm chronic kidney disease. The combination of creatinine, cystatin C and the ratio of urine albumin to creatinine improves the stratification of the risk of progression of kidney disease and mortality. The latest developments in biomarkers allow us to hope that in the near future the assessment of the functional ability of the kidneys will be carried out in real time and will become a general clinical practice of a modern doctor. Conclusion. Modern biomarkers of the functional ability of the kidneys allow not only to evaluate the function, but also to screen for diseases, conduct differential diagnosis, and evaluate the effectiveness of the treatment.
