Forty-seven patients received a total of 51 cross-arch prostheses (27 maxillary and 24 mandibular prostheses), supported by 302 implants. Comprehensive clinical and radiographic records were available over a follow-up period ranging from 5 to 13 years. A strict inclusion criteria ensured the use of screw-retained implants and monolithic zirconia frameworks fabricated using standardised CAD/CAM protocols, without cemented titanium bases. Exclusion criteria included systemic conditions affecting healing, bruxism, uncontrolled periodontitis, smoking, and significant health changes during the follow-up period. Descriptive statistics summarised implant and prosthesis outcomes, while complications were evaluated for peri-implantitis at an implant level and framework fractures at a prosthesis level. Peri-implantitis was identified through clinical signs, including bleeding on probing, suppuration, and radiographic evidence of bone loss. Prosthetic outcomes were classified using the modified USPHS criteria. Mixed-effects Cox regression models were applied to analyse risk factors. Hazard ratios were calculated for peri-implantitis and framework fractures, with statistical significance set at p < 0.05. The implant survival rate was 97.64%, with peri-implantitis observed in 27 implants, predominantly in the mandible, resulting in an overall implant success rate of 91.06%. Prosthesis survival was 82.35%, with nine framework fractures reported, eight of which occurred in mandibular prostheses. The mandible was identified as a significant risk factor for both framework fractures (HR = 11.64, p = 0.024) and peri-implantitis (HR = 10.88, p = 0.003). IFCDPs with monolithic zirconia-based frameworks exhibited favourable clinical outcomes over a 5–13-year period. A scoping review was conducted. Individual search strategies were developed for each of the bibliographic databases (Cochrane, Embase, LILACS, LIVIVO, Pubmed, Scopus, PsyINFO, Web of Science), and in the gray literature (Google Scholar, Open Gray), comprehensively, without restrictions on language, publication data, or level of socioeconomic development of the country in which the study was conducted. The search was conducted on April 21, 2021 and the updates on May 24, 2024. Randomized controlled studies conducted in patients up to 18 years of age were included. Variables collected: characteristics of the included studies, topical anesthetics evaluated, time and the mode of application. Initially, 8579 studies were retrieved. Then 3127 were identified as eligible, of which (n = 3127), 52 studies were included in the narrative synthesis. The most commonly observed anesthetic methods with the most favorable outcomes were lidocaine, benzocaine, ice cooling, and EMLA. Spray, gel or patch were used, with application times ranging from 1 to 10 min. Mucosal cooling an ice stick was analyzed by eight studies. Only one showed a smaller reduction in pain perception scores than others. Cooling is a technique with numerous clinical advantages from the clinical point of view, for reduction of pain/discomfort prior to needle puncture of dental local anesthesia in children and adolescents. It is an accessible, inexpensive and pleasant method that does not resemble needles or unpleasant tastes, nor involves systemic toxicity and complications that may be associated with others anesthetics agents. Data extraction procedure and quality appraisal of this umbrella review adhered to the JBI critical appraisal checklist. Systematic reviews of RCTs in which schoolchildren of 5 to 18 years of age received oral health education in a classroom-based school setting from dentists, schoolteachers, or caregivers were included. Key outcome measures included distinct oral health indices, as well as knowledge, attitudes, and practices.</p><p><strong>Results: </strong>A total of 27 relevant systematic reviews were included for the umbrella review. Methodological quality was substantial among most systematic reviews, thus enhancing the strength of reliability of findings. Nineteen were graded excellent (scores 30 to 33), four were classified as good quality (scores 26 to 29), four rated as fair quality (scores 21 to 25) and none rated as poor-quality (scores 20 or below). Numerous studies from various nations were identified, with reviews peaking in 2019 and 2021. The overlap assessment reveals slight degree of overlap (3.73% overlap, -0.45% CCA-corrected covered area). Meta-analyses show that oral health education interventions had a favorable impact on a number of outcomes, such as reduced plaque and gingivitis, enhanced knowledge, attitude, and behavior, and improved oral cleanliness.</p><p><strong>Conclusion(s): </strong>This umbrella review provides a comprehensive synthesis of the effectiveness of oral health education interventions in school settings. Positive outcomes in plaque reduction and gingivitis prevention affirm the benefits of these programs, though the evidence for caries prevention remains inconclusive. The findings highlight the broader impact of these interventions on students' knowledge, attitudes, and behaviors, emphasizing the need for tailored approaches and further research to optimize oral health education. Parallel mixed methods study. Focus groups of patients with documented penicillin allergies and healthcare worker targeted questionnaires were used in gathering feedback on the PARTI tool’s design and functionality. Transcriptions of the focus group discussions and free text answers from the questionnaires were coded for analysis using a modified grounded theory approach. A total of 15 patients engaged in the focus groups and 50 healthcare workers responded to the questionnaire. Mean age of the patients was 52 years and 53.3% were female. Most patients (73.3%) had visited a dentist in the preceding year and 86.6% were asked about drug allergies. Healthcare worker respondents included 30% pharmacists and dentists, dental hygienists, and dental assistants (28%). Feedback from patients and healthcare workers on the PARTI tool was constructive and suggestions were made for improvement. Many healthcare workers (68%) highlighted the importance of the section on penicillin allergy testing in the PARTI tool. Feedback from the participants was incorporated into the final PARTI tool. Eligibility criteria were based on the PICOS framework, focusing on randomized clinical trials and clinical studies involving pediatric patients undergoing dental procedures with local anesthesia. The interventions included TENS, vibrotactile devices, or LLLT. Pain and anxiety were assessed using validated scales such as the Wong-Baker Faces Pain Rating Scale and physiological measures like heart rate.</p><p><strong>Results: </strong>Nineteen studies with a total of 1094 pediatric patients aged 4-12 were included. TENS and vibrotactile devices consistently showed significant reductions in pain and anxiety compared to standard methods. LLLT results were more variable, with some studies reporting significant pain reduction while others showed no significant difference. Meta-analysis was not possible due to heterogeneity among the studies. Overall, TENS and vibrotactile devices were more effective in alleviating injection-related pain, especially when combined with cold applications or topical anesthetics.</p><p><strong>Discussion: </strong>The findings highlight the potential of these auxillary aids as non-invasive and effective adjuncts for reducing pain and anxiety in pediatric dental procedures. LLLT showed mixed outcomes, likely due to variability in application protocols, suggesting a need for standardized methods. While all interventions demonstrated promise, inconsistencies in study designs and subjective measures limit the generalizability of the results.</p><p><strong>Conclusions: </strong>TENS and vibrotactile devices are effective auxiliary aids for reducing pain and anxiety during dental local anesthesia in children. Gray literature and reference lists of included studies were also examined. The guiding question of the study was formulated using the PECO strategy (Population, Exposure, Comparator, Outcome): P: primary teeth submitted to pulpectomy; E: measurement of working length using the radiographic methods; C: measurement of working length using electronic apex locator; O: difference in working length. The methodological quality (QUADAS-2) and certainty of the evidence (GRADE) were assessed. Meta-analyses were performed, e foram considerados os tipos de radiografia, grupo de dentes, geração de localizadores apicais. I² statistics were calculated.</p><p><strong>Results: </strong>Twenty-five studies were included in the review. Of these, eighteen studies were eligible for meta-analysis. Most studies demonstrated fair methodological quality. The working length measured using CR was, on average, 0.52 mm greater when compared to EAL (95% CI: 0.15-0.89, I<sup>2</sup> = 75%). When comparing EAL and DR, no statistically significant difference was found (MD = 0.06 mm, 95% CI: -0.12 to 0.24; I2 = 0%). The majority of articles (n = 20) were considered to have fair quality and five had high quality.</p><p><strong>Conclusions: </strong>The findings of the present review demonstrate a difference in the measurement of working length using CR in comparison with EAL. No difference was found between the EAL and DR methods. {"title":"Efficacy of different adhesive systems in bonding direct resin composite restorations: a systematic review and meta-analysis.","authors":"Ravinder S Saini, Rajesh Vyas, Sunil Kumar Vaddamanu, Syed Altafuddin Quadri, Seyed Ali Mosaddad, Artak Heboyan","doi":"10.1038/s41432-024-01095-3","DOIUrl":"https://doi.org/10.1038/s41432-024-01095-3","url":null,"abstract":"<p><strong>Objective: </strong>This study aimed to identify and evaluate scholarly research on the efficacy, durability, and long-term stability of various adhesive systems used for bonding direct resin composite restorations and to identify factors influencing bonding performance, such as adhesive composition, application protocol, substrate type, and etching technique.</p><p><strong>Materials and methods: </strong>An all-inclusive electronic database search for peer-reviewed scholarly journal articles was conducted using PubMed, ScienceDirect, Google Scholar, Dimensions, and the Cochrane Library for research articles investigating the effectiveness of different adhesive systems in direct resin composite restorations, excluding reviews, meta-analyses, opinion pieces, and case reports. The risk of bias was assessed using the Cochrane tool (Robvis 2.0). Data were extracted and thematically analyzed, and quantitative data were statistically analyzed using Review Manager software. Dichotomous data were evaluated using the Mantel-Haenszel method, applying a random effects model and 95% confidence interval.</p><p><strong>Results: </strong>The database search yielded 1632 potential articles, of which 14 were included in this study. Several adhesive systems exhibit excellent tensile bond strength. In addition, the type of resin composite, adhesive system, and artificial aging significantly influenced the bonding performance of resin composite restorations. However, the failure rates were low in the different adhesive systems, showing comparable results between the two-step etch-and-rinse and one-step self-etch adhesive systems (p = 0.71). A comprehensive search of MEDLINE, EMBASE, Cochrane Central Register of Controlled Trials, and Dentistry and Oral Sciences databases for randomized controlled trials (RCTs) up to February 2023 was conducted. The inclusion criteria were studies involving adult patients treated with SLA and SLActive implants, with assessment of implant stability through ISQ levels up to 12 weeks post-placement. From the initial 180 potentially eligible publications identified, six RCTs were included in our analysis, comprising 326 implants (50.6% SLA and 49.4% SLActive). Three studies were classified as low risk, while three had an unclear risk of bias. Overall, SLActive implants demonstrated comparable stability levels, as measured by ISQ, to SLA implants within the 12-week interval for implants placed in the maxillary or mandibular region. However, findings from the RCTs suggest that the SLActive surface led to an earlier transition point, a faster return to stability levels, and higher ISQ values at the end of 12 weeks compared to the SLA surface for implants placed in the palatal region. SLActive surfaces exhibited stability levels similar to SLA surfaces for maxillary and mandibular implants. Notably, for palatal implants, SLActive resulted in a quicker transition point and higher stability levels at the 12-week mark.
