The authors concluded that: “acute hospital induced hyponatraemia is caused by electrolyte-free fluids, or ‘volumetric overload type1 (VO1)’, in the presence of elevated vasopressin. The condition affects men, women and children, particularly postoperative patients. This is a rare condition that can present with massive hematochezia that carries a high degree of morbidity and mortality. Classically, AEF are categorized in primary versus secondary. Primary AEF are typically a result of trauma, radiation, tumor invasion, or an atherosclerotic aneurysm. A secondary AEF develops as a repercussion of surgical intervention [2]. Renal Sympathetic denervation and Nephropexy (RSD & N) Surgery has proved curable for the condition. Patient and Methods: The reported case in which IVU with erect film shows the IVU7 sign is measure of renal pedicle stretch. Retrograde Pyelography (RGP) showed the internal renal damage while all ancillary imaging were normal. The surgery of RSD & N was used for treatment of the condition. Results: An IVU with erect film and RGP detected the diagnosis and the renal pathology while no other imaging could. Comparing IVU supine and erect revealed the IVU7 sign which is a measure of renal pedicle stretch causing artery stenosis and ischaemia. The findings on RGP show erosion of renal papillae and leakage of contrast medium into renal veins which is the sie of hamaturia. The surgery of RSD & N cured the condition. Conclusion: An IVU with erect film and RGP clinched the diagnosis and revealed the renal pathology while no other imaging could. It should be essential diagnostic investigation in every LPHS case. The internal renal damage causing pain and haematuria is best shown on RGP (Figure 3). Unfortunately no other investigation including CT and MRI is capable of revealing the pathology of LPHS complicating SN. Upright IVU is currently undone and has not been reported previously in LPHS. Retrograde pyelography findings have not previously been documented in either condition (Figure 3). The use of IVU started early in the 20th century while clinical evidence on the genuineness of SN pain dated back to the 15th century. Loin pain hematuria syndrome was reported in 1967 while Dietl’s crisis is known for centuries [5]. Organic reno-vascular complications demonstrated on conventional arteriography of SN and LPHS are of advanced cases. These congenital anomalies pose a definite risk of biliary injury during operative interventions like laparoscopic cholecystectomy. Precise diagnosis pre-operatively is required for precise plan of management and to make sure both the gallbladders are removed completely without complications. As these anomalies are frequently associated with stone formation due to inadequate bile drainage. Laparoscopic cholecystectomy is the treatment of choice. Although there is currently no consensus about the treatment of these infections, it is generally recognized that bacterial necrotizing dermohypodermitis and necrotizing fasciitis (BNDH-NF) are serious infections that may be life-threatening and required emergency medico-surgical treatment. Epidemiologic data about these serious infections are nearly inexistent. In occidental countries, BNDH-NF are rare with incidence of 1 case for 1,00,000. In sub-Saharan Africa, studies on BNDH are rare and focus on non-necrotizing infections in hospital medical ward in the big cities (capitals). Data are nearly inexistent in secondary cities where the people have rural activities. Heterotaxy syndrome is a rare congenital condition that occurs from failure of intra-abdominal and/or intrathoracic organs to rotate correctly during embryologic development, resulting in abnormal arrangement of the organs across the left-right axis of the body [1]. It has two broad subcategories which include polysplenia and asplenia [1]. As most of patients remain asymptomatic; the prevalence of this anomaly remains underestimated, although one study has suggested that 1 in every 2500 people may have heterotaxy syndrome with polysplenia [2]. This condition is mostly detected incidentally in the adults during multi-detector computed tomography scanning for diagnosis of other pathologically related or unrelated complaints as in this reported case [3,4]. Herein authors report unusual anatomical findings that posed many operative challenges during a laparoscopic repair of a hiatus hernia in an adult patient with heterotaxy syndrome. Preoperative radiological evaluation and intra-operative appreciation of the anomalies with considering an alternative feasible technique allowed authors to achieve a safe outcome.
