Therapy is based on propylthiouracil (PTU), starting from the first trimester of pregnancy. Among the complications of the antithyroid drugs (ATDs) there is the risk of hypothyroidism and fetal goiter, in addition to well - known possible teratogen effects. Maternal hormonal monitoring with a personal ized pharmacological treatment and fetal ultrasound control, starting from the 18 th week of pregnancy, are mandatory for the mother and fetus’ health and therefore for a successful pregnancy. According to the WHO, two thirds of these deaths can be prevented with simple and effective interventions. The HBB (Helping Babies Breathe) program provides effective care for asphyxiant newborns regardless of the level of care. Objective: The objective of our work was to assess the knowledge of neonatal resuscitation equipment and procedures according to the HBB program by qualified nursing staff at the level of referral health structures in the Sikasso region of Mali. Methodology: This is a multicenter cross-sectional survey over a period of 4 months. It was carried out in two maternities and two referral neonatal units in Sikasso. The target population studied was made up of the nursing staff involved in the reception and care of newborns in the delivery room, in postpartum suites and in neonatal departments. Results: Eighty percent (112/141) of targeted caregivers were interviewed. More than half (44%) had more than 6 years of experience in neonatal care. Only 27% had received training in neonatal resuscitation including HBB. The most cited materials used for resuscitation were the mucus aspirator (82%), the ventilation bag (67%) and the heated table (60%). The bestknown resuscitation procedures were airway aspiration (82%), mask ventilation (60%), and hypothermia control (48%). Conclusion: Knowledge of newborn resuscitation remains very insufficient in the reference structures in Sikasso. The incidence of urinary tract infection of fungal etiology has increased notably. Yeasts are isolated in 7 to 8% of urine cultures. Different species of Candida cause urinary infection, with Candida albicans being the most frequent, followed by Candida tropical. The majority favoured by invasive devices, many of them requiring intensive care, as well as the use of broad spectrum antibiotics for prolonged periods, within these devices are by far the permanent urinary tract catheter. Next we present 3 cases diagnosed in the Hospital Civil de Guadalajara Fray Antonio Alcalde in pediatric patients in the pediatric infectious disease service. They are not quite frequent lesions, but due to all of the above and having better imaging techniques, they have been diagnosed more frequently. Role of ultrasound in early identification of dengue and its complications along with clinical and laboratory investigations. Ultrasound was performed on 112 patients (1 – 14 years) referred from dept. of paediatrics for ultrasound with features of dengu e and its complication and later confirmed the diagnosis with serology proven. 30 patients were serologically negative for dengue fever hence excluded from the study. Out of 92 serologically positive cases 31 cases visited hospital at early stage and under gone ultrasound during 2/3 rd day and 5/7 th day of fever. During 2/3 rd day ultrasound showed as almost all showed gall bladder wall thickening and pericholecystic fluid (in different patterns), 32.5% had hepatomegaly, 16.12% had splenomegaly and 19.35% righ t minimal pleural effusion, 3.25% left plural effusion. Follow - up ultrasound on fifth to seventh day revealed ascites in 93.5%, hepatomegaly in 90.3%, right plural effusion 80.6%, left pleural effusion in 41.9% and pericardial effusion in 29%. Of the 61 pa tients who underwent the study on fifth to seventh day of fever for the first time almost all had gall bladder wall thickening, 95.0%had hepatomegaly, 27.8% had splenomegaly, 96.7% had ascites, 95.08% had right pleural effusion, 75.4% had left pleural effu sion and 22.9% had pericardial fluid. This pathology is rare in children under 2 years old. The present study aims to present the case of a one-year-old child who developed complicated suppurative appendicitis. Despite the efforts, the patient evolved to death with 15 days. Suppurative acute appendicitis is one of the differential diagnosis of acute abdomen in infants. The diagnosis is difficult due to the non-specific signs. Early intervention and proper fluid replacement may lessen this risk. Acute gastroenteritis (AGE) is an infection of the gastrointestinal tract that commonly affects young children. It causes an increase in loss of water and electrolytes due to vomiting and diarrhea resulting to dehydration, which is the main cause of mortality in children under five years with acute gastroenteritis. Therefore, this study was designed to find out the incidence of AGE and the status of associated electrolyte derangements, renal involvement and other biochemical and haematological variables. Material and Methods: This cross sectional study comprised of 100 cases of AGE in children aged <5 years of age. Demographic profile of the patients, details of diarrhea, and accompanying degree of dehydration (defined as per World health Organization criteria) with any associated complication were included. Various electrolyte parameters along with biochemical and hematological parameters (serum urea, serum creatinine, hemoglobin, total leukocyte count) were investigated. Results: This study found that urea and creatinine were elevated in 29(29%) and 62(62%) of the cases respectively. Electrolyte disturbances were seen in 42% of cases among which hyponatremia was the most common electrolyte disturbance seen. Majority of patients had isonatremic dehydration 58%. Hyponatremic dehydration was most common in some dehydration, which was 30%. We found hypokalemia in 31% cases. Over a period of 10 years we had two cases successfully treated in our departments. Cases: It was a girl and a boy, vaginely delivered in the same year. The pregnancy was well followed in the first case and badly in the second. APGAR score at birth, at 2700 g girl was 10-10-10, and at 2900 g boy at 7-7-8 after resuscitation. On admission, newborns were cyanotic, tachycardic and tachypnotic; there was abdominal distension with diffuse abdominal tympanism, occurring on the second day, and scrotal swelling in addition for the boy. The abdominal X-rays objectified a massive pneumoperitoneum in the shape of a “saddle horse”. Laparotomy found gastric perforation in the area of parietal hypoplasia in both cases. An excision suture had been performed. The postoperative course was simple. Conclusion: Spontaneous neonatal gastric perforation is rare. However, there are still small groups of people who delay or reject immunizations causing isolated cases or outbreaks of preventable diseases such as measles or tetanus. In Peru, the National Vaccination Scheme is mandatory in all health facilities and starts within the first hours of life, when the child does not yet have the capacity to make decisions for himself, therefore, the responsibility for their heal th lies with the parents or guardians. Objective : To report a clinical case of an under - aged patient with generalized tetanus, with a history of incomplete vaccination, emphasizing parental refusal to vaccination and medical care leading to the child’s de cease. Case Report : Three - year - old female patient, with parents of low socioeconomic and educational status, who reported incomplete vaccination scheme (only received BCG and HVB newborn vaccines) due to parents’ rejection. The patient presented an unquan tified thermal rise sensation, facial paralysis, trismus, hyperextension of both lower limbs in supine position, and hypertonicity. She was treated in Víctor Lazarte Hospital and Virgen de la Puerta Hospital for 9 days, presenting little improvement with t he support treatment. The parents requested voluntary discharge and while returning home the patient dies. In the following days the family refused to vaccinate their other children.
