Lipoprotein (a) (Lp(a)) is a predictor of many forms of vascular disease, and may serve as a tool in identifying subjects at risk. Despite accumulat- ing evidence from cross sectional and prospective studies, its role in the development of stroke is still in doubt. Methodo- logical issues regarding measurement remain to be resolved. Whether reductions in Lp(a) levels will result in a reduction of vascular events, including stroke, remains to be established. Aggressive management of established cardiovascular risk factors is advocated in subjects exhibiting elevated serum Lp(a) concentrations.","PeriodicalId":112973,"journal":{"name":"The Open Clinical Chemistry Journal","volume":"15 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2010-04-08","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"133692381","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

During the last years several markers have been studied as potential predictors of CV events. Hemostatic and thrombotic factors such as fibrinogen, homocysteine, factor VII, von Willebrand, tissue plasminogen activator, plasminogen activator inhibitor - 1, D-dimer and lipoprotein ( ) have been found to be closely related to CVD. Many epidemiological studies have indicated that raised levels of these factors are associated with higher incidence of CVD, while some others have yielded conflicting results. As a consequence, it remains obscure whether they contribute to prediction of future CV events on top of conventional risk factors. Although data suggest that most of them are somehow implicated in CVD pathophysiology, it is not clearly de- fined yet whether treatment of abnormal levels leads to CV risk reduction. As a result measurement and treatment of these factors are justified only in exceptional cases. Further investigation is required in order to conclude whether and which of these factors may claim a position in everyday clinical practice.","PeriodicalId":112973,"journal":{"name":"The Open Clinical Chemistry Journal","volume":"3 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2010-04-08","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"130642704","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Because heparin is a common component of blood col- lection tubes, we tested the effect of OSCS on the laboratory analysis of plasma. Blood from healthy volunteers (N=50) was collected into tubes containing various mixtures of heparin and OSCS. Samples were inspected for microclots and were analyzed for a panel of 28 routine laboratory tests. No microclots were observed in tubes that contained only heparin but were detected in 18%, 88% and 76% of plasma samples containing 5%, 15%, 20% OSCS (%weight relative to hepa- rin), respectively. OSCS at the highest dose (20%) caused a systematic bias for the following 6 tests: Lactate Dehydro- genase: 18% (12% to 24%); Triiodothyronine: -5.7% (-8.1% to -3.3%); Potassium: -2.8% (-4.2% to -1.4%); Total Protein: 2.5% (1.4% to 3.6%); Chloride: -1.4% (-1.8% to -1.0%) and Uric Acid: 1% (0.5% to 1.4%). We attempt to use a combination of tests to reduce the need for echocardiography without missing any cases of CHF. We compared 50 CHF patients with an ejection fraction of less than 60%, to 100 patients with non-specific symptoms but with a normal echocardiogram. After setting sensitivity at 100%, specificities for BNP, amino-terminal probrain natri- uretic peptide (NT-pro BNP), and homocysteine were 34%, 28% and 36% respectively. Combinations of two tests if ho- mocysteine was included increased the specificity to 56%. The AUC for BNP and NT-proBNP were not significantly dif- ferent (92.8 and 95.9, respectively) whereas the AUC for BNP and NT-proBNP tests were superior to homocysteine (83.2). Compared with performing echocardiography in all patients, savings were dependent on the pretest probability. At a 1% pretest probability of CHF, savings were 44.5% while using a combination of NT-proBNP with homocysteine test. Homocysteine can be used in combination with either of the BNPs to decrease costs of early detection of CHF.","PeriodicalId":112973,"journal":{"name":"The Open Clinical Chemistry Journal","volume":"107 4","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2010-02-04","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"120983202","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Dietary Met may undergo transmethylation processes to release homocysteine (Hcy) which may either be regenerated to Met following remethylation (RM) pathways or catabolized along the transsul- furation (TS) cascade. The activity of enzymes governing RM and TS pathways is depending on pyridoxine, folate and cobalamin bioavailability. Dietary restriction in any of these watersoluble B-vitamins may lead to hyperhomocysteinemia (HHcy) causing a panoply of cardiovascular disorders. Taken together, the vitamin triad only affords partial account of Hcy variance, prompting the search for additional causal factor(s). Body composition studies demonstrate that nitrogen (N) and sulfur (S) maintain tightly correlated concentrations in tissues of both healthy subjects and diseased patients. Any morbid condition characterized by insufficient N intake or assimilation, as seen in protein malnutrition or intestinal malabsorption, reduces body S accretion rates. Excessive urinary N-losses, as reported in acute or chronic inflammatory disorders, entail proportionate obligatory S-losses. As a result, lean body mass (LBM) undergoes downsizing and con- comitant depletion of N and S body stores which depresses the activity of cystathionine- -synthase, thereby promoting upstream accumulation of Hcy and overstimulation of RM processes. HHcy thus appears as the dark side of efforts devel- oped by S-deprived patients to safeguard Met homeostasis. Irrespective of vitamin-B status, Hcy values are negatively correlated with LBM shrinkage well identified by the serial measurement of plasma transthyretin (TTR). The S- deprivation theory fulfills the gap and allows full causal coverage of the metabolic anomaly, hence providing together with vitamin-deficiencies an unifying overview of the main nutritional determinants implicated in HHcy epidemiology.","PeriodicalId":112973,"journal":{"name":"The Open Clinical Chemistry Journal","volume":"40 5","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2010-01-04","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"120839986","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The aim of this study was to compare a direct high performance liquid chromatography (HPLC) method with an indirect enzymatic method to assess flavin status and establish reference intervals. The HPLC method was used to assess prevalence of flavin deficiency in total parenteral nutrition (TPN) patients. Method: Blood was obtained from 90 healthy volunteers and 51 TPN patients. Results for each method were compared us- ing Spearmans Rank correlation. Results from reference groups and TPN patients were compared using Mann-Whitney U tests. Results: Erythrocyte glutathione reductase activity coefficients (EGRAC) correlated with flavin levels measured by HPLC (p<0.001). HPLC was chosen for TPN samples analyses due to superior sensitivity. There was no significant difference between FAD and FMN levels in controls and TPN patients, although TPN patients had significantly higher levels of ribo- flavin than controls (p<0.001). Conclusion: HPLC is superior to the EGRAC assay for assessing flavin status. A previous study from our laboratory showed reduced seminal and plasma ascorbic acid in subfertile patients. Method: Fifteen subfertile male patients mean age 36.4±0.9 years with sperm counts of 31.4±7, 08 x10 6 /cm 3 were given 200mg/day dose of ascorbic acid as supplement for four weeks and ten highly fertile male mean age 35.6±1.7 years with sperm count of 50.9±10.3 x10 6 /cm 3 volunteers were selected as controls. The biochemical parameters and anthropometric measurements were determined using standard methods. Result: Significant decreases were obtained in baseline sperm cell count (p<0.01), percentage (%) motility and %vitality (p<0.01), and the % of morphologically normal sperm cells (p<0.01) in the subfertile compared with the controls. The baseline plasma and seminal ascorbic acid concentrations were significantly reduced (p<0.001). The post-supplementation sperm cell count (p<0.001), % motility and %vitality (p<0.01) were significantly increased compared with the baseline values. Conclusion: The enhanced reversal of poor semen quality and semen ascorbic acid status with dietary supplementation of vitamin C in subfertile is a pointer to its positive role in the maintenance of sperm cell integrity. The N-type calcium channel is associated with sympathetic nerve activation. This effect may improve the glome- rular hemodynamics in the injured nephron, and may mitigate the progression of renal injury. To clarify the effect of cil- nidipine in instances of already existing decreased renal blood flow, we examined the alteration of renal hemodynamics before and after cilnidipine therapy in patients with advanced chronic kidney disease (CKD). Cilnidipine was adminis- trated daily to 17 CKD patients with hypertension for 12 months. Another 16 patients were similarly administered am- lodipine during this study, a long-acting L-type calcium channel antagonist has also been shown to be renoprotective. Glomerular filtration rate (GFR), effective renal plasma flow (ERPF), and protein excretion in 24-hour accumulated urine were measured at the start and end of the study. The parameters of renal hemodynamics were calculated by Gomez's es- timation equation. Systolic blood pressure decreased to 80 % of the level at the beginning of the study, and ERPF in- creased to 127 % of the level at baseline. Glomerular capillary pressure on single nephron was reduced to 90 %, although total GFR decreased within the non-statistical change. Especially, renal vascular resistance ratio (RA/RE) on single neph- ron improved to 120 %. Cilnidipine improves ERPF and glomerular hypertension without worsening total renal function. N- and L-type calcium channel antagonist is effective and safe for patients with advanced CKD as a result of improve- ment of glomerular capillary resistance.","PeriodicalId":112973,"journal":{"name":"The Open Clinical Chemistry Journal","volume":"17 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2009-08-13","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"126470333","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
