Although both anomalies clinically appear to be similar, with the formation of an unusually wide tooth, the distinction between the two anomalies is always a challenge for practitioners. Both gemination and fusion are found in both dentitions, although a higher prevalence, 0.5% -2.5%, was observed in the primary dentition. The teeth in the anterior region are more commonly affected, with a higher incidence in the lower jaw and an equal predilection for sex. {"title":"Toxic osteomyelitis of the jaws. The statistical study based on the data of the Institute of Emergency Medicine archive and current literature sources, indicates a tendency to increase in the last 15 years the frequency of toxic osteomyelitis of the jaws in drug users. The late addressing of patients, determined by the psycho–emotional and social aspects, creates difficulties in their treatment and in their subsequent rehabilitation. From the same aspects, it is difficult to monitor in time the results of the treatment, which often requires repeated interventions at the level of soft tissues, bone, teeth for morpho– functional rehabilitation. The causes of this condition can be as follows: supernumerary teeth, insufficient space, traumas, etc. Purpose: To search for ways to increase the effectiveness of the treatment of canine impaction by modernizing the appliances for moving the impacted maxillary canines in the dental arch. Material and methods. The work was performed based on the analysis of data of 28 patients aged 15–24 years with the diagnosis of impacted maxillary canine. The treatment was performed with fixed MBT appliances, .022 slot. For the extrusion of the impacted tooth it was proposed to use the closed coil spring. Results. The causes of the maxillary canine impaction were as follows: supernumerary teeth –3 cases; persistence of the temporary canine– 5 cases, anodontia of the lateral incisor — 4 cases; size anomalies of the lateral incisor — 5 cases; undetermined causes — 7 patients. The higher rate of the complications of the deep dental decay treatment motivates the creation of new remedies and algorithms of work, which will ensure the formation of a tertiary dentin pontoon in order to isolate the pulpal body from decay cavity. {"title":"Guided bone regeneration through autogenous laminated bone blocks","authors":"Adelin Bolea, A. Mostovei","doi":"10.53530/1857-1328.21.60.01","DOIUrl":"https://doi.org/10.53530/1857-1328.21.60.01","url":null,"abstract":"Implant–prosthetic rehabilitation of edentulous patients with various forms of bone atrophy is a current challenge in oral implantology. The aim of the study is to evaluate the bone volume obtained after grafting procedures using cortical blocks according to the Khoury technique. Material and methods: The study was focused on 10 patients with atrophies of the alveolar ridges that required procedures to create the bone supply for implant– prosthetic rehabilitation. The latter were performed using the Khoury technique, using laminated blocks installed at a distance from the recipient area, filling the space created with autogenous bone (50% of cases) and in combination with xenograft (50% of cases). Following the analysis of the data calculated on CBCT, the presence of a bone ridge with a thickness of 4.54±0.28mm (at 1mm apical from the top of the ridge) and 6.5±0.4mm (at a depth of 4mm) was determined. In 8 out of 10 patients, the implants were inserted in the same session, due to the possibility of fixing them in the apical part. Results: Following the surgery, the obtained thickness of the ridge was assessed at the same levels determined preoperatively, which constituted on average 9.5±0.26mm (at 1mm subcrestal) and 10.85±0.29mm (at 4mm depth) . At the end of the healing period, at the same levels the width of the ridge decreased by 0.51±0.14mm and 0.63±0.17mm (p> 0.05), and was obtained 8.99±0.28mm and 10.22± 0.26mm. Conclusions: Following the analyzed data, the creation of bone supply through autogenous blocks (Khoury technique) allows to obtain predictable results and a suitable bone bed for the insertion of dental implants. OMM represents about 1% of all melanomas and approximately 0.5% of all oral malignancies. In the case study presented, is a 68‑year‑old female patient, which examination findings were pigmented lesion growth involving the palate and alveolar ridge of maxilla, the histopathological analysis diagnosed malignant melanoma. The following objectives were set: 1. To determine the assessment of the clinical parameters of the face during the treatment of Angle class II Malocclusion with Clark Twin Blocks. 2. To study the degree of correct occlusal relation after the functional treatment of Angle class II malocclusion. 3. To assess changes of cephalometric indices in patients with mandibular retrognathia treated by this method. The study is based on the analysis and assessment of clinical and paraclinical parameters of 14 patients with Angle class II malocclusion, who were subject to orthodontic treatment using Clark Twin blocks. Patients` age ranged from 8 to 12 years. The following diagnostic methods were used: clinical examination, biometric model study (Pont, Korkhaus methods), and radiological examination (orthopantomography, teleradiography). Sagittal occlusal relation improved in all patients receiving treatment. Expansion contributed to an improved tortoposition of the front teeth in 10 patients and correct relation of the lateral teeth in the transverse plane in 3 patients. Asymmetric expansion was performed in 2 cases. In all patients in the study, the clinical parameters of the face, as well as the frontal and lateral occlusal relation in the three planes of space improved significantly. Coronectomy of the wisdom teeth is an alternative surgical procedure to tooth extraction, which aims to preserve the roots of the wisdom teeth in the dental socket after surgical separation of the crown, in order to avoid the injury of the inferior alveolar vascular-nervous bundle. Purpose. Minimizing the damage to the inferior alveolar nerve when extracting lower third molars caused by the intimate relationship between the nerve and the roots of the teeth, by using the technique of coronectomy, or intentional root retention. Material and Methods. Patients who reported to University Dental clinic during the period 2020–2021 for surgical removal of impacted L3M were screened for nerve–root relationship with OPG or CBCT. 15 patients underwent coronectomy as a procedure to remove the crown and upper third of the roots of a lower third molar to reduce the risk of damage to the inferior alveolar nerve. Results. 15 patients were enrolled in this study, with a total of 17 lower third molars. Sixteen sites healed primarily, but in 1 case the sockets on both sides opened and failed to close secondarily. In this case, the root fragments were later removed and found to be mobile. Conclusion. Coronectomies are safer to perform than complete extractions in situations in which the third molar is in close proximity to the mandibular canal. They appear as a result of the disruption of odontogenesis, the main sources being the dental lamina with its remains and the extensions of the basal layer of the oral covering epithelium. It is characterized by a potential aggressive, infiltrative or diffuse behavior. Due to the fact that the condition is associated with an asymptomatic clinical picture, it is diagnosed late, when the formation is in a phase of chronic evolution, reaching considerable dimensions. Thus, there is a dilemma among specialists regarding the choice of the optimal treatment method, addressing the need to combine surgical-conservative methods with surgical methods that have a radical character, such as mandibular resection. The aim of the study is the early diagnosis of odontogenic keratocyst and the treatment of the disease in a minimally invasive way. The study examined a patient who was diagnosed with odontogenic keratocyst and treated by the cystectomy method. Usually, free hand surgery does not allow in all the cases to place implants in best position from the prosthetic and biomechanical point of view. Due to the development of scanning technologies and three–dimensional printing, the planning of final results allows the printing of surgical guides, to ensure the most accurate position of dental implants [1,2]. The comparative analysis of conventional and guided implant placement methods has demonstrated significant advantages in favor of guided surgery.
